This phenomenal gradient as well as the demonstrated ability of gastric juice to digest tissues has motivated clinicians and investigators alike to emphasize acid secretion and acid ablation in studying the pathogenesis and therapy of peptic ulcer disease. Conse quently, over the past 150 years, we have made considerable progress in under standing the mechanisms and regulation of acid secretion by the stomach. Not surprisingly, therapy for both peptic disease and mucosal injury has also been predominantly directed at either neutralizing acid or suppressing its production. During the past 10 years, attention has been focused on factors other than acid in the genesis and therapy of ulcer disease. Work done worldwide demon strated that acid hypersecretion is not a common event in peptic ulcer disease. Therefore, we began realizing that factors other than acid secretion may be important in the genesis of ulcer disease or in gastroduodenal mucosal damage. In addition, new physiological information has established that the gas troduodenal mucosa is normally protected by a complex series of events includ ing mucus and bicarbonate secretion, cell renewal, surface mucosal restitution, and preservation of the microvasculature and mucosal proliferative zone.

The current knowledge is clearly incontestable that AEPF pent-up the aggressive issue, gastric acid secretion. The anti-ulcerogenic impact of the AEPF is also associated with its antisecretory action since acid may be a major consideration the event of ulceration. The current data also clearly demonstrated that the 400 mg/kg is more effective than the 200 mg/kg and 100mg/kg dose of AEPF and has shown increased pH and decreased total acidity of gastric fluid. The ulcerogenic effect of cysteamine-induced duodenal ulcers was developed in rats that received cysteamine HCl 400mg/kg. The exact mechanism of pathological process within the cysteamine-induced peptic ulcer model isn't totally known however but hypersecretion of gastric acid, deterioration of mucosal resistance and promotion of gastric emptying are among the possible mechanisms. In cold restraint stress induced ulcer model blood parameters such as Glucose, cholesterol and Triglycerides were estimated. The significant increase in blood sugar level was discovered because; beneath nerve-racking conditions, ductless gland secretes corticosterone in man and glucocorticoid in rats. AEPF significantly reduced the elevated serum cholesterol and triglycerides levels, which may be due to inhibition of stimulation of the sympathetic nervous system. Therefore it could act as a potent therapeutic agent against peptic ulcer disease.

Book excerpt: Over the past decade a great deal of research activity has occurred on either side of the gastroduodenal epithelial interface, with the common goal of elucidating the mechanisms of mucosal protection and how these may be compromised in peptic ulcer disease. A prime stimulus to such research has been the realization that abnormal acid secretion or its delivery into the duodenum in excessive amount cannot fully explain ulcer pathogenesis in the majority of patients. One may envisage the stomach and proximal duodenum as being in a dynamic equilibrium, with aggressive luminal factors, such as acid and pepsin, being counteracted by protective mucosal mechanisms. Until recently, the mechanisms involved in mucosal protection had been poorly defined while the physiology and pathophysiology of acid and pepsin secretion had been elucidated. The impression that gastroduodenal mucosa was protected by a single mechanism has at last been replaced by a more realistic view, suggesting a number of protective zones acting in series. Some of these zones may act as first or second line defences against aggressive factors while certain zones may specifically protect against certain aggressors.
