Book excerpt: This article is drawn from information from the Safety Center database, and from a study conducted for the Flight Safety Foundation of approach and landing accidents worldwide. Summary statistics, conclusions of the study, and approach tips and techniques are presented to highlight the risk involved and possible ways of assessing and dealing with that risk. USASC DATA A review of US Army Safety Center accident data of IFR accidents since FY80 provided the basis for the following results. The review encompassed all Class A accidents involving Army aircraft on instrument-flight plans. The data did not include any data associated with inadvertent IMC mishaps. Since FY80 there have been 18 Class A IFR accidents. Of these accidents, 33% were rotary wing and 67% were fixed-wing. There were 25 fatalities with approximately the same ratio of rotary-wing/fixed-wing fatalities - 32% and 68% respectively.

As part of the winter academic program, it may be useful to survey aircrews to determine which hazards they consider the most severe, and evaluate the effectiveness of the controls that are in place. From such a survey, necessary upgrades to winter training plans and development of new controls can be put in place. Winter has been a regular on the calendar for a long, long time. There's nothing we can do about that, even if we wanted to. In fact, the very predictability of changing seasons gives us time to plan our training for the different kinds of flying problems each season brings. If you haven't already done it, get your refresher training, review FM 1-202, and be alert to the hazards associated with winter flying.

Book excerpt: As an Army we are involved in missions around the world and doing a lot of things without the experience base we once enjoyed. This lack of experience, continuous deployments, and not having the discipline to maintain and enforce standards are basic causes of accidental losses. Some of us wearing wings are not executing fundamental tasks, those taught to us in flight school, to standard. Analysis of major FY99 aviation accidents reveals that most of the accidents didn't happen at the time of impact or during the crash sequence; they really occurred much earlier with a breakdown in leadership, standards or discipline. In fact, FY99 produced Army aviation's worst safety performance since Desert Shield/Desert Storm. With 18 Class A aviation flight accidents and 20 fatalities, the upward trend in accidents that began in FY98 continued to climb. When leaders fail to enforce established standards, the natural result is accidents-and accidents cost. They cost lives, they cost time, and they cost equipment: a total cost of more than $ 139 million for aviation in FY99. The bottom line: soldiers are dying and we are destroying costly equipment at a rate that is unacceptable. Leaders who understand and accept responsibility will help solve this Armywide problem. Effective leaders will make sure soldiers know what the standards are and will ensure the standards are enforced.

Relative to this time last year we are seeing reductions in the number of accidents in nearly every category: Total aviation ground accidents, military fatalities, off-duty accidents, and privately owned vehicle (POV) accidents. These numbers are direct results of leaders integrating risk management into training and battlefield operations, as well as off-duty safety. As of 31 March 2000, total aviation and ground Class A accidents are 10.1 percent lower than FY99, and equal to the three-year average. Total military fatalities are also reduced 7.5 percent from last year) but still are 1.4 percent higher than the 3-year average.

Every day we expose our soldiers to hazards in uncertain and complex operational and training environments. Increased mission optempo, leader inexperience, constant changes with personnel resource issues in terms of time, equipment, etc., are all present. None of these elements alone is the inherent cause of accidents. However, when left uncontrolled, seemingly low-risk hazards can collectively raise risk to an unacceptable level. The cumulative effect of these risks may create) breakdowns in leadership, discipline, training, and standards, which in turn can quickly set the accident chain of events in motion. In fact, an analysis of Class A accidents reveals that breakdowns in discipline, leadership, training, and standards were the main contributing causes of these accidents. Data also supports that, in many cases, the accident didn't just happen on the day of the helicopter crash or the tank rollover. Sometimes the sequence of events that culminated in the accident started days, weeks, and even months before-and not always at the accident unit level.
