Importantly, it argues that this urgent problem of justice is changeable. Sprague uses the capabilities approach to bring a theory of health justice, together with multiple sources of evidence, to investigate the complex problem of HIV and accompanying poor health outcomes in black South African women. Motivated by a concern for application of knowledge, this work discusses how to better conceptualise what health justice demands of state and society, and how to mobilise available evidence on health inequities in ways that compel greater state action to address problems of gender and health. HIV in women, and possible responses, are investigated on four distinct levels: conceptual, social structure, health systems, and law. The analysis demonstrates that this problem is indeed modifiable with long-term interventions and an enhanced state response targeted at multiple levels. It is estimated that more than 60 million people worldwide have lived with HIV/AIDS since the beginning of the epidemic and 20 million of these have died (UNAIDS 2002). HIV/AIDS now affects every country in the world. Despite advances made in knowledge about HIV prevention, the disease continues to spread. Globally, sub-Saharan Africa is the most severely affected, with the Southern African Development Community (SADC) being home to half of the estimated 24 million people living with HIV/AIDS in this region (UNAIDS 2000). Country level estimates of HIV infection are usually based on surveys of women attending antenatal clinics. In South Africa over the past decade, this has been the primary means of monitoring the spread of HIV. Antenatal surveillance systems provide countries with a low-cost tool for regularly monitoring key aspects of the HIV epidemic. For example, the data can be used to track the epidemic in different parts of a country or among a specific age cohort. The antenatal survey's major usefulness is to track trends of HIV infections over time.

Five major themes are explored in the book such as the need for a basic definition of "older person," the need for national governments to invest more in basic research and the coordination of data collection across countries, and the need for improved dialogue between local researchers and policy makers. Book excerpt: The authors report results from a randomized evaluation comparing three school-based HIV/AIDS interventions in Kenya: (1) training teachers in the Kenyan Government's HIV/AIDS-education curriculum; (2) encouraging students to debate the role of condoms and to write essays on how to protect themselves against HIV/AIDS; and (3) reducing the cost of education. Their primary measure of the effectiveness of these interventions is teenage childbearing, which is associated with unprotected sex. The authors also collected measures of knowledge, attitudes, and behavior regarding HIV/AIDS. After two years, girls in schools where teachers had been trained were more likely to be married in the event of a pregnancy. The program had little other impact on students' knowledge, attitudes, and behavior, or on the incidence of teen childbearing. The condom debates and essays increased practical knowledge and self-reported use of condoms without increasing self-reported sexual activity. Reducing the cost of education by paying for school uniforms reduced dropout rates, teen marriage, and childbearing.
