Book excerpt: In the United States, people living in low-income neighborhoods frequently do not have access to affordable healthy food venues, such as supermarkets. Instead, those living in "food deserts" must rely on convenience stores and small neighborhood stores that offer few, if any, healthy food choices, such as fruits and vegetables. The Institute of Medicine (IOM) and National Research Council (NRC) convened a two-day workshop on January 26-27, 2009, to provide input into a Congressionally-mandated food deserts study by the U.S. Department of Agriculture's Economic Research Service. The workshop, summarized in this volume, provided a forum in which to discuss the public health effects of food deserts.

Book excerpt: Abstract: The study showed that about 184,000 low-income families participated in the Extension Service's Expanded Food and Nutrition Education Program (EFNEP) prior to October 1969. A sample showed that family incomes were very low--less than $2,700, of which more than a third was spent for food. Families with annual incomes of less than $1,200 per year spent nearly one-half for food. Most families were urban, members of minority groups, and had homemakers with relatively low educational levels. Food consumption practices of homemakers upon entering the program indicated that many families had poor diets. Foods in the milk, fruit and vegetable group were most often lacking in diets. After 6 months of EFNEP participation substantial, improvements in food knowledge and consumption practices were evident, particularly in the consumption of foods in the milk, fruit, and vegetable groups. Homemakers with poorest diets showed more improvement than those who had better initial food consumption practices.

Book excerpt: Several changes in the United States over the past two decades have implications for diet, nutrition, and food safety, including patterns of food consumption that have produced an increase in overweight and obese Americans and threats to food safety from pathogens and bioterrorism. The changes raise a number of critical policy and research questions: How do differences in food prices and availability or in households' time resources for shopping and food preparation affect what people consume and where they eat? How do factors outside of the household, such as the availability of stores and restaurants, food preparation technology, and food marketing and labeling policies, affect what people are consuming? What effects have food assistance programs had on the nutritional quality of diets and the health of those served by the programs? Where do people buy and consume food and how does food preparation affect food safety? To address these and related questions, the Economic Research Service (ERS) of the U.S. Department of Agriculture (USDA) asked the Committee on National Statistics to convene a panel of experts to provide advice for improving the data infrastructure on food consumption and nutrition. The panel was charged to review data needs to support research and decision making for food and nutrition policies and programs in USDA and to assess the adequacy of the current data infrastructure and recommend enhancements to improve it. The primary basis for the panel's deliberations, given limited resources, was a workshop on Enhancing the Data Infrastructure in Support of Food and Nutrition Programs, Research, and Decision Making, which the panel convened on May 27-28, 2004. This report is based on the discussions at the workshop and the deliberations of the panel. The report outlines key data that are needed to better address questions related to food consumption, diet, and health; discusses the available data and some limitations of those data; and offers recommendations for improvements in those data. The panel was charged to consider USDA data needs for policy making and the focus of the report is on those needs.

Implementing such an approach would require the use of a comprehensive set of metrics to characterize food systems, set meaningful goals, track food systems performance, and evaluate the impacts of food systems interventions. Food systems metrics are also useful to structure debates and communicate to policy makers and the general public. This paper provides an updated analytical framework of food systems and uses this to systematically identify relevant metrics and indicators based on data availability in low- and middle-income countries (LMICs). The list of indicators partly overlaps with the Sustainable Development Goals (SDG) indicators, but these do not cover all aspects of the food system. We conclude that public data are relatively available on food systems drivers and outcomes, and on some, but not all, of the activities. Book excerpt: Food insecurity and low resources continue to be a burden influencing the health, well-being, growth, and development of millions of U.S. children and adults. Groups and individuals experiencing restrained access to food are our neighbors, individuals we may see each day, and individuals who we may not interact with or see because of their isolated situations. They include the elderly, those experiencing mental illness, veterans, certain race/ethnic groups, adolescents, young women with children, those living in rural areas, and those using food pantries, among others. Many of these groups, both hidden and visible, have rates of food insecurity above the national average that are resistant to national improvements in food security. Yet, attention to these subsets of the population is imperative to improve U.S. health and nutrition and to reduce rates of chronic disease. Many groups face specific barriers to maintaining sufficient food, for example, rural populations may find it difficult to access federal food assistance or other resources such as food pantries and nutrition education because of distance or lack of consistent internet access separating them from these resources. Further, their remote locations may make it difficult to obtain the types of foods that they prefer. Other specific barriers may include limited facilities and equipment for food preparation, access to culturally appropriate foods and preparation supplies, and foods that complement the foods that they already have. Tailored approaches to quantify access to food, the nutrition environment, dietary intake, and other barriers are necessary to build successful interventions and to quantify the needs of these populations.

Weight difference among demographic subgroups, and difference in specific behaviors, health awareness, and eating patterns can be linked to weight outcomes. An economic framework helps explain how socioeconomic factors affect an individual's ability to achieve good health. Our results suggest that income, household composition, and formal education help explain variation in behaviors and attitudes that are significantly associated with weight outcomes.
