Book excerpt: "For nearly all infants, breastfeeding is the best source of infant nutrition and immunologic protection, and it provides remarkable health benefits to mothers as well. Babies who are breastfed are less likely to become overweight and obese. Many mothers in the United States want to breastfeed, and most try. And yet within only three months after giving birth, more than two-thirds of breastfeeding mothers have already begun using formula. By six months postpartum, more than half of mothers have given up on breastfeeding, and mothers who breastfeed one-yearolds or toddlers are a rarity in our society. October 2010 marked the 10th anniversary of the release of the HHS Blueprint for Action on Breastfeeding, in which former Surgeon General David Satcher, M.D., Ph. D., reiterated the commitment of previous Surgeons General to support breastfeeding as a public health goal. This was the first comprehensive framework for national action on breastfeeding. It was created through collaboration among representatives from medical, business, women's health, and advocacy groups as well as academic communities. The Blueprint provided specific action steps for the health care system, researchers, employers, and communities to better protect, promote, and support breastfeeding. I have issued this Call to Action because the time has come to set forth the important roles and responsibilities of clinicians, employers, communities, researchers, and government leaders and to urge us all to take on a commitment to enable mothers to meet their personal goals for breastfeeding. Mothers are acutely aware of and devoted to their responsibilities when it comes to feeding their children, but the responsibilities of others must be identified so that all mothers can obtain the information, help, and support they deserve when they breastfeed their infants. Identifying the support systems that are needed to help mothers meet their personal breastfeeding goals will allow them to stop feeling guilty and alone when problems with breastfeeding arise. All too often, mothers who wish to breastfeed encounter daunting challenges in moving through the health care system. Furthermore, there is often an incompatibility between employment and breastfeeding, but with help this is not impossible to overcome. Even so, because the barriers can seem insurmountable at times, many mothers stop breastfeeding. In addition, families are often unable to find the support they need in their communities to make breastfeeding work for them. From a societal perspective, many research questions related to breastfeeding remain unanswered, and for too long, breastfeeding has received insufficient national attention as a public health issue. This Call to Action describes in detail how different people and organizations can contribute to the health of mothers and their children. Rarely are we given the chance to make such a profound and lasting difference in the lives of so many. I am confident that this Call to Action will spark countless imaginative, effective, and mutually supportive endeavors that improve support for breastfeeding mothers and children in our nation."--Page v.

Book excerpt: The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) began 40 years ago as a pilot program and has since grown to serve over 8 million pregnant women, and mothers of and their infants and young children. Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. This, the second report of this series, provides a summary of the work of phase I of the study, and serves as the analytical underpinning for phase II in which the committee will report its final conclusions and recommendations.

Book excerpt: The benefits of breastfeeding for mothers and infants have been well established in the literature, as have pervasive racial disparities in breastfeeding initiation and duration. The American Academy of Pediatrics recommends exclusive breastfeeding to 6 months of age, and Healthy People 2020 aims to have 60.6% of all infants exclusively breastfed to 6 months of age. A review of existing literature revealed a dearth of research focused specifically on the breastfeeding beliefs or behaviors of low-income, African American women. Reported studies are either atheoretical or utilize health behavior theories focusing on individual-level factors. Little attention has been paid to the unique sociocultural context within which this population is situated. Further, many existing breastfeeding interventions were not specifically designed for low-income African American women, and therefore may overlook their unique barriers to breastfeeding. Taken together, critical feminist theory and a social cultural model could provide a more comprehensive, culturally grounded framework from which to understand breastfeeding disparities, and to begin working to address them. Utilizing these two theories, a multi-phase, mixed methods research study was designed to identify breastfeeding barriers and supportive factors for low-income African American women, and to help empower them to meet their breastfeeding goals through the design and testing of a breastfeeding assessment process. Phase 1 utilized a community-based participatory action research methodology called group model building to answer the following research question: How do low-income, pregnant and postpartum African American women describe breastfeeding within their socio-cultural context, including factors that help or hinder breastfeeding? In Phase 2A, the set of statements that emerged from Phase 1 activities were evaluated by breastfeeding scholars and practitioners for inclusion in a set of Q-sort cards. These cards formed the foundation of a Dynamic Breastfeeding Assessment Process (D-BAP). In Phase 2B, the D-BAP was tested with a sample of lactation specialists and pregnant women to answer the research question: What would an assessment process look like if it were informed by the perspectives of the women as captured in the model building process? Finally, in Phase 3 the D-BAP underwent feasibility testing. Mixed methods were used to answer two research questions: 3A. How do pregnant, low-income African American women experience the culturally grounded Dynamic Breastfeeding Assessment Process? 3B. When the Dynamic Breastfeeding Assessment Process is delivered, is there a measurable difference in breastfeeding self-efficacy and intent among pregnant, low-income African American women? Completion of the D-BAP was hypothesized to be associated with increased breastfeeding intent and higher levels of breastfeeding self-efficacy. A pre-post, paired-samples design was utilized in Phase 3. The Phase 1 and 2 activities successfully resulted in a testable Dynamic Breastfeeding Assessment Process. In Phase 3, women who participated in the D-BAP demonstrated a statistically significant increase in breastfeeding self-efficacy, but not in their breastfeeding intent. The qualitative and quantitative findings from this study can inform future research, particularly as it pertains to supporting low-income African American women to meet their breastfeeding goals: a critically important public health and social work goal.

Book excerpt: ABSTRACT Infant feeding practices can greatly impact a child's life with far reaching implications based upon decisions a mother makes for her newborn. Breastfeeding is the preferred method of infant feeding because of its association with health benefits to both the mother and child. The prevalence of breastfeeding among rural, low income, disadvantaged groups to include Native American (NA) and African American (AA) adolescent mothers is low. NAs and AAs bear a disproportionate burden of illness and health risks compared to other races and ethnic groups in the United States. Breastfeeding is a targeted public health strategy to improve the health of infants and children and decrease or eliminate the gap of health disparities among the maternal-child aggregate populations in our nation. The purpose of the study was to (1) test the Theory of Planned Behavior by examining the relationship of the components (attitudes, subjective norms, perceived control/ self-efficacy, and breastfeeding knowledge) to determine breastfeeding intention and initiation of rural, low-income Native American (NA) and African American (AA) adolescent mothers in rural communities in southeastern North Carolina; (2) determine the significant similarities and differences between breastfeeding and formula feeding adolescent groups related to the constructs; (3) explore the relationship of intention to initiation, and (4) examine the relationship between sociodemographic variables with breastfeeding intention and initiation of adolescent mothers. The sample included 60 NA and 60 AA adolescents recruited through resources of care that were offered to prenatal clients in two public health departments. Findings included the constructs of the TPB were significant in predicting the probability of breastfeeding intention and breastfeeding initiation in rural, low-income NA and AA adolescent mothers. Overall total variance explained by the model related to the prediction of breastfeeding initiation at 4 days postpartum was 73% and correctly classified 92.2% of cases. Three control measures were found to be statistically significant, with Perceived Breastfeeding Control having the highest significance, followed by Negative Breastfeeding Sentiments and Social Professional Support. The model revealed a statistical significance in the relationship of breastfeeding intention and breastfeeding initiation at 4 days postpartum in rural, low-income NA and AA adolescent mothers. The strongest predictor of breastfeeding initiation was the Breastfeeding Control Scale. The Formula Feeding Group scored significantly higher on the Negative Breastfeeding Sentiments scale, whereas the Breastfeeding Group scored significantly higher on the Breastfeeding Control Scale and the Positive Breastfeeding Sentiment Scale. Significant differences were found related to ethnic groups, with NA adolescents initiating breastfeeding more often, being married more often, and living with a significant other more often than AAs. More adolescents who attended childbirth classes and breastfeeding classes breastfed than those who did not. Recommendations of the study included the utilization of the constructs of the TPB in development of research based assessments and interventions that promote breastfeeding behavior among rural, low-income NA and AA adolescent mothers, and identification of at-risk individuals for not breastfeeding and promoting programs to assist with providing help for resolving barriers to breastfeeding, and nurse educators emphasizing the importance of promoting breastfeeding among adolescents and vulnerable populations

Book excerpt: Research Objective: There is international agreement that babies should be exclusively breastfed for at least six months but, the worldwide 6-month exclusive breastfeeding rate is only 40%. Among high-income nations with many breastfeeding services, rates are still poor - it is unclear why. This study investigated if the Five-Factor Theory of personality in conjunction with available Baby Friendly Initiative breastfeeding services explained breastfeeding outcomes. The study estimated BFI service-personality combinations impacts on breastfeeding initiation and duration. The study also tested to see if the Equality Act of 2010 had an impact on breastfeeding outcomes and if any available BFI services had a significant impact on dissemination, stratified by maternal personality. Study Design: To test the Five-Factor Theory, the models use a logistic regression, outcome variables were created by identifying mothers who initiated, breastfed for 2, 6 and 12 months. To estimate the impact of personality-BFI service interactions, marginal probability analyses were used to compare the interactions of the highest level of a personality factor and the lowest when interacted with an available BFI service. To test the impact of the Equality Act 2010, logistic regression was used for a pre/post analysis which included T2 (2010). Maternal personality factors were interacted with available BFI services in all analysis and in the pre/post analysis maternal personality factors, available BFI services and T2 (2010) were interacted. Population Studied: 4,317 mother-baby dyads observed in the United Kingdom Household Longitudinal Survey from 2001 to 2016 were be examined. Results: Results of the study showed that for mothers who are high in neuroticism, the availability of BFI health visitor services had a significant and positive result at initiation and 12 months duration for working mothers. Additionally, maternal agreeableness when interacted with maternity based services had a positive and significant result for mothers reaching 12 months duration. The results related to the Equality Act of 2010 had inconclusive findings. Conclusion: Increased understanding of how personality and health services interact to influence health outcomes has the potential to improve the adaptability and appropriateness of health services. This study provides early evidence of how mothers with different personality factors respond differently to available breastfeeding services. Evidence from this work can be used to inform practice through person-centered approaches. Early evidence provides an area for future research to support policies to expand health visitor programming which was identified to be effective for neurotic mothers - those most vulnerable to worry.
