Book excerpt: Study to review the occupational health concerns of workers exposed to waste anesthetic gases in hospitals and determine whether the findings in the liturature are supported; to review the waste anesthetic gas exposure levels in Ontario hospitals; and to recommend corrective measures to control workers' exposure. The study was conducted from 1981-85 through a questionnaire survey in 100 hospitals throughout Ontario. A walk-through survey of all locations of potential exposure was conducted by an industrial hygienist in a representative sample of 74 hospitals providing surgical services. Air sampling was conducted, observing operating room protocol, and including long-term air sampling for halocarbon vapours (halothane, enflurane and isoflurane), and nitrous oxide sampling by direct reading instruments. The industrial hygiene assessments included an evaluation of the mechanical dilution ventilation, the scavenging system and documentation of the equipment used to administer anesthesia at the time of the assessment.

Book excerpt: The standard applies to all workers, who are exposed to inhalation anesthetic agents that escape into locations associated with the administration of, or recovery from anesthesia. It is designed to protect the health and safety of workers during their working lifetime in locations where exposure to waste gases and vapours occur. The recommended permissible levels of exposure contained in the following sections, however, cannot be defined as safe levels since information on adverse health effects is not completely definitive and many unknown factors still exist. Therefore, the environmental limits presented should be regarded as the upper boundry of exposure.

Book excerpt: Veterinarians and their assistants should consider the potential environmental hazards of chronic exposure to gaseous anesthetics. In humans, epidemiologic surveys have circumstantially shown adverse health effects following exposure to anesthetic gases. These effects include abortion, congential abnormalities, hepatic and renal diseases, CNS disturbances, increased risk of cancer and decreased fertility. In animals, similar effects due to occupational exposure levels of halothane and nitrous oxide have been experimentally established. Previous investigations regarding this personal and personnel health hazard have all concerned medical and dental personnel. There are no documented reports of the potential hazard to veterinary personnel. Therefore, the present survey was conducted to determine the exposure levels of halothane and nitrous oxide in selected USAF veterinary surgeries. In this survey, 11 of 35 halothane samples and 15 of 20 nitrous oxide samples exceeded the maximum exposure levels recommended by NIOSH for those compounds. A potential hazard to the health of personnel working in USAF veterinary surgeries exists. A complete waste anesthetic gas management program and a periodic monitoring program should be maintained. Book excerpt: A complete waste anesthetic gas management program includes application of a well designed waste anesthetic gas scavenging system, anesthetists using work practices which minimize gas leakage, and application of a routine equipment maintenance program so gas leaks are minimized. Work practices of the anesthetist may contribute from 94% to 99% of all waste anesthetic gases in an operating room equipped with properly designed and maintained scavenging components. Improper practices, such as poor choice of the face mask, insufficiently inflated endotracheal tubes, and spillage of volatile anesthetic agents when filling vaporizers, are the chief contributors. Once the proper equipment has been installed, and proper work practices have been established, the Environmental Health Section should initiate a periodic monitoring program geared to detect leaks and the Medical Equipment Section should initiate a compatible preventive maintenance program. This report provides anesthesia-oriented personnel with information necessary to minimize occupational exposure to inhalational anesthetics. (Author).

Book excerpt: Nitrous oxide and halothane, in respective concentrations as low as 50 parts per million (ppm) and 1.0 ppm, caused measurable decrements in performanceon some psychological tests taken by healthy male graduate students. Nitrous oxidealone caused similar concentrations of anesthetics were visual perception, immediate responses required in a task of divided attention to simultaneous visual and auditory stimuli. These effects were absent in subjects exposed to 25 ppm N20 and o.5ppm halothane. Findings from this study suggest that N20 and haloghane levels in the ambient air of hospital operating rooms should not exceed 25 pp. N20 and 0.5ppm halothane.

Book excerpt: This dissertation is a compilation of studies related to the halogenated anesthetic gas isoflurane. Historically, halogenated anesthetic gases have been used in the health care industry. In 1977 the National Institute for Occupational Safety and Health (NIOSH) issued a recommended exposure limit (REL) of two parts per million (ppm) averaged over one hour of exposure for halogenated anesthetic gases (NIOSH 1977). The purpose of the standard was to protect healthcare workers from exposure to halothane, methoxyflurane, and chloroform. However, isoflurane only became available after the NIOSH REL was adopted. Therefore, the NIOSH REL is not directly applicable to isoflurane. Moreover, use of isoflurane in healthcare has diminished over the years, and it is now more widely used in medical research laboratories and veterinary clinics. The purpose of this dissertation is to demonstrate the need for an updated occupational exposure limit for isoflurane.
