The report therefore represents a collective effort by local non-governmental organizations to monitor their own government spending on HIV/AIDS, and thus to act as a watchdog to ensure that government resource allocation decisions reflect vital public priorities. Book excerpt: HIV/AIDS has been one of the largest public health crises in recent history, and the U.S. federal government has spent hundreds of billions of dollars fighting the disease. This study examines the impact of federal funding allocated to U.S. cities through the Ryan White CARE Act, which is the largest program for combating HIV/AIDS in the United States. The empirical approach identifies the impact of the funding by studying funding variation that comes from Ryan White policy features that resulted in large funding differences among cities that were originally on parallel HIV/AIDS trajectories and finds that Ryan White's city-level funding has improved HIV/AIDS outcomes in the cities receiving the funds. The estimates indicate that one HIV/AIDS death has been avoided for every $314,000 allocated through the program and that the program has saved approximately 60,000 lives through 2018. The estimates also indicate that funding differences across cities have contributed to the uneven progress in combating HIV/AIDS across the United States.

The scale of the pandemic and the lack of funds needed to eradicate it require identifying key issues in field interventions and optimal economic policies to fund them. In developing countries, where the epidemic is reaching its peak, the magnitude of governmental and international interventions triggers major crowding-out effects on every other economic decision of those countries, and thus HIV/AIDS affects every aspect of social life. Economic policies alleviating crowding-out effects are thus paramount to foster the economic growth of developing countries and, in turn, their future welfare. Economic issues in the fight against HIV/AIDS are also a primary concern for developed countries, in charge not only of subsidising current treatment campaigns domestically but also of funding R&D in innovative treatments. Designing optimal incentives for public and private agencies to reduce the costs of available medicines, and to develop innovative treatments such as a therapeutic vaccine, is as important as drug delivery or any other field campaign to eventually eradicate the disease. Over two decades of practical implementation of economic policies and academic research have shown many pitfalls in current policies, and they have made it possible to identify previously missed issues. This book shall provide a recent and comprehensive coverage of those policies, and it shall analyse their economic efficiency as well as ways of improvement using state-of-the-art academic findings in Economics and Finance. The authors discuss in detail and provide new economic analyses on the following issues: The nation-wide and international economic consequences of the spread of the disease; Market incentives and disincentives to produce and to develop treatment technologies; The nature and optimality of economic policies devoted to fighting the disease in developing countries, as well as the enhancement of current policies through financial innovations.

[...] National Working Group The National Working Group, based mainly at the offices of the Department of Health, is responsible for assisting the provincial co-ordinators of the programme and facilitating the rollout of the integrated plan. [...] The national departments are the transferring agent for the provincial conditional grants: • For the VCT programme, a portion of the programme funds are retained by DoH for administration and the rest given to provincial health departments in conditional grants. [...] Although DoH is playing this extra role in the NIP of helping to co-ordinate the Life Skills and CHBC programmes, for the first three years of the NIP the DoE still receives the majority of the NIP funds because of the strong policy emphasis on prevention and education. [...] In the first two years of the programme-when each of the national departments were distributing the conditional grant monies based on a rationale fitting for each programme--the cumulative effect was that the provinces with the largest share of the national HIV-positive population were not receiving the corresponding share of total conditional grant funds.
