Book excerpt: One of the major considerations of any environmental resource project must be the effect on human well-being; originally published in 1979, this study aims to deal specifically with the transmission of Schistosomiasis as a human environmental impact. Due to its relationship with water, Schistosomiasis transmission areas can be spread or aggravated by new water resource projects particularly in developing countries, which could potentially lead to an epidemic situation. Rosenfield aims to demonstrate how prevention ideas can be conceived early on in the design process by exploring methodologies for predicting the prevalence of Schistosomiasis in certain areas and management strategies for controlling transmission. This title will be of interest to students of Environmental Studies and professionals.

Available in PDF, EPUB and Kindle. Book excerpt: A comprehensive guide to the technical and practical factors that need to be considered when designing and implementing programmes for the control of schistosomiasis. Noting the success of strategies focused on morbidity control, the report shows how the spectrum of programme goals can now be broadened to include reductions in the prevalence and intensity of established infections and decreases in the intensity of transmission. Throughout, emphasis is placed on knowledge and experiences that can help programme managers establish feasible goals and then select control options in line with the form of infection, its public health importance, the degree and type of morbidity, available resources, and integration into the primary health care system. Information is specific to the different types of schistosomiasis and the distinctive epidemiological features, clinical manifestations, and response to treatment of each. The report has three main parts. The first, which is devoted to strategies for control, gives programme managers a concise, yet complete review of all factors that need to be considered when establishing priorities and deciding on the most appropriate options for control. Emphasis is placed on the many recent advances, including experiences with praziquantel, that have strengthened the tools available for prevention, diagnosis, treatment, and cure. While noting the severe financial constraints faced in many endemic countries, the report cites recent findings and experiences that make it possible for each endemic country to take action against schistosomiasis, even when resources are scarce and health services limited. The second part gives specialists a detailed state-of-the-art review of all technical developments relevant to control. Separate sections describe the distinctive patterns of morbidity and mortality seen in each form of infection, update knowledge about the parasite and its mammalian hosts, and summarize what is known about the snail intermediate host and its susceptibility to control by molluscicides, biological agents, and environmental management. Other sections describe a range of advances in diagnostic tools, from hospital-based radiological investigations to the use of portable ultrasound equipment at the village level, and issue advice on treatmet and retreatment schedules for chemotherapy with praziquantel, metrifonate, and oxamniquine. The final part uses profiles of control programmes in 23 countries to document the feasibility of control and illustrate the diversity of approaches that can be applied according to different national conditions, forms and prevalence of infection, health care systems, and available resources.

Available in PDF, EPUB and Kindle. Book excerpt: An estimated 200 million people in the world suffer from schistosomiasis (bilharzia), and according to the World Health Organization it ranks second behind malaria in terms of socioeconomic and public health importance in tropical and subtropical areas. The disease was present in Egypt in the Old Kingdom (c. 2600 BCE), and in 1998 it was estimated that almost six million Egyptians -- one fifth of the rural population -- were infected. Thus it remains one of the most serious public health problems in rural Egypt. This study is the first to paint a broad picture of schistosomiasis in rural Egypt. The authors' research in three Nile Delta villages between 1991 and 1997 provides an in-depth community-level view of patterns of transmission and strategies for control. An analysis of recent research and policy presents the national context for the study. Schistosomiasis is primarily a behavioral disease, associated with human behavior in relation to water, especially canals; strategies for disease control and treatment need to consider what people do, where, when, and why. Gender, Behavior, and Health stresses an area of particular concern to social scientists: gender issues are most fully revealed at the local level, where an infection such as schistosomiasis is transmitted, diagnosed, treated, and ultimately (it is hoped) prevented. This book is unique in presenting schistosomiasis primarily from the viewpoint of the social sciences, yet fully incorporating material from the biomedical sciences and other relevant disciplines.

Book excerpt: Schistosomiasis is a neglected tropical disease (NTD) affecting about 260 million people worldwide. Elimination of schistosomiasis remains a challenge because of high reinfection rates and limitations of current treatment guidelines and disease control interventions. Despite over 70 years of research on schistosome cercariae anti-penetrants, a personal protective product (PPP) remains elusive for the prevention of schistosomiasis. In this paper we explore perceptions of topical PPPs to identify potential opportunities and barriers in the development, promotion, and use as a tool to control and prevent schistosomiasis. Results from key informant interviews suggest that despite recognized benefits of a cercarial anti-penetrant, translation of research into a practical PPP for endemic areas is hindered by two critical issues: (1) minimal available evidence to demonstrate effective and practical use of topically applied products in community-based settings and (2) limitations of current business models to sustain product availability among high-risk groups in low-income settings. Additionally, introduction of a PPP would require an intensive behavioral change communication strategy to reinforce and enable routine use of the product. The potential additive impact of a PPP on reducing point of source infections, in combination with a comprehensive elimination strategy that includes preventive drug treatment, snail control, and improved water and sanitation, may still present an effective strategy to reduce moderate to high intensity of infection among high-risk groups, but requires additional translational research and business model development.

Book excerpt: Schistosomiasis is a public health problem in tropical and subtropical regions of Africa, Asia, the Caribbean and South America. It is one of the neglected tropical diseases (NTDs) - a group of diseases and conditions that affect particularly low-income populations, worldwide. Last year, WHO launched a new road map for 2021-2030 that aims to end the suffering from NTDs by 2030, in line with the Sustainable Development Goals. The road map specifically targets the elimination of schistosomiasis as a public health problem, globally. This guideline provides evidence-based recommendations in the following areas: prevalence thresholds, target age groups and frequency of PC, establishment of WASH and snail control activities to support control and elimination of schistosomiasis, diagnostic tests for the assessment of schistosomiasis infection in animal reservoirs, in snail hosts, and in humans. The guideline will provide support to Member States, programme managers, health workers and other stakeholders on the implementation of national schistosomiasis control and elimination programme.

Book excerpt: Offering an example for transnational cooperation and successful reduction of a neglected tropical disease, this volume shows how Chinese scientists and local physicians controlled schistosomiasis in Zanzibar. Over a four-year study, local medical specialists and the population of Zanzibar were taught how to diagnose the parasitosis caused by flukes (trematode worms) of the genus Schistosoma. Furthermore, methods to eliminate the disease and prevent new infections were established. The developed control system will avoid repeated increase of human schistosomiasis, which is still prevalent in the tropics and subtropics. Rural populations and poor communities lacking access to clean drinking water and adequate sanitation are most affected. This book is a blueprint of activities urgently needed to combat schistosomiasis in countries with low medical impact. The strategies outlined are particularly relevant to parasitologists and professionals in public health, physicians, medical personnel and also governmental, healthcare and pharmaceutical institutions.
