Sans soutien, ceux-ci peuvent avoir de la difficulté à accomplir leur rôle parental, menant parfois à la perte de la garde de leur enfant. Malgré le fait qu'on relève à cet égard un manque de soutien, très peu de services psychosociaux sont offerts à cette clientèle vulnérable qui présente des besoins particuliers. Cet essai a pour but d'effectuer une analyse critique de la réalisation d'un projet d'intervention, qui visait à mettre sur pied une intervention de groupe basée sur l'aide mutuelle afin de soutenir les parents présentant un trouble psychotique dans le développement ou la reprise de leurs compétences parentales. L'intervention, à laquelle sept membres ont participé, a eu lieu à l'Institut universitaire en santé mentale de Montréal et s'est déroulée sur une période de huit semaines entre les mois d'octobre et décembre 2018, à raison d'une rencontre de deux heures par semaine. Afin d'évaluer l'intervention, plusieurs outils ont été utilisés, soit le questionnaire d'autoévaluation de la compétence éducative parentale, une échelle d'impact différentiel, une grille d'analyse des dynamiques d'aide mutuelle et des entrevues individuelles semi-dirigées. Les résultats de l'évaluation de l'intervention permettent de présumer qu'une intervention de groupe basée sur l'aide mutuelle permet de fournir le soutien nécessaire aux parents présentant un trouble psychotique dans le développement ou la reprise de leurs compétences parentales. Les résultats et le déroulement du groupe ont démontré la nécessité et la pertinence d'une telle offre de service auprès de cette clientèle.

Le fait de devenir parent a un impact important sur la vie de toute personne et peut même affecter sa manière de se définir. En effet, la parentalité implique l'apprentissage de plusieurs nouveaux rôles et l'acceptation des nouvelles responsabilités y étant rattachées. S'inscrivant dans une perspective qui conçoit que l'adulte se développe tout au long de sa vie (Baltes, 1987) à travers les différentes expériences vécues, la présente thèse tente de comprendre le sens qu'il donne à la parentalité et d'en discuter les manifestations concrètes. Plus spécifiquement, elle vise à établir un lien entre le développement du parent adulte et les compétences parentales les plus fréquemment étudiées dans les écrits pertinents (attachement, empathie, fonctionnement réflexif et style d'éducation parentale). Les participants, des parents ayant au moins un enfant âgé de 6 à 12 ans (n=100), ont répondu à des questionnaires; certains d'entre eux ont également accepté d'être vus en entrevue (n=49). Après avoir élaboré une grille d'analyse portant sur le développement de l'adulte dans un contexte de parentalité, en s'appuyant sur le modèle du développement intellectuel et éthique de (Perry, 1970, 1981), un lien a été effectué entre ce développement et les compétences parentales retenues. Les résultats permettent de constater une variabilité des degrés de compétences parentales selon le niveau de développement de la parentalité observé. Considérant ces résultats de façon dynamique et interactive, quatre dimensions interreliées se dégagent : à une perception marquée de la responsabilité qui incombe au parent, vient se greffer une préoccupation à savoir comment gérer le rôle d'autorité qui s'y rattache et un souci d'y arriver à travers le type de communication à privilégier avec son enfant pour aboutir à perception de contrôle qu'il détient en tant que parent sur la situation; ces quatre dimensions varient selon le niveau de développement du parent. En conclusion, considérant l'influence possible du développement de la parentalité en interaction avec les compétences parentales sur le développement de l'enfant, une réflexion est émise quant aux éléments susceptibles d'aider les professionnels en leur offrant des pistes d'intervention intéressantes auprès de parents qui en exprimeraient la nécessité. Les compétences parentales réfèrent notamment aux rôles, aux responsabilités et aux caractéristiques d’un parent dans sa façon de s’occuper de son enfant. Les compétences ont un effet fort sur la relation parent-enfant et sur le développement de l’enfant. La littérature scientifique montre que l’implantation de programmes ciblant les familles à risque peut générer un effet sur les compétences parentales et les comportements de l’enfant, bien que ces effets varient d’un programme à l’autre. Cependant, les services sociaux québécois nous renseignent peu quant aux interventions proposées et leur efficacité pour améliorer les compétences parentales et les comportements de l’enfant. Ce manque de connaissance empêche de définir clairement les variables qui ont une influence sur l’amélioration des compétences parentales et des comportements de l’enfant en intervention. L’objectif général de cette étude consiste à évaluer les effets des interventions sur les compétences parentales et à évaluer l’influence de certaines variables externes sur l’intervention. Afin de répondre à cet objectif, 92 parents consultants pour un enfant de 0 à 12 ans ont répondu à un questionnaire au moment de commencer une intervention et à un autre environ deux mois plus tard. Des MANOVAs à mesures répétées montrent que les interventions engendrent une amélioration de la majorité des compétences parentales, mais certaines compétences parentales demeurent peu influencées. Par ailleurs, aucune variable externe n’a été déterminante pour influencer l’intervention. Cette étude originale entame l’avancement des connaissances sur les interventions auprès des familles dans les services sociaux québécois.

Brown emphasizes that these unconscious transformational processes occur spontaneously, in the blink of an eye, through the "unconscious work" in which the analyst and patient are engaged. Though unconscious, these processes are accessible and the analyst must train himself to become aware of the subtle ways he is affected by the patient in the clinical moment. By paying attention to one’s reveries, countertransference manifestations and even supposed "wild" or extraneous thoughts, the analyst is able to obtain a glimpse of how his unconscious is transforming the ambient emotions of the session in order to formulate an interpretation. Brown casts a wide theoretical net in his exploration of these transformational processes and builds on the contributions of Freud, Theodor Reik, Bion, Ogden, the Barangers, Cassorla, Civitarese and Ferro. Bion’s theories of alpha function, transformations, dreaming and his clinical emphasis on the present moment are foundational to this book. Brown’s writing is clear and aims to describe the various theoretical ideas as plainly as possible. Detailed clinical material is given in most chapters to illustrate the theoretical perspectives. Brown applies this theory of transformational processes to a variety of topics, including the analyst’s receptivity, countertransference as transformation, the analytic setting, the paintings of J.M.W. Turner, "autistic transformations" and other clinical situations in the analysis of children and adults. The Analyst's Reveries finds increasing veneration for the analyst’s use of their reveries, while revealing important differences amongst post-Bionians in how reverie is defined and used clinically. Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation. The Analyst's Reveries will be of great interest to psychoanalysts and psychoanalytic psychotherapists.
