Book excerpt: Introduction : L'infection du matériel de neurostimulation est une complication redoutée, de par son traitement complexe et sa morbidité. Notre objectif était d'en déterminer l'incidence et les facteurs de risque d'infection de site opératoire dans cette chirurgie. Matériel et méthodes : Cette étude prospective, monocentrique, observationnelle a inclus et suivi pendant un an, tous les patients opérés d'une implantation ou d'un changement de matériel de neurostimulation médullaire, cérébral profond et nerveux périphérique entre 2013 et 2015. Résultats : Trois cent quatre-vingt-deux patients opérés d'une implantation (n=238) ou d'un changement de matériel de neurostimulation (n=144) par 4 chirurgiens ont été inclus. 198 (52%) étaient des femmes, avec un âge médian de 54 [46-64] ans et un score ASA à I-II pour 317 (83%) patients. Soixante seize pour cent des interventions se sont déroulées sous anesthésie générale. Nous avons observé une infection du site opératoire chez27(7,1%) patients. Le symptôme le plus fréquent était l'inflammation locale ou une collection au niveau du boîtier (21 (78%)), avec un délai médian d'apparition de 30 jours [14-90]. Staphylococcus aureus était le germe le plus fréquemment identifié. Onze patients (41%) ont nécessité le retrait complet du matériel, tandis que 13 (48%) patients ont bénéficié d'une explantation partielle intéressant le plus souvent le boitier de stimulation. Deux patients ont bénéficié avec succès d'un traitement conservateur. Conclusion : Notre étude nous a permis de déterminer l'incidence des infections de matériel de neurostimulation dans notre service, et d'en observer l'épidémiologie, l'écologie bactérienne et la prise en charge. Nous allons poursuivre les inclusions afin d'obtenir un effectif suffisant pour identifier de façon fiable des facteurs de risque d'infection.
