This was followd by serial angiography, and then came cineradiography which made simultaneous morphological and functional radiological examinations feasible. 2 Physiology of Kidney 2. 1 Hemodynamics of Renal Arteries Because of the negligible vascular elasticity of the rani arteries, the diameter variation resulting within one cardiac action is about 1 % [14]; but with these variations being lost in the accuracy of measurement, however, blood flow can be compared to the flow of liquid in rigid tubes. In a rigid tube, the velocity of the individual particles of liquid varies in relation to its distance from the tube axis. Under normal circumstances, a laminar flow prevails in the arteries, the blood flowing parallel to the vascular wall in coaxial cylindrical layers. The velocity is referred to here as v. The flow rate, I, stands for the ratio of the volume of liquid, V, flowing through a cross section of a tube to the time, t, required for this, or also for the product of flow velocity v and the cross section of the tube q: 1= Llv: Llt and I = v x q.

While long-term morbidity can only be improved by changing the patient's life-style and removing any risk factors that may be present(hypertension, hyperlipidemia,obesity,diabetesmellitus, nicotine abuse), symptomatic relief may be obtained by various procedures. Besides conservative treatment of arterial occlusive disease (physical therapy, medical treatment) and surgical vascular interventions (thromboendarterectomy, bypass procedures), balloon catheters introduced percutaneously under local anesthesia have found increased application in virtually all areas for the dilatation and recanalization of obstructive lesions in the past few years. The technique of balloon dilatation, as introduced by Griintzig as a further development of the percutaneous interventions by Dotter and Judkins, is no longer the decisive issue. The main objective of present clinical research is to de termine the proper indications and patient selection for this procedure, which has found its place between conservative treatment and surgery. The contents of this Symposium should provide some guidelines for the indi cations and postprocedural therapy for the referring physician, the angiologist, and for the radiologist and cardiologist performing the dilatation. Every day new ideas arise which are published in a multitude of papers. This heavy flow of information limits the possibility of selecting from a core of information and often prevents radiologists from communicating efficiently with their colleagues in other countries or continents. European radiologists are additionally hampered from participating in international communication because of the language barriers created by the national journals. As a result, Europe's contribution to scientific progress in this interdisciplinary field has influenced only regional developments.

As a physiologist: "What is the effect of this stenosis on coronary blood flow and myocardial function?"; as a clinician: " Is this lesion responsible for the patient's symptoms?"; and finally as an interventionalist: "Will revascularization of this artery improve the patient?" Fundamentally, the answer to these questions can be given to a large extent by measuring coronary pressure. That is the rationale of writing this book. 1. 1 Historical overview. Andreas Gruentzig and most interventional cardiologists in the early days of PTCA, had the intuitive feeling that pressure measurements could help to establish the severity of a coronary stenosis and to monitor the progress and result of a coronary intervention. At that time, measuring coronary pressure by the balloon catheter was part of a standard procedure. A residual transstenotic gradient of less than 15 mmHg was generally considered as a good result. Later, however, it turned out that measuring these (resting) gradients with balloon catheters was inaccurate an only had a limited prognostic value. Moreover, because there was no consistent theory to correlate pressure measurements to blood flow, the interest in measuring coronary pressures faded and disappeared almost completely with the introduction of new balloon catheters not intended for pressure measurement.

Following an emphasis on improved angiographic and subsequent intracoronary imaging techniques, functional assessment of coronary circulation has demonstrated to have major implications for diagnosis and treatment of coronary artery disease. Fractional flow reserve, a pressure derived index of stenosis severity, constitutes the best example of the current importance of physiological assessment in clinical practice. However, the acceptance of FFR by cardiologists contrasts with important voids in knowledge on the basic principles of coronary physiology and of other available techniques that, as an alternative to FFR, allow a more comprehensive assessment of coronary circulation. This is particularly noticeable in the assessment of microcirculation, an unavoidable compartment of coronary circulation that is frequently affected in acute coronary syndromes of in the presence of cardiovascular risk factors or non-coronary heart disease. A deeper understanding of the relationship between epicardial vessel and microcirculatory involvement has started with the advent of newer imaging techniques like invasive optical coherence tomography, and non-invasive CT and NMR techniques.
