Book excerpt: Aujourd'hui, alors que le développement atypique du langage oral est toujours l'objet de nombreux débats, différentes études commencent à s'intéresser à l'évolution des difficultés des enfants dysphasiques. En effet, de nombreux questionnements sont exprimés par les professionnels concernés mais aussi par les familles, soucieuses de l'avenir de leurs enfants. Les orthophonistes sont de plus en plus sollicités pour intervenir auprès des adolescents et jeunes adultes dysphasiques, et notamment pour contribuer aux choix d'orientation scolaire, professionnelle. Pourtant, peu d'outils d'évaluation ou de remédiation concernent spécifiquement cette population. Une étude est ici menée chez douze sujets (sept garçons et cinq filles), âgés de 15,9 ans à 20,5 ans (la moyenne étant de 18 ans, écart-type de 1,5), diagnostiqués dysphasiques à l'Hôpital Clocheville de Tours et ayant suivi un cursus d'intégration scolaire normal. En plus de s'intéresser à leurs parcours personnels, scolaires voire professionnels, l'étude a pour objectif d'évaluer les troubles persistants de la dysphasie concernant les langages oral et écrit. Dans ce but, deux types d'outils sont utilisés : des épreuves processuelles nous permettent tout d'abord d'objectiver les séquelles dans les différents domaines linguistiques et de définir les mécanismes déficients ; en complément, des épreuves dites fonctionnelles nous renseignent sur le degré d'autonomie d'utilisation du langage à partir de supports adaptés de la vie quotidienne. Nous cherchons alors à savoir s'il existe une corrélation entre les capacités linguistiques et leur mise en œuvre dans l'environnement familier. L'intérêt est de pouvoir discuter de la contribution de l'utilisation d'épreuves écologiques dans l'évaluation des adolescents et jeunes adultes, et ce, de façon complémentaire à celle des batteries classiques de tests.

Il existe en effet peu d'outils d'évaluation concernant spécifiquement les adolescents dysphasiques et ces outils apprécient peu l'utilisation quotidienne du langage et, par conséquent, la gêne résiduelle réelle de ces jeunes. L'étude conduite ici concerne 20 jeunes dysphasiques âgés de 10 ans 11 mois à 15 ans 7 mois, ayant reçu le diagnostic de dysphasie à l'hôpital Clocheville de Tours et ayant suivi une scolarité ordinaire. Elle s'inscrit dans la continuité des travaux de Tanguy (2004) et Carmès (2005) et s'intéresse, cette fois-ci, à une population plus jeune, jamais explorée jusque là en ce qui concerne le langage fonctionnel. Elle vise, d'une part, à identifier les difficultés langagières persistantes à l'adolescence et les éventuels moyens de compensation développés, et, d'autre part, à estimer l'utilisation que les jeunes font du langage, oral et écrit, dans des situations réelles. L'évaluation est réalisée grâce à l'utilisation d'une batterie classique, «processuelle» de tests étalonnés pour cet âge, et, d'une série d'épreuves fonctionnelles, également proposée à des jeunes témoins de 11 ans et de 14 ans. Les performances aux épreuves fonctionnelles sont confrontées à celles sur les tests standardisés ainsi qu'aux variables cliniques recueillies pour chaque sujet afin d'évaluer ce qui est lié à la mise en œuvre des capacités linguistiques en contexte. Les résultats aux épreuves fonctionnelles sont également comparés à ceux des sujets témoins de manière à appréhender l'incidence des séquelles langagières des adolescents dysphasiques sur leur autonomie, par comparaison à des sujets tout-venants. Ce travail contribue à mieux définir les troubles caractéristiques de cette population et à apprécier dans quelle mesure les épreuves écologiques apportent des informations complémentaires et offrent de nouvelles pistes de rééducation adaptées à cet âge, comparativement aux épreuves classiques.

Une prise en charge orthophonique intensive permet à l'adolescent et au jeune adulte dysphasique de maîtriser les aspects formels du langage oral. Si la littérature montre des difficultés résiduelles concernant à la fois le langage oral et le langage écrit, peu d'études reflètent leurs impacts sur la qualité de vie de l'adolescent et du jeune adulte dysphasique. Ainsi, nous avons étudié les difficultés résiduelles de 13 sujets dysphasiques âgés de 15 à 20 ans. D'une part, nous leur avons proposé une évaluation concernant le langage élaboré et le langage écrit à travers des tests standardisés (PELEA et Vol du PC). D'autre part, nous leur avons proposé une évaluation plus subjective pour évaluer les répercussions de leur trouble du langage sur l'aspect fonctionnel du langage et sur la qualité de vie, par le biais d'un questionnaire que nous avons créé et proposé en parallèle à des sujets témoins pour établir des normes. Nos résultats montrent que les sujets dysphasiques ont des difficultés durables pour comprendre et utiliser le langage élaboré mais également dans la maîtrise du langage écrit. De plus, nos résultats reflètent un impact des troubles du langage sur l'utilisation fonctionnelle du langage et la qualité de vie. Enfin, cette étude nous a permis d'établir quelques corrélations entre les évaluations standardisée et subjective. Ainsi, ces deux types d'évaluation permettent d'éclairer davantage l'évolution de la dysphasie chez les sujets diagnostiqués et le retentissement de cette pathologie sur leur qualité de vie.

Book excerpt: Although speech-language pathologists are expected to be able to administer and interpret oral examinations, there are currently no screening tests available that provide careful administration instructions and data for intra-examiner and inter-examiner reliability. The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training. The time required to administer the examination typically ranged from 5 to 10 minutes. (Author/GK)
