Toutefois, la littérature scientifique médicale manque de données solides sur ce sujet. Nous avons comparé la corrélation entre les mesures oscillométriques au brassard et les mesures intra-artérielles de la pression artérielle chez les patients en arythmie, par rapport aux patients en rythme sinusal régulier. Les patients de 3 services de réanimations ont bénéficié de trois paires de mesures intra-artérielles et au brassard automatique qui ont été respectivement moyennées. L'ensemble des patients souffrant d'arythmie disposant d'un cathéter intra artériel a été inclus de manière consécutive. Après chaque inclusion dans le groupe arythmie, un patient en rythme sinusal régulier était inclus en tant que témoin. La validation de la norme ISO exigeait qu'un biais moyen et sa déviation standard (DS) devaient être 5 et 8 mmHg, respectivement. Chez 135 patients souffrant d'arythmie, concernant la pression artérielle systolique (PAs), la pression artérielle diastolique (PAd) et la pression artérielle moyenne (PAm), la corrélation entre la mesure oscillométrique au brassard et la mesure intra-artérielle était similaire à celle observée chez 136 patients avec un rythme régulier: pour la PAm comparativement, le biais moyen ± DS (-0,1 ± 5,2 et 1,9 ± 5,9 mmHg). Dans les deux groupes, la norme ISO était satisfaite pour la PAm et PAd, mais pas pour la PAs (DS 10 mmHg) dans notre population de réanimation. La capacité du brassard oscillométrique pour détecter l'hypotension (PAs 90 mmHg ou PAm 65 mmHg), la réponse au traitement (augmentation de 10% la PAm après une intervention hémodynamique) et de l'hypertension (PAs 140 mmHg) était bonne et similaire chez les patients de réanimation en arythmie et en rythme sinusal régulier (Aires sous la courbes ROC respectivement de 0,89 et 0,96, chez les patients en arythmie par rapport aux patients en rythme sinusal régulier, avec p > 0,3). Contrairement à la croyance largement répandue, l'arythmie n'altère pas de façon significative la précision de la mesure au brassard automatique (oscillométrique).
