Book excerpt: En Midi-Pyrénées, le réseau DIAMIP propose des programmes d'éducation thérapeutique pour patient diabétique de type 2 en échec thérapeutique, en hospitalisation. Dans un souci d'amélioration de leurs pratiques, quinze équipes de ce réseau ont voulu évaluer la qualité de ces programmes. Elles ont choisi une méthodologie validée par la Haute Autorité de Santé : une Evaluation de Pratiques Professionnelles de type audit clinique ciblé. Trois catégories de critères ont été évaluées: la démarche éducative auprès du patient, l'organisation du programme et la qualité du courrier de sortie adressé au médecin traitant. Deux visites sur chaque site ont été réalisées à 1 an d'intervalle. Au total 400 dossiers- patients sur 95 stages ont été traités. A l'issue de la visite 1, 11 critères sur 19 étaient satisfaits dans plus de la moitié des dossiers alors qu'à l'issue de la visite 2, 17 critères sur 19 sont satisfaits dans plus de trois-quarts des dossiers traités. Ce qui montre une nette amélioration des résultats. Cette progression est liée avec la mise en place d'actions d'amélioration dont la création de nouveaux outils. La qualité du courrier de sortie adressé au médecin traitant a été l'un des points défaillants révélés par ce travail. Ce courrier est essentiel pour la continuité des soins et la pérennisation de la démarche éducative auprès du patient.
