The apical design of implants is important for immediate placement. Sufficient primary stability has to be obtained with the only 4~5 mm at the apical part of implants, because it would be the part inserted into alveolar bone of extraction sockets.The Aim of this research is to evaluate the primary stability of implants with different body designs in vitro. Two types of implants (Straight type and Tapered type) were tested. The primary stability was assessed by the insertion torque.Straight and Tapered type implants were inserted into a bone model and insertion torque was recorded. The insertion torque for an insertion depth of 5mm was compared. Tapered implants had tendency to show bigger insertion torque compared with straight implants.Tapered implants can be screwed into narrower drill holes because of its narrower apical end and advance into bone, expanding the hole with its tapered body. This feature seems to have led to sufficient insertion torque at the depth of only 4~5mm. These results suggested that the tapered implants can be used with good primary stability even in cases of immediate placement.

Abstract: Background: Type I immediate implant placement has gained popularity because it may reduce treatment time, number of surgeries and post-extraction bone loss. However, this is potentially challenged by inadequate keratinized mucosa for flap adaptation and difficulties in achieving primary stability. Moreover, it has been proven that post-extraction bone loss is an inevitable biological process, which affects treatment outcomes. Objectives: To estimate survival and success rates of implants and the implantsupported prostheses, the prevalence of biological, technical and aesthetic complications, and the magnitude of soft and hard tissue changes following implant placement immediately into fresh extraction sockets. Material & methods: An electronic search in MEDLINE (PubMed) and the Cochrane library from 1991 to July 2010 was performed to include prospective studies on immediate implants with a mean follow-up time of at least 1 year. The survival rates were computed using the STATA statistical software. Weighted means of soft and hard tissue changes were obtained by the inverse variance method. Results: A total of 46 prospective studies, with a mean follow-up time of 2.08 years, were included. The annual failure rate of immediate implants was 0.82% (95% CI: 0.48%-1.39%), translating into the 2-year survival rate of 98.4% (97.3%-99.0%). Among the 5 factors analyzed [reasons for extraction, antibiotic use, position of implant (anterior versus posterior, maxilla versus mandible), type of loading], only the regimen of antibiotic use affected the survival rate significantly. Lower failure rates were found in groups that were provided with a course of post-operative antibiotics. The success of implant therapy was difficult to assess due to scarce reporting on biological, technical and aesthetic complications. Soft tissue changes occurred mostly in the first 3 months after the provision of restoration, and then stabilized towards end of the first year. Marginal bone loss predominantly took place in the first year after implant placement, with a magnitude generally less than 1mm. Controversy on hard tissue preservation with platform-switching technique remained unsolved. Conclusions: Despite the high survival rate observed, more long-term studies are necessary to determine the success of implant treatment provided immediately after tooth extraction. Two maxillary central incisors and 1 mandibular canine were extracted. A totalof 3 implants were inserted, the average initial stability obtained 42.5 N.cm with a range of 40-45 N.cm, allowing in each cases theplacement of an immediate provisional restoration. At 6 months recall the implant survival rate was 100%.The results of this case series suggest that the use of the BLX Straumann implant design is predictable to achieve sufficient primarystability, which is necessary for immediate temporization and subsequent osseointegration for replacement of maxillary incisors.

Book excerpt: Introduction: As a result of continuous development, changes in the macro geometry of dental implants over time have had a profound influence on their mechanical behavior and the osseointegration process.Aim: The objective of the present in vitro study was to evaluate the mechanical performance of dental implants with macro geometric changes to the apical region, relative to conventional implants. Material & Methods: Comparative insertion torque measurements were conducted between conventionally tapered implants and the newly designed reverse tapered body shift implants. All implants were placed in simulated extraction sockets created in orthopedically-certified synthetic bone blocks with a density of 0.32g/cm3. The simulated extraction sockets were created by drilling a 5mm deep osteotomy with a diameter of 5.3mm, to ensure that the coronal section of the implants had no influence on the insertion torque performance. Thereafter, the osteotomies for the implants were prepared as per the manufacturers guidelines. For the sake of comparison, all implants had a total length of 11.5mm and a coronal diameter of 3.0mm, whereas the newly designed implant had a reverse tapered body shift with a diameter of 3.5mm. Results: In total, 40 insertion torque measurements were recorded (20 in the control group and 20 in the test group). The average insertion torque for the conventionally tapered implants was 8.45 Ncm (SD= 0.69), whereas the average insertion torque in the test group was 12.55 Ncm (SD= 1.61). To convert these in-vitro results to a more clinically relevant situation, the percentage increase in insertion torque between the two groups was calculated. On average, the newly designed reverse taper body shift implant achieved 49% greater insertion torque, compared with a conventional implant of the same coronal diameter and length.Conclusion: Reverse tapered body shift implants achieve an average of 49% greater primary stability in extraction sockets than conventionally tapered implants with the same coronal diameter and length. This may overcome issues with primary stability in cases of immediate placement.

Resonance frequency analysis (RFA) and insertion torque (IT) has been proposed to assess this stability with a quantitative method.Aim of the Study: To investigate the clinical outcome, marginal bone maintenance and implant survival for implant treatment using the fast loading protocol which include immediate provisionalization and early delivery of the definitive prosthesis. Materials and Methods: 25 patients were treated with 48 Neoss ProActive Tapered implants placed in both maxilla and mandible in healed bone as well as in fresh extraction sockets. Minimum primary stability, (IT >35 ncm and RFA >60), for all single implants was established before subjected to immediate (within 48 h), non-functional, load using screw-retained temporary restorations (single or maximum 2 splinted implants with 2-3 teeth units). Definitive prostheses were delivered within 6-8 weeks from implant placement, based on RFA measurements during early healing phase. IT, RFA, intraoral radiographs, and impressions of implant positions were registered at implant placement. During clinical follow-up RFA was measured at 2, 4, 6 weeks and 6 months from implant placement. Marginal bone level measurements were performed from intraoral radiographs taken at implant placement, 6-, and 12-month follow-up visits.Results: 48 implants were subjected to the fast loading protocol. IT was over 40 Ncm for 46 implants. Two implants with IT35 Ncm were both splinted to another implant with IT50 Ncm, tightening the retention screw with low forces. No implants were lost during the observation period, giving an implant survival rate of 100%. Mean RFA measurements remained stable without any decrease during the initial healing phase, 78.3u00b14.0 in the mesiodistal direction and 77.6u00b14.4 in the buccolingual direction at placement and 81.3u00b13.7 and 80.8u00b13.6 respectively at 6 months. Radiographic readings showed that at placement the implant platform was at bone level (+0.13 mm). Mean marginal bone remodeling was -0.47u00b10.38 mm from implant placement to 1 year, -0.49u00b10.38 mm from implant placement to 6 months, and only 0.01u00b10.28 mm between 6 to 12 months follow-up. No significant differences could be found for marginal bone remodeling between implants placed in healed bone and those placed in fresh extraction sockets.Conclusions: Achieving good primary stability (IT of 40 Ncm) with maintained implant stability during initial healing phase can guarantee predictable clinical outcome for patients subjected to a fast loading protocol. Fast loading showed no significant differences in marginal bone loss in the first year of follow-up as compared to other studies investigating the same implant type but followed different surgical and prosthetic protocols.
