We find that social protection programs are an important component of Government expenditures, and complements other Government social spending, including education and health spending. In recent years, the Government has strengthened social protection by: (i) increasing social protection expenditures; (ii) shifting social assistance from generally inefficient food and in-kind programs to more efficient cash-based programs; (iii) shifting social assistance from relatively untargeted programs to those which are targeted to poor people; and (iv) easing demand side constraints faced by households investing in human capital. Despite these positive developments, challenges to social protection remain: (i) social assistance and employment programs remain underfunded relative to the needs of the population; (ii) development partner financing remains crucial even though they are prone to external risks; (iii) little isknown about which social welfare services and employment programs work well; (iv) many pensionparameters are not in line with best-practice and therefore, sustainability can be improved; (v) generalized subsidies, which are notoriously bad instruments to target poor people, are absorbing Government resources in a tight fiscal environment.

In Tanzania, measures targeting informal-sector workers have been evolving over time. Earlier researchers reported low participation of such workers in the country's public health-insurance schemes, which mainly complemented workers in the formal sector, ignoring or not responding to the needs and challenges of informal workers. For reasons of sustainability, healthinsurance schemes need to incorporate a package of other benefits that address the actual needs of informal workers in both rural and urban areas. Implementation of the National Informal Sector Scheme (NISS) was thought to be of particular interest for informal-sector workers, as, by its design, it combined health insurance for such workers with access to small loans in order to cater for other life contingencies than health. In this paper, we use the case of health insurance under the NISS to answer the following questions: What strengths, weaknesses, opportunities and challenges are occasioned by the scheme? Was the scheme participatory and needs-based? Is it appropriate to integrate the health-insurance scheme with benefit packages like loans and credits as a pension scheme? What are the potential measures that could be used to boost the enrollment of informal workers in the NISS scheme? The study adopted a cross-sectional research design and gathered data and information from 35 key informants. Findings from the study indicate that performance of the health-insurance scheme under NISS mainly was influenced by: 1) the termination of medical service benefits for new NISS members after April 2019; 2) the limited number of accessible health facilities; 3) limited benefit packages, 4) inconvenient payment mechanisms for the members, and 5) a generally negative attitude on the part of informal-sector members and a lack of awareness of what NISS is all about. Like those working in formal-sector jobs, informal-sector workers in Tanzania also need access to good health services, soft loans and pension schemes. All key players, such as NHIF, NSSF, financial organizations and the apex organizations of informal-sector workers like the VIBINDO Society and SHIUMA Tanzania, have a great role to play in addressing the needs of informal-sector workers.

Part of the agenda in recent years to tackle poverty in Africa has been the launching of social safety nets programs. All countries have now deployed safety net interventions as part of their core development programs. The number of programs has skyrocketed since the mid-2000s though many programs remain limited in size. This shift in social policy reflects the progressive evolution in the understanding of the role that social safety nets can play in the fight against poverty and vulnerability, and more generally in the human capital and growth agenda. Evidence on their impacts on equity, resilience, and opportunity is growing, and makes a foundational case for investments in safety nets as a major component of national development plans. For this potential to be realized, however, safety net programs need to be significantly scaled-up. Such scaling up will involve a series of technical considerations to identify the parameters, tools, and processes that can deliver maximum benefits to the poor and vulnerable. However, in addition to technical considerations, and at least as importantly, this report argues that a series of decisive shifts need to occur in three other critical spheres: political, institutional, and fiscal. First, the political processes that shape the extent and nature of social policy need to be recognized, by stimulating political appetite for safety nets, choosing politically smart parameters, and harnessing the political impacts of safety nets to promote their sustainability. Second, the anchoring of safety net programs in institutional arrangements †“ related to the overarching policy framework for safety nets, the functions of policy and coordination, as well as program management and implementation †“ is particularly important as programs expand and are increasingly implemented through national channels. And third, in most countries, the level and predictability of resources devoted to the sector needs to increase for safety nets to reach the desired scale, through increased efficiency, increased volumes and new sources of financing, and greater ability to effectively respond to shocks. This report highlights the implications which political, institutional, and fiscal aspects have for the choice and design of programs. Fundamentally, it argues that these considerations are critical to ensure the successful scaling-up of social safety nets in Africa, and that ignoring them could lead to technically-sound, but practically impossible, choices and designs.
