Book excerpt: It is with great pleasure that I write this Foreword to the Proceedings of the International Conference on Behçet’s Disease which was held in Berlin in June 2002. This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.

Cet anti métabolite présente une activité anti inflammatoire spécifique (10 à 25 mg/sem) justifiant de son utilisation large en monothérapie ou en association avec d'autres biothérapies dans le cadre de protocoles d'intensification thérapeutique. La survenue de mélanomes sous monothérapie par méthotrexate de patients atteints de PR soulève la question de l'imputabilité de ce traitement. L'objectif principal de notre étude a été de décrire la survenue de mélanomes malins chez des patients atteints de polyarthrite rhumatoïde traités en monothérapie par méthotrexate et de rechercher dans la littérature les éléments d'imputabilité afin d'adapter la prise en charge des ces patients. Méthode: Nous avons donc analysé et comparé aux données de la littérature 6 cas de patients atteints de polyarthrite rhumatoïdes ayant présenté une apparition ou la récidive de mélanomes après l'introduction de leur traitement par méthotrexate. Résultats: Nous avons observé 2 cas de mélanomes primitifs, 3 cas de lésions secondaires malignes de mélanomes primitifs antérieurs à la mise en route du traitement par méthotrexate et 1 cas de non récidive. 4 mélanomes superficiels extensifs, 1 mélanome Choroïdien et 1 lentigo malin de Dubreuilh. La durée moyenne d'exposition au méthotrexate pour les 6 patients était de 18,2 mois [min 1 mois; max 35 mois]. Conclusion: Les articles étudiant des relations entre méthotrexate, mélanome et polyarthrite rhumatoïde sont peu nombreux. Entre 1997 et 2013 seuls 6 articles décrivaient les liens possibles entre ce traitement immunomodulateur et la survenue de mélanome chez des patients atteints de polyarthrite rhumatoïde. Un article de 2008 portant sur une population australienne décrit une augmentation du risque relatif de mélanome sous méthotrexate chez les patients atteints de polyarthrite rhumatoïde en comparaison avec la population générale. Notre étude soulève le bienfondé d'un observatoire spécifique avec une analyse croisée des registres de patients atteints de mélanomes et de ceux atteints de polyarthrite rhumatoïde qui pourrait ainsi évaluer un groupe plus important de patients. Suite aux données de la littérature, nous pouvons recommander chez les patients atteints de polyarthrite rhumatoïde un bilan initial cutané précédent l'initiation du méthotrexate, puis un suivi dermatologique annuel ou plus rapproché, adapté à leurs facteurs de risques de mélanome.
