At the heart of the concern is the issue of reproductive rights and freedom. While many researchers have assessed demographic and socio-cultural factors influencing contraceptive use in Africa, few attempts have been made to assess the same in Kenya. This book examines socio-economic and communication-based socio-cultural factors that can encourage or inhibit the up-take of LAPMs including problems associated with their use such as the prospects of new methods that are likely to emerge from ongoing research and development. It also examines the use and/or abuse of LAMPs and describes new reproductive health shifts. Thus this book will be especially useful to Medical and community health workers, community media workers and policy makers.

Book excerpt: The Regional Meeting on Social and Cultural Factors Affecting the Acceptance, Continuation and Discontinuation of Family Planning Practice was held at Pattya, Thailand from November 2 to 8, 1982. Objectives identified at the meeting were: 1) to review and exchange information among the countries of the region on the factors affecting the acceptance, continuation and discontinuation of family planning practice; 2) to recommend, on the basis of available experience in the region, strategies and experimental measures for overcoming the critical communication as well as other social and cultural barriers adversely affecting the practice of family planning; 3) to enhance regional cooperation in conducting further necessary research on the sociocultural factors affecting decision making on the use continuation of contraceptive methods by suggesting an appropriate conceptual framework and guidelines for a methodology in conducting such regional or subregional studies; 4) to suggest strategies for promoting improved evaluation and monitoring systems for family planning programs which take into account the extent and nature of the continuation and discontinuation of family planning practice. 8 countries participated: Bangladesh, India, Korea, Malaysia, Pakistan, Philippines, Sri Lanka, and Thailand. Recommended communication strategies to facilitate the continuation of family planning practice are: 1) improvement of the instructional or counseling activities by family planning personnel concerning technical information on contraceptive methods to counteract unfavorable rumors on contraceptive use. 2) Utilizing mass media and interpersonal channels to reinforce directly the continuance of family planning practice, as an alternative to simply promoting 1st acceptance. 3) Counteract unfavorable rumors related to contraceptive use by identifying influential persons at key locations in the communication networks of local communities and by having them serve as "trouble shooters" should unfavorable rumors arise. 4) Organize communication to increase the level of social support for acceptors of family planning. Design special communication strategies to increase acceptability of a small family norm, and to decrease fatalistic attitudes toward pregnancy and childbirth.

A significant number of useful studies have been conducted which account for aggregate fertility trends or explain individual fertility behaviors with some degree of power. It can be firmly asserted that the weakness of fertility research to date is not one of the poor theoretical formulations but its failure to place fertility within its local socio cultural and economic context. This study is an attempt to examine the possible socio-cultural and economic factors affecting women's role in family planning decision making with specific reference to Addis Ketema Sub-City, Addis Ababa. The findings indicate that socio-cultural and economic factors interact on fertility and women's fertility behaviors is greatly influenced by the socio-cultural values attached to child bearing.

Book excerpt: Sample survey reports from 8 villages in Malaysia, 8 villages in Korea, and 2 villages in India constitute this regional pilot study on the role of community communication networks (cliques) on the acceptance and continuance of family planning (FP). NEGOPY software was used in the analysis. The prototype questionnaire for ever-married men and ever-married women are included in the appendix. The convergence model of communication, which focuses on interrelationships (dyads and cliques) between individuals, subsystems, and systems, and the social network method on acceptance and continuance of family planning practice, were selected to overcome the deficiencies in atomistic and mechanistic models of communication. Several assumptions were made: 1. family planning communication is unique, 2. husband's and wives communication structures are different, 3. interconnectedness based on frequency of interaction is considered to be different from closeness structure, and 4. the number, size, and composition of cliques and individuals communication roles vary based on socio-economic characteristics and the number of users of family planning methods in cliques. The findings are ungeneralizable due to small sample size. In spite of methodological differences, some non-statistical evidence shows that family planning behavior is shaped by cliques. Some of the findings were that other members of a clique where there is maximum connectedness and persons using FP will also use FP, and the converse. Strong clique leadership influences FP and the selection of methods. When clique leaders act as a bridge, they exert more effective influence on other cliques. Young educated women tend to be bridges. There is a need to continue the work through the region on a theoretical and methodological level with some standardization of network concepts. NEGOPY needs to be replaced or made user friendly and accessible. Categorization of cliques, comparison of different cliques, valid generalization of communication roles, diffusion of information within and between cliques, and changes in formation and leadership in cliques require further analysis. Policy implications for long term strategy and service delivery involve the need to improve women's education. FP field staff in training or work settings need to focus on cliques and convergence models of communication, thus increasing individual contact and maximizing effectiveness.
