Book excerpt: EVOLUTION OF A SURGEON - "You're going to be very successful. And someday you are going to a faraway place, a place where people speak a different language," the eccentric old woman told me. "It's called America." - I saw our once-robust neighbor slowly succumb to illness because he was too poor to seek medical help. I was just a kid, but I wanted to become a doctor to help people like him. - "The only way you can go to medical school is through full-scholarship. I HAVE NO MONEY!" my father's words challenged me. - Decades later and after performing thousands of successful surgeries, I am still reminded of that particular moment that fateful day when I first experienced the loss of a patient. As a paralyzing pain and fear of failure overcame my being, I felt a light touch on my shoulder. I turned and saw my senior attending physician, Dr. Hadder. Understanding and kindness shone from his eyes. "Don't beat yourself up, Robbie," he said. "We do our best. But we are not God. We can't save everybody." - When I set out to write this book, I wanted it to be an inspiration to the reader, to share my thoughts on what is essential in life. My journey to become the surgeon that I have always wanted to be is nothing compared to my conversion and renewal of faith: it is as insignificant as a grain of sand in the vastness of the ocean.

Book excerpt: “I am a doctor.” Every year, thousands of medical school graduates utter these four simple words. But as you will see in Playing God, earning an M.D. is just the first step to becoming a real physician. In this page-turning, thrilling, and moving memoir, Dr. Anthony Youn reveals that the true metamorphosis from student to doctor occurs not in medical school but in the formative years of residency training and early practice. It is only through actually saving and losing patients, taking on the medical establishment, wrestling with financial and emotional survival, and fighting for patients’ lives that a young doctor becomes a mature and competent physician. Dr. Youn takes you from the operating rooms of a university surgery residency program to the gleaming offices of top Beverly Hills plastic surgeons to opening the doors of his empty clinic as a new doctor with no money, no patients, and mountains of debt. Playing God leaves you with an unexpected answer to that profound question: “What does it mean to be a doctor?” In Playing God, you will take a journey through the world of surgery, hospitals, and the practice of medicine unlike any that you have traveled before.

Available in PDF, EPUB and Kindle. Book excerpt: From an eminent surgeon and historian comes the “by turns fascinating and ghastly” (The New York Times Book Review, Editors’ Choice) story of surgery’s development—from the Stone Age to the present day—blending meticulous medical research with vivid storytelling. There are not many life events that can be as simultaneously frightening and hopeful as a surgical operation. In America, tens-of-millions of major surgical procedures are performed annually, yet few of us consider the magnitude of these figures because we have such inherent confidence in surgeons. And, despite passionate debates about health care and the media’s endless fascination with surgery, most of us have no idea how the first surgeons came to be because the story of surgery has never been fully told. Now, Empire of the Scalpel elegantly reveals surgery’s fascinating evolution from its early roots in ancient Egypt to its refinement in Europe and rise to scientific dominance in the United States. From the 16th-century saga of Andreas Vesalius and his crusade to accurately describe human anatomy while appeasing the conservative clergy who clamored for his burning at the stake, to the hard-to-believe story of late-19th century surgeons’ apathy to Joseph Lister’s innovation of antisepsis and how this indifference led to thousands of unnecessary surgical deaths, Empire of the Scalpel is both a global history and a uniquely American tale. You’ll discover how in the 20th century the US achieved surgical leadership, heralded by Harvard’s Joseph Murray and his Nobel Prize–winning, seemingly impossible feat of transplanting a kidney, which ushered in a new era of transplants that continues to make procedures once thought insurmountable into achievable successes. Today, the list of possible operations is almost infinite—from knee and hip replacement to heart bypass and transplants to fat reduction and rhinoplasty—and “Rutkow has a raconteur’s touch” (San Francisco Chronicle) as he draws on his five-decade career to show us how we got here. Comprehensive, authoritative, and captivating, Empire of the Scalpel is “a fascinating, well-rendered story of how the once-impossible became a daily reality” (Kirkus Reviews, starred review).

The book will look at advances in medicine and surgery that improved the lot of the American soldier. In particular, the book will examine the impact of disease upon troop strength, which had special impact in the Revolutionary War through the post-Civil War period. Forward surgery in the modern sense came of age in World War I. The challenge of so many different theaters of conflict in World War II will be examined from the portable surgical hospital of the China-Burma-India Theater of Operations to the surgical evacuation hospital teams of the European Theater of Operations. The evolving care models will feature the story of the Korean War mobile army surgical hospital. The defining performance of helicopter air evacuation in Vietnam, along with improved surgical techniques, will be discussed. Finally, the many advances of forward surgery from the post-Vietnam era to the present will be presented."--Provided by publisher.

Available in PDF, EPUB and Kindle. Book excerpt: Wars in the 19th Century were accompanied by a very heavy loss of life from infectious diseases. Typhus fever, dysentery, malaria, typhoid fever and yellow fever caused many more deaths than wounds inflicted by enemy actions. During the Peninsular War, for example, for every soldier dying of a wound, four succumbed to disease. This book examines the development and evolution of surgical practice against this overwhelming risk of death due to disease. It reviews three major conflicts during this time: the Peninsular War, the Crimean War and the Boer War and also considers many minor wars fought by the British Empire in the intervening years, and highlights significant medical and surgical developments during these conflicts. War surgery in the first part of the 19th Century was brutal, and it had to be carried out swiftly. It was performed at speed because there were no anaesthetics and the wounded often died during the procedure. Surgeons focussed their attention on wounds of the arms and legs, because limbs were both easily accessible to the surgeon (unlike organs inside the abdomen and chest) and lent themselves well to amputation. This was commonly the operation of choice for many war wounds of arms and legs. Some surgeons performed more difficult surgical procedures to try to preserve the limbs and attempted to repair damaged tissues, but these operations took longer and caused greater suffering to the patient. Abdominal and chest wounds were not treated since surgeons did not have the means, the ability, or the understanding, to cut into the abdomen and chest to repair the damaged organs successfully. An important development which contributed to surgery moving forwards was the discovery of general anaesthesia, which became available in time for the Crimean War. However, whilst it certainly rendered operations pain-free, it was associated with significant numbers of deaths during surgery on wounded soldiers because of the poorly understood effects that anaesthetics had, particularly on the heart. As a result, operative surgery did not extend its scope a great deal, and military surgery remained focussed on surgery of the limbs. However, fewer amputations were performed during the Boer War at the end of this period. Britain sent observers to several wars in which it was not involved to learn military lessons and to understand the medical and surgical aspects of war. The American Civil War and the Franco-Prussian War were two such conflicts. The Russo-Japanese War resulted in a very significant advance in surgery for abdominal wounds, but Western observers either failed to notice or ignored pioneering work performed by a Russian female surgeon called Vera Gedroits. As a result, when the Great War began in 1914, lessons had to be re-learned by British surgeons, and many soldiers who suffered penetrating abdominal wounds lost their lives when they should have survived. Unfortunately, one of the hallmarks of war surgery is that successive generations of surgeons make the same mistakes as their forebears and the same lessons have to be learned time and again.

Book excerpt: Throughout its development, from an ancient craft of magic and religion to a field of science and technology, surgery has inspired strong feelings--hope and admiration, fear and censure, but never indifference. Here, for the first time-- in The Illustrated History of Surgery--is a readable and chronological account, across the whole spectrum of world history, of the development of surgery and of the great personalities whose skill and courage paved the way for the modern surgeon. The book also includes information on the use of drugs, herbal remedies, early anaesthetics and a whole range of procedures related to surgery and its evolution. The text ranges from primitive surgery in prehistoric times to today's transplants and implants - with a glimpse into how modern surgery is likely to develop in the future. There are portraits of the great surgeons throughout the ages, detailed accounts of the milestones in the progress of the profession - the breaking of new ground and forming of solid bases from which the next generation of surgeons could advance to new and revolutionary techniques. The Illustrated History of Surgery is a beautifully presented book, with more than 200 colour illustrations gathered from around the world; it tells the story of surgery in a way that is both intelligible and enthralling.

Available in PDF, EPUB and Kindle. Book excerpt: Historians have examined the development of surgical techniques and of the surgical profession itself, but have paid scant attention to the tools that made surgery. Surgeon and historian Kirkup (honorary curator, Royal College of Surgeons, UK) demonstrates how surgical instruments as sophisticated as ultrasound or lasers began as teeth, mouth, fists, fingernails, and fingers. Far from being a compendium drawn from instrument catalogs, this volume is a masterpiece of scholarship. The instruments are situated in the surgical theory and practice of their times. Kirkup's skill and devotion in his presentation and description raise the work from a register to a natural history of the instruments. (The only caveat is that some pictures are not for the squeamish.) An extensive bibliography and an excellent index add to the book's value.
