Book excerpt: El objetivo general de la presente tesis ha consistido en profundizar en el conocimiento de las bases genéticas que subyacen a la etiopatogénesis de una patología compleja de origen multifactorial como es la artritis reumatoide (AR), mediante la búsqueda de posibles nuevos marcadores genéticos con valor predictivo de susceptibilidad y/o pronóstico de la enfermedad. Los genes HLA de la clase II son los únicos genes que hasta la fecha se ha asociado con un total consenso con la susceptibilidad a la enfermedad, observaciones clásicamente explicadas mediante la teoría del epítopo compartido. El modelo alternativo y recientemente propuesto de susceptibilidad genética a la AR (modelo RAP) permite una mejor distinción entre enfermos y controles que la teoría del epítopo compartido en términos de susceptibilidad y protección. En nuestra población, el haplotipo DQ3-DR4 es el que confiere una mayor susceptibilidad a la enfermedad. Se conoce que existen locus de susceptibilidad adicionales a los genes de clase II en la región HLA. Tras el análisis de baterías de microsatélites (ms) HLA, hemos establecido la existencia de clusters extendidos de ms empleando de manera independiente 2 poblaciones europeas de distinto fondo genético y con diferencias en cuanto a la prevalencia y a la severidad de la AR. Esta reproducción de los datos es una garantía de la validez de los mismos y a su vez que nuestra aproximación metodológica del empleo de ms polimórficos HLA es útil a la hora de definir configuraciones HLA. La segregación de los haplotipo definido como c1 (D6S1014*143-D6S273*139-D6STNFa*99-MIB*350-C1-2-5*196), correspondiente al haplotipo DRB1*0301-B*0801 resultó estar asociado a la predisposición a la AR independientemente a los genes de clase II y confirma la contribución de un locus segundo locus de susceptibilidad en la región telomérica de la región HLA. El mencionado haplotipo c1 (que aparece disminuido en población española respecto a la holandesa) cabe contribuirá la baja prevalencia y actividad moderada de la AR observada en el Sur de España. Estos resultados apoyarían la hipótesis de que las diferencias en prevalencia y severidad de la enfermedad descritas poblaciones podrían tener una base esencialmente genética.En una extensión de estos estudios se ha podido acotar el locus HLA de susceptibilidad genética a una región central del MHC en el haplotipo 8.1., que incluye no más de 30 genes funcionales. Hemos demostrado que los elementos HERV-LTR (repeticiones terminales largas de retrovirus endógenos humanos) de la región HLA de clase II se encuentran en total desequilibrio de ligamiento con los alelos DQ, por tanto las asociaciones descritas de estos elementos, especialmente de LTR3, con susceptibilidad a AR y a otras enfermedades autoinmunes no se justifican experimentalmente. Además hemos caracterizado por primera vez la presencia de una delección de alrededor de 5kb en determinados alelos DRB1, que junto a posibles indeterminaciones en el subtipaje HLA, justificaría las asociaciones previamente reportadas. Hemos caracterizado en su totalidad una fuente adicional de variación genética debida al polimorfismo existente entre genes HLA de clase II, consistente en la diversidad alélica en el primer exón del locus HLA-DQB1 y que codifica para la secuencia del péptido señal. Se ha caracterizado un nuevo polimorfismo en el alelo DQB1*0601 que ha permitido descifrar que este ha sido generado por un fenómeno de recombinación entre un haplotipo DR53 y un no-DR53. Se estima que 1/3 de la contribución genética a la AR reside en genes que mapean fuera del MHC por lo que así mismo hemos desarrollado la búsqueda de marcadores genéticos fuera del MHC, con un posible efecto sobre el desarrollo y el pronóstico de la AR: Hemos observado como los puntos polimórficos -174 y -622 en la región promotora del gen de la IL-6 se presenta en un total desequilibrio de ligamiento. Las variaciones de los mismos no parecen influir en la predisposición a sufrir AR, sin embargo existen diferencias significativas entre los genotipos de IL-6 con respecto a la edad de comienzo de la enfermedad. El análisis del polimorfismo del promotor NOS2 en relación a la AR sugiere que no juega un papel determinante en la susceptibilidad y/o pronóstico de la enfermedad. Sin embargo, la tendencia de asociación observada para algunos alelos, junto al hallazgo del efecto significativo que alelos NOS2 de repeticiones medianas en protección a la AR, hacen interesantes estudios en cohortes mayores y en otros grupos étnicos para categóricamente implicar o descartar a NOS2 como gen candidato de susceptibilidad a la AR. Hemos descrito la existencia de 2 haplotipos únicos conservados del promotor de la poli(ADP)ribosa y polimerasa (PARP) que además juegan un papel en la susceptibilidad genética a la AR. Nuestra hipótesis es que repeticiones largas CA y en especial el alelo de 97 bp, en conjunción con el resto de variaciones de secuencia presentes en el haplotipo extendido denominado "B", conducen a la expresión correcta de PARP, convirtiéndose por lo tanto en un factor de riesgo para la AR, promoviendo (vía NF-kB) la liberación de óxido nítrico al comportamiento sinovial, o bien conduciendo a la muerte celular a través de roturas en las hebras de ADN. El análisis del polimorfismo del gen que codifica para el receptor tipo IIIA de la fracción Fc de la IgG (FcgamaRIIIA) en la población 158 ha indicado que los genotipos FcgamaRIIIA-158 confiere diferente susceptibilidad a la AR y que el gen FcgamaIIIA, puede ser considerado como un nuevo factor genético de riesgo de la nfermedad. Es posible que 158F induzca un cambio conformacional en FcgamaRIIIA lo que resulte endiferencias cuali- y/o cuantitativas en la señalización que sean transducidas por FcgamaRIIIA-158F únicamente cuando contenga agregados bajo los estímulos apropiados. Nosotros sugerimos que esto explicaría la susceptibilidad incrementada de la enfermedad mediada por inmunocomplejos e individuos homocigotos FcgamaRIIIA-158FF.
