Cependant, la majorité d'entre eux manquent d'expérience et d'entrainement, surtout en pédiatrie. Dans les pays anglo saxons, la formation des gestes d'urgence par la simulation est devenue obligatoire. En France, il n'y a pas de programme standardisé ni d'outil d'évaluation validé et répandus. Notre étude a comme objectifs de développer un programme de simulation d'enseignement de 4 gestes d'urgence pédiatrique et d'évaluer sa faisabilité, sa validité ainsi que la persistance des compétences dans le temps et en situation d'urgence. Matériel et Méthode : Nous avons conduit une étude prospective, interventionnelle, bi centrique entre les Hôpitaux universitaires de Nice et Nancy, entre les mois de novembre 2013 et novembre 2014. Trente-cinq internes des deux premières années de pédiatrie ont été inclus dans notre programme de simulation. Ils étaient évalués au début de l'étude (M0) puis 1, 3, 6 et 12 mois (M1, M3, M6, M12) sur la pratique de 4 gestes : la libération des voies aériennes supérieures et la ventilation, l'intubation, la pose d'un cathéter intra-osseux et la pratique d'un massage cardiaque. A 12 mois, les gestes étaient évalués dans un contexte de scénario d'arrêt cardiaque. Une évaluation des connaissances théoriques était réalisée au début de l'étude et à 6 mois. Résultats : Entre M0 et M6, on note une amélioration significative des scores théoriques ainsi que des scores pratiques: 8.49 contre 15.91 pour la ventilation ; 5.26 contre 17.79 pour le cathéter intra-osseux ; 6.64 contre 13.33 pour l'intubation ; 11.03 contre 16.52 pour le massage cardiaque, avec des coefficients de corrélation de Spearman significatifs pour chaque geste. A 12 mois, les scores pratiques restent élevés et ne sont pas altérés par les conditions de stress.

Matériel & méthode: Des items techniques en cotation binaire ont été conçus à partir de recommandations internationales, puis validés en multicentrique par 13 séniors experts via la méthode Delphi modifiée. Trente sujets répartis en 5 groupes de niveau : 6 étudiants en deuxième année de médecine, 6 externes, 6 internes de pédiatrie, 6 séniors non experts, 6 séniors experts ont été mis en situation sur mannequin basse-fidélité pour les quatre gestes. L'étude de la fiabilité a consisté en la comparaison des scores de chaque item donnés par deux évaluateurs, l'un en direct, et l'autre par retransmission vidéo. Pour l'étude de la validité des grilles, les scores médians obtenus par les cinq groupes de niveau ont été comparés. Résultats: La fiabilité de nos grilles est démontrée, avec un accord entre les deux évaluateurs parfait ou presque parfait pour 82,5 % des items. Les scores médians des groupes pour les quatre grilles augmentent de manière significative en fonction du niveau d'expertise. Conclusion: Nos grilles RESCAPE sont des outils fiables et valides. Elles pourront être intégrées à des programmes de formation par simulation afin d'en évaluer l'efficacité, ou pourront être utilisées dans l'évaluation des performances de professionnels.

Book excerpt: Introduction : l'enseignement par la simulation a montré son efficacité dans la formation médicale, notamment pour la prise en charge des urgences vitales. Cette technique a été intégrée partiellement depuis plusieurs années dans la formation initiale des internes de pédiatrie de Bordeaux. Notre objectif est de proposer un programme d'enseignement par la simulation pour les internes de pédiatrie. Méthode : un état des lieux national sur l'enseignement par la simulation pour les internes de pédiatrie a été réalisé à l'aide d'un questionnaire. Les compétences des internes de pédiatrie dans la prise en charge des urgences vitales de l'enfant ont été évaluées par deux études : l'une a évalué les connaissances de la réanimation de base de l'arrêt cardiaque chez tous les internes de pédiatrie bordelais ; l'autre a évalué les capacités de team leader des internes en fin de cursus, lors d'une séance de simulation immersive. Résultats : l'ensemble des subdivisions françaises proposent une formation par la simulation aux internes de pédiatrie mais de grandes disparités persistent. Les connaissances des internes de pédiatrie bordelais dans la prise en charge initiale de l'arrêt cardio respiratoire sont insuffisantes. Les compétences techniques et non techniques en tant que team leader dans la prise en charge d'un arrêt cardio respiratoire intra-hospitalier étaient insuffisantes chez les internes de dernière année. Conclusion : en s'appuyant sur l'expérience des autres subdivisions et sur les besoins évalués, nous proposons un programme d'enseignement alliant formation théorique et pratique mêlant plusieurs types de simulation adaptés à chaque objectif d'apprentissage sur une durée de cinq ans.
