Book excerpt: L'objectif de ce travail de thèse était d'évaluer l'état des processus de récupération en mémoire dans la maladie d'Alzheimer afin de développer des programmes de prise en charge cognitive novateurs. Les deux premières études ont porté sur la caractérisation de ces processus à travers l'évaluation des performances de patients avec une maladie d'Alzheimer à des tâches de reconnaissance mnésique. Les deux études suivantes se sont intéressées au développement de programmes d'intervention cognitive ayant pour objectif de réduire les troubles cognitifs et leur retentissement, l'un en améliorant les processus cognitifs altérés à partir d'un entraînement cognitif appliquant la répétition-lag procédure et l'autre, en sollicitant les processus cognitifs préservés à partir d'un apprentissage par des techniques de réhabilitation cognitive, à recourir à l'utilisation d'aides externes. Les résultats ont mis en évidence une détérioration sélective des processus de reconnaissance chez les patients à un stade léger de la maladie ; la recollection serait particulièrement altérée alors que la familiarité resterait préservée. De plus, le déficit des capacités de reconnaissance de la source serait lié à l'altération des processus stratégiques de reconnaissance de la source, les processus associatifs resteraient préservés. Ensuite, le programme d'entraînement cognitif expérimental s'est révélé efficace, des gains cognitifs ont été objectivés à des tâches cognitives de transfert. De même, le programme de réhabilitation cognitive par le biais d'un apprentissage combinant les techniques de la récupération espacée et de l'apprentissage sans erreur a permis de réduire l'impact des troubles cognitifs dans la vie quotidienne.

Book excerpt: La mémoire autobiographique (Mau), composante fondamentale de la mémoire humaine, permet de conserver des souvenirs du passé propre à chaque individu. Cette mémoire très importante est altérée dans de nombreuses pathologies neurologiques notamment dans la maladie d'Alzheimer (MA). Nos objectifs visaient à développer un protocole expérimental permettant d'évaluer la mémoire autobiographique chez des patients présentant un déficit cognitif léger (MCl), sujets potentiellement à risque de développer une MA. Nous avons évalué cette mémoire chez 14 sujets MCl et chez 14 sujets contrôles au moyen d'une épreuve standard utilisant un matériel verbal standardisé (test de Kopelman, 1989) et d'une épreuve expérimentale utilisant un matériel visuel (test des photos familières). Ainsi, nous avons évalué cette mémoire chez les sujets MCl en étudiant notamment les processus cognitifs impliqués et les variables qui peuvent l'influencer (matériel verbal et visuel, charge émotionnelle). Sur l'ensemble des épreuves autobiographiques, les sujets MCl obtiennent des performances statistiquement inférieures à celles des sujets contrôles. Il apparaît également que les MCl mettent en jeu des processus cognitifs différents selon la nature du test utilisé: processus de récupération pour le test de Kopelman et processus de reconnaissance pour le test des photos familières. Par contre, ils ne semblent pas présenter de déficit du ressenti émotionnel. Cette atteinte de la Mau serait donc au moins en partie liée à un déficit des processus cognitifs de stockage et de récupération. Cette recherche a permis de révéler l'existence de troubles de la Mau chez les MCl ce qui permettra d'établir une prise en charge thérapeutique plus approprié.

Or, en 2012, Bellassen et al., dans des recherches en neurobiologie, ont obtenu des résultats suggérant que la mémoire de l'ordre serait une compétence atteinte de façon précoce dans le cadre de cette maladie et que l'évaluation de celle-ci aurait « le pouvoir prédictif le plus élevé pour le diagnostic », notamment sur le versant visuo-spatial. A la lumière de ces recherches, nous avons posé l'hypothèse que l'évaluation de la mémoire de l'ordre, dans les différents domaines où elle est à l'oeuvre, pourrait apporter une aide au diagnostic de cette maladie dégénérative. De même, nous nous interrogerons sur les liens entre les domaines verbaux, visuo-spatiaux et moteurs pour savoir si la mémoire de l'ordre se relève d'un processus global ou spécifique à chacun des domaines évoqués. Pour valider cette hypothèse, nous avons créé un test évaluant les versants langagiers, visuo-spatiaux et moteurs en rappel immédiat et différé. Il a été produit, à la fois, une cotation-ordre et une cotation-items. Par la suite, il a été présenté à une population témoin constituée de 13 participants et à une population pathologique de 3 participants. L'analyse quantitative des données recueillies a souligné plusieurs corrélations significatives pour les domaines verbaux et moteurs pour la restitution des informations-ordre et items. Une corrélation significative plus légère a été observée pour le rappel des items sur le versant visuo-spatial. L'analyse qualitative permet de dire que nos observations sont en accord avec le tableau pathologique et le score au MMS-E. A ce jour, les résultats obtenus semblent insuffisants pour nous permettre de valider avec certitude notre hypothèse. Cependant, ils sont très encourageants et nous incitent à poursuivre le travail dans ce sens, tant au niveau de la recherche qu'au niveau de la prise en charge orthophonique.

At this point in time, when the older population is rapidly growing in the United States and across the globe, it is important to examine what is known about cognitive aging and to identify and promote actions that individuals, organizations, communities, and society can take to help older adults maintain and improve their cognitive health. Cognitive Aging assesses the public health dimensions of cognitive aging with an emphasis on definitions and terminology, epidemiology and surveillance, prevention and intervention, education of health professionals, and public awareness and education. This report makes specific recommendations for individuals to reduce the risks of cognitive decline with aging. Aging is inevitable, but there are actions that can be taken by individuals, families, communities, and society that may help to prevent or ameliorate the impact of aging on the brain, understand more about its impact, and help older adults live more fully and independent lives. Cognitive aging is not just an individual or a family or a health care system challenge. It is an issue that affects the fabric of society and requires actions by many and varied stakeholders. Cognitive Aging offers clear steps that individuals, families, communities, health care providers and systems, financial organizations, community groups, public health agencies, and others can take to promote cognitive health and to help older adults live fuller and more independent lives. Ultimately, this report calls for a societal commitment to cognitive aging as a public health issue that requires prompt action across many sectors.
