Book excerpt: EXPERIENCE OF CGM (CONTINUOUS GLUCOSE MONITORING) FOR CHILDREN AND ADOLESCENTS IN A PEDRIATRIC DIABETES UNIT.Graciela Louu2020, Gemma Larramonau2021, Teresa Montaneru2021, Sara Barbedu2020u2020Hospital Pediu00e1trico Miguel Servetu2021 Universidad de ZaragozaObjective: Estimate the use of CGM in a Pediatric Diabetes Unit from a Spanish region. To find out differences between the different CGM systems (included flash type or intermittent) and according to the type of treatment. To analyze the metabolic control, frequency of mild and severe hypoglycaemia among the same patients before and after using CGM and to compare patients with and without CGM.Methods: Data are collected from September to December 2017, from patients whose diagnosis was between 2003 and 2017. Patients are grouped in age groups (5; 6-10; 10) and demographic, metabolic control and treatment variables are collected. Results: 120 patients collaborated, implying a response of 80%.70% use MDI, of which 48% use CGM, being 89% in the case of patients using CSII. In older than 10 years there is a predilection for intermittent measurement systems, unlike those under 5 years prefer continuous measurement. The use of CGM significantly reduces HbA1c compared to those who do not use them and significantly reduces the number of mild hypoglycaemia, with the disappearance of severe hypoglycaemia in 6 months. 43% of patients reduce the number of glycemias/day significantly after 6 months of use. CGM decreases almost a 5% the HbAc1 value and a 19% the Coefficient of Variation. Conclusion: There exists an improvement in the metabolic control and the glycemic variability of using CGM. Hypoglycemia is diminished by GCM, to a greater extent by the Integrated sensor-augmented pump system. Adolescents prefer to use intermittent measurement systems, reducing the number of glycemia/d.

Available in PDF, EPUB and Kindle. Book excerpt: Currently, available information on pediatric and adolescent diabetes is limited to chapters in larger books covering the broader topic of pediatric endocrinology, and these do not have the space to delve into specific topics. This concise, timely book contains everything that a practicing provider needs to know in order to provide comprehensive, up-to-date care for children and adolescents with diabetes, from the latest methods for diagnosing various types of diabetes to integrating cutting-edge technology in the care of this patient population. Initial management, the use of insulin pumps, continuous glucose monitoring, and automated insulin delivery are discussed in detail, as are nutrition therapy, exercise, psychosocial challenges, acute and long-term complications, and future directions for treatment and research. Further, this book provides clinicians with guidelines for the implementation of best practices as outlined by leading associations such as the American Diabetes Association (ADA) and International Society of Pediatric and Adolescent Diabetes (ISPAD). The Yale Children’s Diabetes Program has been ranked among the best in the United States, including clinicians and researchers who are world-renowned for their efforts in improving the care of children with diabetes. This wealth of knowledge and experience positions the author team well as experts in this field.

Book excerpt: The American Diabetes Association/JDRF Type 1 Diabetes Sourcebook serves as both an evidence-based reference work and consensus report outlining the most critical components of care for individuals with type 1 diabetes throughout their lifespan. The volume serves not only as a comprehensive guide for clinicians, but also reviews the evidence supporting these components of care and provides a perspective on the critical areas of research that are needed to improve our understanding of type 1 diabetes diagnosis and treatment. The volume focuses specifically on the needs of patients with type 1 diabetes and provides clear and detailed guidance on the current standards for the optimal treatment of type 1 diabetes from early childhood to later life. To accomplish the book’s editorial goals, Editors-in-Chief, Drs. Anne Peters and Lori Laffel, assembled an editorial steering committee of prominent research physicians, clinicians, and educators to develop the topical coverage. In addition, a Managing Editor was brought on to help the authors write and focus their chapters.

Available in PDF, EPUB and Kindle. Book excerpt: Written by a Johns Hopkins pediatric endocrinologist and an award-winning medical writer, Clinical Management of the Child and Teenager with Diabetes is a comprehensive and accessible clinical guide to the modern medical management of Type 1 diabetes. Emphasizing the concerns and needs of the child and family, the book is addressed to primary care clinicians, who are increasingly responsible for managing the care of patients with diabetes. With detailed guidelines for establishing a comprehensive patient management plan, this book discusses how to effectively ensure the health and well-being of the child or adolescent with diabetes. The authors cover the basic mechanisms of disease, provide diagnostic and treatment guidelines, describe the spectrum of management options, and offer valuable advice on how to achieve effective communication among parent, child, and members of the health care team. Throughout, the authors stress the importance of helping children with diabetes to live as normally as possible. They address a wide range of management alternatives, including practical issues — snacking at the mall after school (and how to adjust insulin to cover it), what to do about insulin shots during sleepovers, what to tell friends and classmates as well as teachers and other educators, and how to counsel adolescents about the effect of alcohol on blood sugar levels. The text is enhanced by a variety of sample forms and documents: assessment and other record-keeping forms; sample letters to teachers describing activity restrictions and warning signs they should watch for; growth charts; tables of timing and duration of action for different types of insulin; and diabetic ketoacidosis treatment flow sheets.

This book was released on 2022 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Type One Diabetes Mellitus (T1DM) poses social and health threats to the lives of many individuals. In the past, management of diabetes mellitus has revolved around the use of Self-Monitoring Blood Glucose devices such as fingersticks. Today, advancement in diabetes technology has led to the innovation of Continuous Glucose Monitoring (CGM) devices, which has drastically influence how diabetes is treated. The leaders of this DNP project hoped to bring awareness of CGM to young people with diabetes and provide resources that would assist them in making informed decisions about how to manage this illness. Because CGM use continues to be the lowest among young people (Tanenbaum et al., 2018), an educational program titled Level-Up Workshop was developed to improve how this population perceives and uses CGM devices. The Level-Up Workshop was a virtual educational workshop designed to provide education on CGM use to youth with T1DM receiving diabetes-related care from a pediatric clinic. Workshop activities were geared towards improving problem-solving, trouble-shooting, and social support. Two workshop sessions were implemented and during each session, participants engaged in a series of activies led by a diabetic educator, a CGM device representative, and young people with T1DM. Recruitment to the workshops was challenging, and a total of 4 young people were eventually enrolled. Both patients and staff participants were positive about the experience. Despite the low number of participants, the format of this project shows promise as a means of educating young people with diabetes.

It provides advice, but also a constructive critique of research methods and, in turn, the authors also make a contribution to the theories of research methodology. Topics covered include ontology, epistemology and engagement with academic literature, as well as research design approaches and methods of data collection. There is a keen focus on quality in both the analysis and evaluation of research and new models are proposed to help the new researcher. The authors conclude by examining the challenges in getting work published and close with some words on quality of thought and action. The ideas in the book come from the authors’ extensive experience in teaching research methods courses in higher education, health and the corporate sector, as well as several empirical research projects that have helped provide a methodology for higher education. It will be of particular interest to postgraduate students, academic developers and experienced academics from a wide variety of disciplines.
