L’enquête que nous avons menée auprès de 92 sages-femmes exerçant en Champagne-Ardenne et en Picardie, nous a permis d’étudier leur pratique de l’examen clinique du nouveau-né en salle de naissance et d’en dégager des tendances. Nous apprenons que les sages-femmes n’effectuent pas l’examen complet qui leur a été enseigné pendant leurs études. D’une part, parce qu’elles manquent de connaissances sur certains gestes précis, mais surtout parce qu’elles l’adaptent à la salle de naissance. Ainsi, elles recherchent de manière systématique les pathologies pouvant avoir des conséquences graves ou nécessitant une prise en charge immédiate, et adaptent le reste de l’examen à la clinique. Elles limitent ainsi les gestes traumatiques pouvant perturber le nouveau-né et favorisent une rencontre parents-enfant dans de bonnes conditions. L’enjeu principal de la naissance est là : allier bien-être et sécurité du nouveau-né et de ses parents.

La prise en compte environnementale et la qualification des examinateurs étaient également recherchées. La finalité était d'améliorer la fiche de transmission écrite. Matériels et Méthode : Il s'agissait d'un audit de pratique clinique, prospectif, transversal et mono centrique au CHMS de Chambéry. Notre étude descriptive était effectuée par observations directes d'examens cliniques de nouveau-nés à terme, nés par voie basse en Salle de Naissance, pratiqués par des sages-femmes diplômées d'état et des étudiantes sages-femmes en dernière année d'étude. Résultats : Les examens observés n'étaient pas complets pour 94.4% d'entre eux. Les gestes très fréquemment pratiqués comprenaient la cotation du score d'Apgar et la vérification de l'intégrité : du crâne, de la face, labio-palatine, des narines, périnéal et anal , des membres et du rachis. A contrario les gestes peu réalisés et à améliorer comprenaient les examens cardio-respiratoire, abdominal, locomoteur complet et neurologique détaillé. L'environnement du nouveau-né était globalement respecté mais les informations étaient rarement données aux parents. 36 examens sur 55 étaient pratiqués par les sages-femmes ou étudiantes sages-femmes. Conclusion : Les examens cliniques des nouveau-nés en salle de naissance ne sont pas complets mais permettent de dépister les anomalies et pathologies d'urgence vitale. Les axes d'amélioration correspondaient à certaines pratiques trop peu réalisées. La feuille de transmissions écrites a été mise à jour.

Au travers de cette étude nous avons noté les gestes effectués lors de l'examen du nouveau-né et recherché les éléments pouvant l'influencer. En s'appuyant sur les recommandations nationales et internationales ainsi que sur des ouvrages pédiatriques, nous proposons une ébauche de protocole concernant les gestes à réaliser au cours de l'examen du nouveau-né afin d'optimiser le dépistage en salle de naissance.

Une étude quantitative observationnelle à visée descriptive a été réalisée pour évaluer les connaissances et les pratiques des étudiants sages-femmes, leurs difficultés et leurs besoins face à cet examen clinique du nouveau-né à terme en salle de naissance. A l'aide d'un questionnaire en ligne, nous avons pu recenser les réponses des étudiants sages-femmes de la troisième à la cinquième année des écoles de sages-femmes du Grand Est. Une moyenne de 38.94 points sur 45 a été obtenue par les étudiants sur les items sur leurs connaissances, ce qui montre que les étudiants ont des connaissances adéquates et complètes sur l'examen clinique du nouveau-né malgré des enseignements qui diffèrent entre les écoles de sages-femmes. Concernant leurs pratiques cliniques, 17 items sur les 20 proposés aux étudiants sont réalisés systématiquement, ce qui montre une bonne pratique de la part des étudiants. Les étudiants sages-femmes ont des connaissances et une pratique clinique adaptée pour la réalisation d'un examen clinique complet du nouveau-né en salle de naissance mais éprouvent le besoin de compléter et d'améliorer leurs connaissances et leurs pratiques.

Les sages-femmes interrogées provenaient de maternités de type III des régions Auvergne-Rhône-Alpes et Île-de-France. Le recueil des données a été fait à partir d'un questionnaire en ligne. Les résultats ont été obtenus après analyse statistique et analyse du contenu. Résultats : En Auvergne-Rhône-Alpes les sages-femmes n'assurent pas l'examen clinique du nouveau-né au premier jour de vie contrairement à certaines maternités de type III d'Ile-de-France (21.43%). Les connaissances théoriques, l'expérience acquise sur le terrain et le désir d'une prise en charge globale du couple mère-enfant font parties des éléments favorisants. Les principaux freins évoqués sont la surcharge de travail des sages-femmes, le fonctionnement actuel des services avec le pédiatre, au centre du suivi du nouveau-né, ainsi qu'une formation initiale insuffisante sur certains points (notamment l'auscultation cardiaque). Conclusion : La place de la sage-femme par rapport à l'examen clinique du nouveau-né au premier jour de vie n'est pas clairement définie. Cette étude a montré qu'une modification des pratiques est possible. Une réorganisation des services ainsi qu'une remise à niveau des sages-femmes semblent nécessaires pour une évolution générale des pratiques.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.

Yet in 1992-93, some five hundred people died from cholera in the Orinoco Delta of eastern Venezuela. In some communities, a third of the adults died in a single night, as anthropologist Charles Briggs and Clara Mantini-Briggs, a Venezuelan public health physician, reveal in their frontline report. Why, they ask in this moving and thought-provoking account, did so many die near the end of the twentieth century from a bacterial infection associated with the premodern past? It was evident that the number of deaths resulted not only from inadequacies in medical services but also from the failure of public health officials to inform residents that cholera was likely to arrive. Less evident were the ways that scientists, officials, and politicians connected representations of infectious diseases with images of social inequality. In Venezuela, cholera was racialized as officials used anthropological notions of "culture" in deflecting blame away from their institutions and onto the victims themselves. The disease, the space of the Orinoco Delta, and the "indigenous ethnic group" who suffered cholera all came to seem somehow synonymous. One of the major threats to people's health worldwide is this deadly cycle of passing the blame. Carefully documenting how stigma, stories, and statistics circulate across borders, this first-rate ethnography demonstrates that the process undermines all the efforts of physicians and public health officials and at the same time contributes catastrophically to epidemics not only of cholera but also of tuberculosis, malaria, AIDS, and other killers. The authors have harnessed their own outrage over what took place during the epidemic and its aftermath in order to make clear the political and human stakes involved in the circulation of narratives, resources, and germs.
