Cependant, en raison d’un manque de détection, la douleur reste sous traitée, notamment chez les patients déments. Objectif principal : Déterminer comment est évaluée la douleur chez la personne âgée démente en EHPAD, notamment en recherchant si des outils d’hétéro-évaluation adaptés aux capacités cognitives sont utilisés. Méthodes : L’étude a été effectuée dans 8 unités de vie protégées de 4 EHPAD parisiens d’octobre 2012 à janvier 2013. Les dossiers de 170 patients de plus de 65 ans, ayant un diagnostic de démence, ont été étudiés à la recherche de signalements de douleurs. 41 professionnels de santé (infirmiers, aides-soignants, médecins, rééducateurs) ont rempli un questionnaire sur leurs pratiques d’évaluation de la douleur. Résultats : 72% des patients avaient une pathologie ou un soin douloureux et 52% de ces patients recevaient un antalgique. 42 dossiers (25%) contenaient un signalement de douleur dont 2 (5%) contenaient une évaluation par une échelle. Les patients non communicants avaient autant de pathologies douloureuses (p=0,73) et recevaient autant d’antalgiques (p=0,35) que les patients communicants. 73% des professionnels souhaitaient une formation sur la douleur chez la personne démente. 80% des professionnels n’utilisant pas les échelles d'hétéro-évaluation invoquaient le manque de formation comme principal frein. Conclusion : Les échelles d’hétéro-évaluation sont très peu utilisées par les professionnels de santé dans les EHPAD, principalement en raison d’un manque de formation.

Book excerpt: Introduction : Le vieillissement de la population s'accompagne de complications parmi lesquelles on retrouve fragilité, troubles cognitifs, chute et douleur. Afin d'améliorer leur prise en charge, une équipe fixe de gériatres a été mise en place aux urgences du CHU de Limoges. Objectifs: Notre étude avait pour but de comparer la prise en charge de la douleur des patients souffrant de TC versus ceux sans TC par les médecins de la MUPA. Méthodes : Une étude de cohorte unicentrique aux urgences du CHU de Limoges. Les patients avaient été pris en charge par la MUPA du 1er janvier 2015 au 31 mars 2015. Résultats : 177 patients ont été inclus. 33,9% des patients avaient des TC connus à leur admission. 80% des patients ont été évalués par l'ALGOPLUS et 13% par l'EVA. 7% des patients n'ont pas eu d'évaluation de douleur. 66,7% des sujets évalués par ALGOPLUS et 78,3% de ceux évalués par l'EVA, n'avaient pas de TC connu à l'admission. Les patients avec TC connus évalués par ALGOPLUS étaient en moyenne cotés à 0,81 contre 0,98 chez ceux sans TC (p=0,401). Les patients avec TC évalués par EVA cotaient en moyenne à 0,40 contre 0,67 chez ceux sans TC (p=0,635). La différence entrée/sortie était nulle chez les patients évalués par EVA dans les 2 groupes (p=0,310). Avec ALGOPLUS la différence était de 0,53 chez ceux avec TC et 0,24 chez ceux sans TC (p= 0,323). Conclusion : Cette étude montre que les patients âgés admis pour chute et pris en charge par la MUPA, bénéficient d'une prise en charge de la douleur adéquate qu'ils souffrent de troubles cognitifs ou non. La douleur reste un indicateur majeur de bonne prise en charge dans les établissements de santé.
