The magnitude of these changes is important for decision-making and comprehensive treatment planning, with provisions for possible solutions to expected complications during prosthetic rehabilitation. Objectives: to review all English dental literature to assess the magnitude of dimensional changes of both the hard and soft tissues of the alveolar ridge up to 12 months following tooth extraction in humans. Materials and methods: An electronic MEDLINE and CENTRAL search complemented by manual searching was conducted to identify randomised controlled clinical trials and prospective cohort studies on hard and soft tissue dimensional changes after tooth extraction. Only studies reporting on undisturbed post-extraction dimensional changes relative to a fixed reference point over a clearly stated time period were included. Assessment of the identified studies and data extraction was performed independently by two reviewers. Data collected were reported by descriptive methods. Weighted means and percentages of the dimensional changes over time were calculated where appropriate. Results: The search provided 3954 titles and 238 abstracts. Full text analysis was performed for 104 articles resulting in 20 studies that met the inclusion criteria. In human hard tissue, horizontal dimensional reduction (3.79 0.23 mm) was more than vertical reduction (1.24 0.11 mm on buccal, 0.84 0.62 mm on mesial and 0.80 0.71 mm on distal sites) at 6 months. Percentage vertical dimensional change was 11-22 % at 6 months. Percentage horizontal dimensional change was 32% at 3 months, and 29-63% at 6-7 months. Soft tissue changes demonstrated 0.4-0.5 mm gain of thickness at 6 months on the buccal and lingual aspects. Horizontal dimensional changes of hard and soft tissue (loss of 0.1 mm to 6.1 mm) was more substantial than vertical change (loss 0.9 mm to gain 0.4mm) during observation periods of up to 12 months, when study casts were utilised as a means of documenting the changes. Conclusions: Human re-entry studies showed horizontal bone loss of 29-63% and vertical bone loss of 11-22% after 6 months following tooth extraction. These studies demonstrated rapid reductions in the first 3-6 months that was followed by gradual reductions in dimensions thereafter. Book excerpt: The annals of orthodontics are filled with studies aimed to understand how extraction orthodontic treatment might change the face. Although many studies have addressed profile changes due to extraction treatment, fewer studies have focused on how extractions change a patients smile. With the advent of surface imaging systems such as 3dMD, it is now possible to visualize the smile, and any changes incurred during orthodontic treatment, in three dimensions. Subjects for this study were chosen from the pool of 11-18 year old patients treated at the Podray Orthodontic Clinic at the Temple University Kornberg School of Dentistry. Subjects were Cl II patients, and must have been treated with either extraction of any combination of premolars or treated without extraction. Subjects were divided into four experimental groups based on two characteristics- mandibular angle (those with angles greater than 28o versus those with angles less than 28o) and treatment (extraction versus non-extraction). The resulting groups were separated as follows: high-angle extraction patients (n=8), low-angle extraction patients (n=6), high-angle non-extraction patients (n=7), and low-angle non-extraction patients (n=15). For each subject initial and final 3dMD images were superimposed using 3dMD Vultus software. A color histogram was constructed to visualize changes during treatment. The cheeks, commissures, upper and lower lips, chin, and nose, were also landmarked, and the changes in these landmarks were calculated. Volume changes were also calculated between pre and post treatment 3D data. Results showed that the lower lip and right commissure changes between high-angle extraction and non-extraction groups were statistically significant. A qualitative analysis of the histograms further supported these findings. In general, a greater change in soft tissue landmarks and soft tissue volumes could be seen in high-angle patients than low-angle patients. Differences in the changes that result from treatment type (extraction vs. non-extraction) were seen in the high-angle group. In contrast, similar changes result from treatment type (extraction vs. non-extraction) in the low-angle groups. Furthermore, the lip changes seen in extraction patients upon smiling are very similar to those changes seen in the same patient in repose. Most interestingly, soft tissue differences of the face due to treatment, growth, or both, seem to disappear upon smiling, with the exception of the lips. Qualitative assessment of these changes in the smile might be a more appropriate method for identifying soft tissue changes than statistical analyses. Similar studies with larger sample sizes are a promising direction for future research.

Recent research evaluating orthodontic expansion has shown osteoblastic activity on the buccal cortical bone apical to the dehiscence. We hypothesize that the negative effects seen during orthodontic expansion is a result of tipping rather than expansion. The aim of the present study was to produce buccal translation, with little or no coronal tipping, and evaluate hard tissue changes of the dental-alveolar complex. A prospective, randomized, split-mouth study was conducted with 11 patients (average age 14.1 years, range 12.5-16.9 years) requiring maxillary first premolar extractions for comprehensive orthodontic treatment. Pre and post-treatment records included models, photographs, and small field of view cone beam computed tomographic images (FOV CBCT) of the right and left maxillae. One maxillary first premolar was randomly chosen and moved to the buccal with 50 grams of force applied approximately at the tooth's center of resistance. The other premolar served as the control tooth. Forces were re-activated every 3 weeks for approximately 9 weeks of active movement, after which the tooth was held in place for 3 weeks to allow for maturation of the surrounding tissue. Pre and post treatment records were analyzed and superimposed to evaluate changes in the dental-alveolar complex. The results showed significant movement (0.96 mm, p=0.008) of the experimental premolar occurred 3 mm apical to the CEJ. There was minimal buccal tipping (2.2°, p=0.003). Maximum and minimum buccal bone heights decreased 0.60 mm (p=0.003) and 0.25 mm (p=0.262) respectively. The distribution of the maximum bone height measurement was bimodal, with 6 patients showing 0.42 mm (IQR -0.25 mm to 0.52mm) and 5 patients showing 8.3 mm (IQR 7.15 to 10.05) of vertical bone loss. Buccal bone thickness at the midline 3 mm apical to the CEJ decreased 0.63 mm (p=0.016). Based on direct measurements and CBCT superimpositions, buccal bone grew 0.46 mm (p=0.005) and 0.51 mm (p=0.036), respectively. Using light continuous forces, it is possible to produce buccal tooth movement with only limited amounts of tipping.
