Book excerpt: The majority of college students will experience a traumatic event either before or during college, and a significant subset of those students experience PTSD symptoms. Additionally, many college students engage in problematic alcohol use. There is evidence that individuals with co-occurring PTSD symptoms and alcohol and other substance use disorders often use substances in an attempt to reduce their PTSD symptoms, and college students with PTSD symptoms experience more negative alcohol and drug consequences than those without PTSD. The relationship between PTSD and alcohol and other substance use disorders is often cyclical: individuals use substances to cope with PTSD symptoms, which in turn exacerbate symptoms and make them vulnerable to further traumatic experiences. Mindfulness interventions have been successfully utilized for individuals with PTSD or substance use disorders. However, to date, no previous studies have evaluated treatments for college students with co-occurring PTSD symptoms and problem drinking. This study evaluated the feasibility, acceptability, and efficacy of a 4-week group loving-kindness meditation (LKM) compared to referral to treatment as usual (RTAU) for college students with PTSD symptoms and problem drinking. Seventy-six college students participated in the current study. Overall, the LKM group was feasible and acceptable to college students, although recruitment of eligible students into the study was lower than expected and attendance at LKM groups was modest. Participants’ PTSD symptoms, drinking quantity, and negative drinking consequences decreased over the course of the study, although there was no significant differences between the LKM and RTAU conditions on these outcomes. Additionally, higher coping drinking motives predicted greater PTSD symptoms and more drinking consequences, highlighting the need for early effective intervention for individuals who drink to cope with their PTSD symptoms.

Additionally, there is a gap in the literature examining the feasibility of virtual MBIs for this population, and there are no studies examining the feasibility of adding accessibility features (e.g., closed captioning, image descriptions, chair yoga) to virtual MBIs. Koru mindfulness is an MBI that has been feasible with recruitment/enrollment, attrition, intervention processes, and acceptability of the intervention and significantly decreased symptoms of depression, anxiety, perceived stress, and sleep problems for emerging adults and college students when implemented in-person (Greeson et al., 2014, Weis et al., 2021). In an RCT of the Koru MBI delivered virtually during the Fall 2020 semester, researchers found that compared to a waitlist control group, Koru increased mindfulness and physical activity, and decreased anxiety, stress, worry, rumination, and other mood difficulties (Mirabito & Verhaeghen, 2022). Virtually administered versions of Kour had no significant differences in depression or sleep difficulties, which differs from the in-person Koru studies (Mirabito & Verhaeghen, 2022; Smit & Stavrulaki, 2021). In this way, more studies examining the feasibility of virtual Koru interventions are needed. The current study modified the Koru mindfulness intervention to include accessibility features for college students with mental and physical conditions/disabilities for virtual implementation of the intervention based on the ORBIT model of behavioral treatment (Powell et al, 2021). Based on the refinement phase of the ORBIT model, the aim of this study was to examine the feasibility of a virtual format of the Koru mindfulness intervention refined for college students with mental and physical disabilities by integrating accessibility features. Specific elements of feasibility examined in this study: recruitment and enrollment, participant adherence to the intervention, attrition, intervention processes, and participant acceptability of the intervention. With regard to recruitment and enrollment, the recruitment duration, group formation was not sufficient to meet the minimum group size of four participants, recruitment methods were not sufficient to reach the target sample size, and there were barriers to recruitment and enrollment that need to be addressed. The findings indicate corrective actions are needed to increase recruitment and enrollment to reach the target sample size needed for a clinical trial, increase adherence to the intervention, more adequately assess attrition in assessment completion, and further modify study protocol and intervention elements based on suggestions provided by participants. There are a variety of corrective actions needed to address these areas. For recruitment and enrollment, corrective actions include increasing recruitment duration, increasing the number of groups offered and varying days/times they are offered and possibly exploring asynchronous options, continuing to involve disability support services to aid in recruitment, recruiting from more than one university, making the eligibility survey shorter and moving some questions from the eligibility survey into the pre-survey, eliminating the confirmation step, and allowing individuals to participate as long as they commit to attending at least three out of four group sessions. Corrective actions for adherence include allowing participants who missed a session to attend a different group that week or attend a makeup session with others who also missed a session could increase connection with others during mindfulness practices. Recommendations related to attrition include increasing flexibility by eliminating the confirmation step and including a gift card for the completion of the pre-survey. Recommendations with regard to intervention processes include reviewing how the Koru app differs from the current study (e.g. no accessibility features, includes practices that are not mentioned in the current study, and does not include some current study features like chair yoga), potentially eliminating the Koru app and using accessible study materials instead, having cameras on at the beginning of each session to ensure privacy and confirm that there are no others in the room with the participant. After these changes are made, randomized controlled trials with larger samples from multiple universities may be implemented to determine efficacy using phase three of the ORBIT model of behavioral treatments (Powell et al., 2021). During future interventions, quantitative measures will be used not only to examine assessment protocols and attrition, but also to look for symptom change over time. Additionally, during future interventions, it is likely that COVID-19 will no longer be considered a pandemic, but rather an endemic. This could influence the feasibility of certain study processes. Because of this continuing to examine qualitative feedback via Koru evaluations and focus groups will be helpful, as well as continuing to examine participant acceptability and feedback on virtual and accessibility features.

Book excerpt: Self-medication theory (SMT) posits that individuals exposed to trauma and resulting posttraumatic stress symptoms (PTSD) are at risk for heavy drinking and associated negative consequences. Estimates of PTSD among college students are comparable to rates in the general adult population, and recent research has linked PTSD and negative alcohol-related consequences in college individuals. The social environment (e. g., perceived peer norms, active peer influences) is a critical predictor of alcohol involvement in college. Research also shows that emotionally supportive peers are critical to the diminution and resolution of PTSD symptoms. As such, drinking behaviors of supportive peers may have an effect on drinking outcomes for students struggling with posttraumatic distress. The present project aimed to assess the dynamic relationship between PTSD, alcohol behavior, and the influence of emotionally supportive peers at the daily level. Specifically, the study investigated: (1) whether higher daily levels of PTSD symptoms were associated with increased within-subject levels of daily alcohol use and consequences; (2) whether daily PTSD symptom expression and alcohol behavior was moderated by the typical drinking patterns in one0́9s emotional support group; and (3) whether the relationship between daily PTSD symptoms and daily alcohol behavior was moderated by the daily drinking behavior of emotionally support peers. In addition, multilevel modeling (MLM) analyses were used to test the relationship between weekly PTSD symptom severity and weekend alcohol behavior to delineate temporal associations posited by SMT. Exploratory MLM analyses also assessed how participants0́9 other friends/acquaintances moderated the daily and week to weekend PTSD-alcohol relationship. Trauma-exposed heavy drinking college students (N=128) completed a baseline assessment and subsequent 30 days of daily web-based surveys assessing alcohol use and related consequences, PTSD symptoms, emotionally supportive and other friend/acquaintance alcohol behavior. Results directly testing SMT were not supported. However, both supportive peer and other friend/acquaintance alcohol behavior moderated the relationship between daily PTSD and daily alcohol behavior, as well as weekday PTSD and weekend alcohol behavior. Overall, supportive peer drinking behaviors were not as harmful to those experiencing more daily/weekly PTSD symptoms relative to other friends/acquaintances' alcohol behaviors. These findings highlight the importance of social support as a buffer against problematic drinking and provide useful information for interventions aimed at high-risk drinkers.

However, few studies have investigated EFT's potential to reduce alcohol use outside the lab. This study piloted an academic goal-relevant (A-EFT) intervention for heavy drinking college students. Forty-five heavy drinking undergraduated were randomized to complete a brief A-EFT intervention, or control task. Recruiment rates supported the feasibility of our approach; interest and scheduling rates were high, and booter and follow-up completion rates were good. Participants assigned to A-EFT increased the amount of time spent studying in the evening compared to students assinged to control. Within-group analyses revealed significant decreases in alcohol demand and alcohol consumption, and an increase in protective drinking strategies in the A-EFT group. The current study supports the feasibility, acceptability, and preliminary efficacy of an A-EFT intervention for college student heavy drinkers. .

Book excerpt: A randomized controlled trial was conducted to test the relative efficacy of a brief directive protocol (DP) aimed at reducing posttraumatic stress disorder (PTSD) symptoms and an expressive writing protocol (EW), which has more than a 30-year history (Pennebaker & Chung, 2011) of research showing mostly positive effects. The DP included components with research support from studies of expressive writing, prolonged exposure, cognitive processing therapy, and mindfulness-based therapies. Participants were primarily undergraduate students at a large public university in the southern United States who endorsed at least one traumatic experience and who manifested a minimal level of PTSD symptoms. The primary outcome measure was the Modified PTSD Symptom Scale-Self Report (MPSS-SR; Falsetti, Resnick, Resick, & Kilpatrick, 1993) with a primary endpoint of 14-days post-treatment and 30-day follow-up for maintenance of symptom reductions. Multilevel mixed effects modeling was used to analyze results. Both treatment conditions showed statistically and clinically significant reductions in PTSD symptoms, with more robust effects associated with the DP condition. Additional analyses showed comparable significant reductions in physical illness symptoms between conditions, and significantly larger reductions of depressive symptoms by the DP condition. The DP condition was also significantly better at reducing thought suppression and rumination, and at increasing cognitive acceptance of the trauma. DP also showed significant reduction in alexithymia, which was not shown by EW. Assessment of distress levels during the interventions suggest EW achieves reductions in PTSD symptoms through in-session habituation. For the DP condition, participants demonstrated in-session habituation to explicit emotion cues but not to the trauma itself; however, other evidence also suggests habituation. Recommendations are offered regarding use of these protocols and further study of effective components.

The complexity of this comorbidity is well documented, often resulting in a more difficult clinical course, poorer treatment outcomes, and more severe physical and social health consequences overall when compared to either disorder alone. These problems underscore the importance in attaining a more comprehensive understanding of the role of malleable cognitive-affective factors in PTSD/AUD in order to structure evidence-based interventions to include specific additional treatment targets. Extant examinations of cognitive-affective factors that impact emotion regulation highlight unique associations of reappraisal, suppression, and rumination in anxiety and mood disorders and are strongly associated with PTSD. We examined the differential effects of these factors in trauma- exposed individuals by first empirically examining latent subgroups of PTSD/AUD in trauma-exposed individuals, then modeling these cognitive-affective factors to elucidate their role in specific profile patterns of PTSD/AUD symptom typologies. Our results support a four-class model of PTSD/AUD symptoms, with unique predictive effects of expressive suppression, problem-focused thoughts, repetitive thoughts, and anticipatory thoughts on latent class status. We discuss acceptance-based intervention techniques to reduce these cognitive-affective patterns, and how they may be incorporated to augment current empirically-supported interventions for PTSD/AUD.

Book excerpt: Dramatic rises in levels of anxiety, stress, and depression in college students have been observed over the past decade and is so pervasive that it has been deemed the College Student Mental Health Crisis (CSMHC). A number of experts have argued that much of this crisis can be attributed to students' overall lack of basic knowledge of emotions and adaptive emotional processing. To address this problem, this study sought out to develop a brief mindfulness workshop as an intervention for college students to help increase student well-being, decrease anxious and depressive symptomology, and allow for material to be easily internalized. In this study, 67 total participants, all of whom were college students at James Madison University, completed a mindfulness workshop entitled, From Critical to CALM: A Guided Mindfulness Workshop. Stages of this study included the following: 1) Participant recruitment; 2) Conduct the pre-intervention assessment of participants using the Henriques-10 Well-Being Questionnaire (H10WB), the CORE Outcome Measure (CORE-OM), and the Generalized Anxiety Disorder 7-item (GAD-7) scale; 3) Conduct the workshop; 4) directly after each workshop, administer a brief "Quiz" to assess the knowledge participants obtained as well as a satisfaction survey; and 5) Conduct the post-intervention assessment of all remaining participants using the same pre-intervention measures along with one qualitative question asking participants to indicate what concept(s)/idea(s) from the workshop they remember learning. Students' satisfaction of the workshop overall was high and the vast majority of them reported that they would likely use the workshop's strategies in the future. Two to three weeks after the workshop, students also retained at least some of the key knowledge elements from the workshop. Test-retest results for this study found statistically signficant change in students' well-being, as indicated by the H10WB. A positive trend on the GAD was also found, but there was no change found on the CORE-OM. Due to the limitations of this study, the results are not able to be effectively interpreted to determine causation. However, given the nature and brief time of the intervention, these results were deemed as encouraging. Further development of the workshop and adjustments to the study will need to be performed to determine if, indeed, the workshop may have succeeded in initiating improvement in well-being, potentially through facilitating mindful awareness of intrapsychic process.

Book excerpt: Sexual assault has been found to increase the risk of distressing psychological symptoms including PTSD, depression, somatization, drug and substance use, lower quality of life and experiential avoidance. As such, interventions for reducing the distress and negative impacts of sexual assault are of importance. Mindfulness is of particular interest as it addresses one of the prominent factors known to maintain psychological distress after trauma, experiential avoidance (Polusny et al., 2004; Merwin et al., 2009). Furthermore, an online mindfulness-based intervention can increase the accessibility and reduce the barriers to treatment. A within subjects repeated measures open clinical trial design was used to investigate the effectiveness of an online mindfulness based intervention for trauma related symptoms among college students. Specifically, the impact an online mindfulness-based intervention has on mindfulness, experiential avoidance, and PTSD using self-report measures was evaluated. In order to participate, individuals needed to be 18 years or older, endorse a history of unwanted sexual experiences and endorse moderate PTSD symptoms (scoring at least a 21 or over on the PCL-5). A sample of 10 students currently enrolled at a Midwestern university participated. Participants completed an eight-week online mindfulness based intervention. In addition, participants completed assessments evaluating psychological symptoms throughout the duration of the study. Results of the current study support the use of an online mindfulness-based intervention in reducing PTSD symptoms, experiential avoidance, general psychological distress, and in increasing mindfulness skills among college students. The current study did not support the use of an online mindfulness-based intervention in improving quality of life or in reducing depressive and somatic symptoms.
