This is the title of a new campaign promoted by the International Agency for Prevention of Blindness to raise funds to help tackle avoidable loss of sight in poorly developed countries, truly an admirable initiative. This book could have used a similar leitmotiv: if you see what happens inside of a joint, you will be able to believe in your patient’s symptoms. But it would not be right. Arthroscopy is not out there just to make a diagnosis; it was not developed just to certify that the patient’s complaints are based on something physical. Arthroscopy was introduced to help patients, to make our treatments more reliable, to have better control of our p- cedures. It is merely a tool, indeed, but a marvelous one which nobody should und- score among all surgical options we have when it comes to solving wrist trauma. Seeing is understanding. This could be another leitmotiv for these authors’ c- paign to get more hand surgeons to incorporate arthroscopy in their practices. Certainly, mastering these newly developed techniques help understanding the patient’s problems. But again, that statement would also be misleading for not always what we see through the scope is the real cause of dysfunction. The enemy may be outside of the capsular enclosure. Indeed, arthroscopy provides lots of useful inf- mation, but the surgeon need not accept biased interpretations of the patient’s pr- lem based only on what appears on the screen.

In preparing a third edition, we found it necessary to revise the text extensively and partly restructure it. The reasons for this are numerous. Numerous advances have been achieved in functional anatomy, biomechanics, diagnosis, and treatment. There are, however, many controversial aspects to these problems. Many clinical and associated investigators from around the world have attempted to increase our knowledge of the wrist with enthusiastic and devoted studies. An international symposium was held at the Nagoya Castle Hotel, Nagoya, Japan from March 6th through March 8th, 1991 to further understanding and promote discussion of wrist problems among a representive international group. Approximately 300 participants from 16 different countries assembled for these discussions. This monograph consists of 40 selected papers based on presentations given at the international symposium. The topics are divided into six chapters: Functional Anatomy, Diagnosis and Basic Studies; Kienbock's Disease; Scaphoid Fracture, Distal Radius Fracture; Carpal Instability and Wrist Pain; and Wrist Arthroplasty. A number of unique observations as well as detailed surgical techniques were presented. These include topics such as the vascularity of the triangular fibrocartilage, radial wedge osteotomy and vascular bundle implantation into the lunate for Kienbock's disease, Herbert screw insertion though a minimal exposure for acute scaphoid fracture, and closing wedge oseotomy of the radial styloid for the early stage of the SLAC wrist. Additionally, mate- als that were initially used “off label” or that simply were not FDA approved have completed their governmental review and have received FDA approval. This has increased the legitimacy of the p- cedure from the legal and reimbursement perspective. Controversy over height restoration and device selection has become a progressively bigger issue over time. Kyphoplasty (balloon assisted vertebroplasty) has received tremendous emphasis. This book c- pares and contrasts data and claims that differentiate kyphoplasty from percutaneous vertebroplasty. We also look at other methods that pot- tially can be used for height restoration. New procedures that deal with bone augmentation in other anatomic regions have evolved (i.e., sacroplasty) and are discussed. As this re- lution in image-guided percutaneous bone augmentation has dev- oped, multiple medical specialties have embraced these procedures in their training programs for both residents and practicing physicians.
