Book excerpt: Introduction : Bon nombre de nos confrères expriment des difficultés pour trouver un médecin remplaçant. Notre étude avait pour objectif d’évaluer ce degré de difficulté en Picardie et d’identifier les freins et leviers dans la recherche d’un médecin remplaçant. Matériel et Méthode : 500 médecins généralistes libéraux picards tirés au sort ont été interrogés sur la base d’un questionnaire anonyme. 136 d’entre eux y ont répondu. Résultats : Une majorité des médecins de l’enquête (52%) a indiqué qu’il était très difficile de trouver un médecin remplaçant, 40% des médecins ont fait savoir que c’était moyennement difficile et seulement 8% ont déclaré que c’était pour eux facile. Une large majorité des médecins (64%) a précisé ne pas toujours trouver de remplaçant. Les médecins de l’étude avaient besoin d’être remplacés en moyenne 47 jours par an, leur cabinet étant fermé en moyenne 23 jours par an faute de remplaçant. Discussion : Le fait d’exercer en cabinet groupé ou en MSP, la qualité de maître de stage, un nombre élevé de consultations, la mise à disposition d’un secrétariat, ainsi qu’un besoin élevé d’être remplacé, ont été identifiés comme des facteurs facilitateurs pour la recherche d’un remplaçant. En revanche aucune disparité significative dans la difficulté à trouver un remplaçant n’existait entre les médecins selon leur département ou milieu d’exercice (rural, semi-rural ou urbain). De même, le taux de rétrocession élevé, la mise à disposition d’un logement, un faible nombre de visites à domicile, la garanti d’un minimum de consultations, n’influençaient pas le degré de la difficulté ressentie. Conclusions : Notre étude a confirmé qu’il était particulièrement difficile de trouver un médecin remplaçant en Picardie.

Face à des difficultés d'accès aux soins primaires, les CPTS ont pour mission de faciliter l'accès au médecin traitant sur leur territoire. Objectifs : évaluer la perception du rôle de médecin traitant et les freins à la prise de nouveaux patients par les médecins généralistes, afin de proposer des outils pour favoriser l'accès au médecin traitant sur le territoire de la CPTS Initiatives Santé. Méthodes : étude qualitative par entretiens semi-dirigés auprès de médecins généralistes installés sur le territoire de la CPTS Initiatives Santé. Neuf entretiens ont été menés puis retranscrits et analysés par théorisation ancrée. Résultats : le médecin traitant est perçu comme un médecin de famille institutionnalisé. Outre la pénurie de médecins généralistes, la gestion des visites à domicile et des soins non-programmés ainsi que les exigences administratives sont les principaux freins à l'acceptation de nouveaux patients. L'exercice coordonné, la délégation de tâches et les outils numériques font partie des solutions pour améliorer l'accès au médecin traitant sur le territoire. Conclusion : les lois de santé publique de 2016 et 2018 ont pour ambition de déployer les outils qui permettront d'améliorer l'accès aux soins primaires. Les résultats des entretiens ont permis d'élaborer une stratégie à déployer au sein de la CPTS Initiatives Santé pour faciliter l'accès au médecin traitant spécifiquement sur ce territoire.
