The literature revealed that the rate of recidivism and mental health issues affecting youth in the juvenile justice system is alarmingly high. Comprehensive mental health and community reintegration services are needed to reduce recidivism and improve youth functioning. The grant was developed to identify funding sources to implement the evidence-based practice of family integrated transitions for youth with co-occurring substance abuse and mental health disorders transitioning from secure juvenile facilities back to their homes. Actual submission and/or funding of this grant was not required for the successful completion of this project.
