Estudio unicéntrico, observacional, prospectivo y de seguimiento durante 1 año, de una serie de 112 pacientes tratados con VMNI. Book excerpt: ANTECEDENTES: La tasa de fracaso de la ventilación no invasiva (VNI) permanece elevada pese a la experiencia creciente en la técnica. Además, la VNI se realiza con diferentes tipos de ventiladores. El tipo de ventilador utilizado puede tener un impacto sobre la sincronización paciente-ventilador o la calidad del sueño. OBJETIVOS: Determinar la tasa de intubación e identificar los factores de riesgo de fracaso de la VNI en pacientes con insuficiencia respiratoria hipercápnica aguda (IRHA). Comparar la sincronización paciente-ventilador entre ventiladores de UCI, transporte (ambos con y sin activación del modo VNI) y ventiladores específicos de VNI. Evaluar el impacto del tipo de ventilador sobre la calidad del sueño de los pacientes hospitalizados en reanimación y que recibe VNI. Además comparar la calidad del sueño entre los periodos de VNI y de respiración espontánea. METODOLOGÍA: Se evaluó la capacidad de sincronización paciente-ventilador de diferentes ventiladores mediante un estudio de laboratorio usando un pulmón artificial y un modelo de fuga calibrada y en dos estudios fisiológicos usando electromiograma de superficie y pletismografía de inductancia. La calidad del sueño se evaluó mediante polisomnografía. RESULTADOS: La tasa de fracaso de VNI puede reducirse a menos del 15%. La ausencia de patología respiratoria crónica subyacente o edema agudo de pulmón se estableció como factor predictivo de fracaso de la VNI, pero no la disminución del nivel de conciencia al ingreso. Los ventiladores específicos permiten una mejor sincronización paciente-ventilador que los ventiladores de UCI o los ventiladores de transporte, incluso con la activación del modo VNI, aunque existe una gran variabilidad en la eficacia de estos algoritmos entre ventiladores. El tipo de ventilador no tiene influencia sobre la calidad del sueño y los pacientes duermen durante los periodos de VNI. CONCLUSIONES: En una unidad con experiencia la tasa de fracaso de la VNI puede reducirse a menos del 15% y la VNI de pacientes con encefalopatía hipercápnica grave puede ser exitosa. Los ventiladores específicos han mostrado una mejor capacidad de sincronización con el paciente, sin haber demostrado diferencias en la calidad del sueño con respecto a los ventiladores convencionales. Mantener la VNI durante las horas de sueño no impide dormir a los pacientes con IRHA.

Book excerpt: Las enfermedades de caja torácica y neuromusculares son trastornos que representan una importante causa de insuficiencia respiratoria crónica hipercápnica. La ventilación mecánica no invasiva domiciliaria (VMNID) se considera el tratamiento de elección en estos pacientes cuando la insuficiencia respiratoria está establecida. El proceso de adaptación a la VMNID puede realizarse con el paciente ingresado o bien de forma ambulatoria. El hecho de que no existan estudios que demuestren la superioridad de una localización con respecto a otra, condiciona que la elección del lugar de adaptación se base en las preferencias de cada equipo instalador. Otro factor de gran interés y poco estudiado es la evolución a largo plazo de los pacientes con VMNID, y qué factores se asocian con la mortalidad. Los objetivos de esta tesis fueron comparar la eficacia y el coste que representa la adaptación de VMNID de forma ambulatoria, en relación a la adaptación con el paciente ingresado, en pacientes con insuficiencia respiratoria secundaria a enfermedades neuromusculares o a alteraciones de la caja torácica. Así como analizar la mortalidad y los factores pronósticos de una cohorte de pacientes con enfermedad de caja torácica tratados con VMNID. Para ello se realizaron dos estudios, un ensayo clínico randomizado multicéntrico que evaluó la eficacia mediante la presión parcial de anhídrido carbónico en sangre arterial (PaCO2) basal y a los seis meses de haber iniciado la VMNID en cada uno de los grupos de adaptación (ambulatoria y hospitalaria), así como los costes económicos de cada uno de los grupos. El segundo trabajo consistió en un estudio observacional prospectivo que analizó la mortalidad de una cohorte de pacientes con VMNID, y las posibles variables asociadas. Los principales resultados de los dos estudios realizados fueron que la adaptación ambulatoria a la VMNID no presenta diferencias en cuanto a eficacia respecto la adaptación hospitalaria en pacientes con insuficiencia respiratoria crónica secundaria a enfermedades neuromusculares o a alteraciones de la caja torácica, resultando además más barata y que una PaCO2 mayor o igual a 50 mmHg al mes de haber iniciado la VMNID y la presencia de comorbilidad son factores predictivos de mortalidad en pacientes con insuficiencia respiratoria crónica secundaria a enfermedades de caja torácica. Así pues se puede concluir que en la práctica clínica habitual la adaptación ambulatoria a la VMNID es una opción a considerar ya que puede representar un ahorro en los gastos sanitarios y que una vez el paciente esté adaptado a la VMNID, la PaCO2 al mes aporta información relevante que puede condicionar decisiones terapéuticas.

This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. The Eighth Day reflects a conviction that the human trajectory, for all its uniqueness and indeterminism, will never be satisfactorily understood until it is framed in dynamics that are common to all of nature. The problem in doing this, however, lies in ourselves. The major social theories have failed to treat human social evolution as a component of broader natural processes. The Eighth Day argues that the energy process provides a basis for explaining, comparing, and measuring complex social evolution. Using traditional ecological energy flow studies as background, society is conceived as a self-organization of energy. This perspective enables Adams to analyze society in term of the natural selection of self-organizing energy forms and the trigger processes basic to it. Domestication, civilization, socioeconomic development, and the regulation of contemporary industrial nation-states serve to illustrate the approach. A principal aim is to explore the limitation that energy process imposes on human social evolution as well as to clarify the alternatives that it allows. Richly informed by contemporary anthropological historicism, sociobiology, and Marxism, The Eighth Day avoids simple reductionism and denies facile ideological categorization. Adams builds on work in nonequilibrium thermodynamics and theoretical biology and brings three decades of his own work to an analysis of human society that demands an extreme materialism in which human thought and action find a central place.
