La relation patient-médecin englobe deux composantes ; émotionnelle et cognitive qui inclut le recueil des informations médicales. Les objets connectés médicaux (OCM) et la télémédecine sont des moyens possibles d'évolution de la prise en charge des patients, tout en maîtrisant les coûts de santé. Objectifs. Évaluer si l'utilisation d'OCM pour la mesure de la PA entraine d'éventuelles modifications de la relation patient-médecin en médecine générale. En objectifs secondaires : identifier les avantages et les inconvénients de leur utilisation et leur rôle dans le partage des tâches professionnelles. Méthode. Étude observationnelle quantitative conduite auprès de 470 médecins généralistes (MG) français, le 10 et 11 juin 2016 lors d'un congrès national de MG. L'OCM de mesure de la pression artérielle a servi d'exemple. Résultats. 177 MG ont complété le questionnaire, 66% n'avaient jamais recommandé d'OCM mais les 2/3 étaient prêts à le faire à l'avenir. La majorité pensait que la relation patient-médecin en serait modifiée et pour 65% de manière positive. Les MG avaient des avis globalement positifs sur la fiabilité, le partage des informations, l'autonomie du patient et le dialogue avec lui. Les freins identifiés concernaient la responsabilité médico-légale, le temps passé et l'aspect financier (rémunération et remboursement du matériel). Conclusion. Nos participants MG sont prêts à partager plus d'informations avec le patient et favorables à ce que les patients prennent leur santé en main. Ils seraient aussi favorables à une redistribution des compétences professionnelles.

Cependant, leur place mal définie ne favorise par leur usage dans la pratique médicale actuelle. l'objectif principal était la quantification du taux d'utilisation actuel des OCS par les médecins généralistes (MG). les objectifs secondaires étaient l'évaluatio de leurs atouts et des freins à leur utilisation. Matériel et méthodes: étude descriptive observationnelle par questionnaire adressé par email aux MG de France via des unions régionales et des organismes de formatio continue, complété d'un site web informatif, entre le 10 mars et le 10 juillet 2015. les critères d'exclusion étaient un remplissage insuffisant du questionnaire et l'absence de réponse à la question principale. Résultats: Sur 1086 réponses reçues, 1084 réponses de MG ont été analysées. 19,5% (IC95% [17,1 - 21,8]) MG utilisaient au moment de l'étude un OCS, versus 81 ,5% (IC95% [78,2 - 82,9],) qui n'en utilisaient pas. Seulement 10,2% (IC95% [8,5 - 12,1]) MG en prescrivaient. leur intérêt était souligné dans le suivi (pou 85,0%) et l'éducation du patient (pour 75,8%), notamment dans le diabète (89,7%) et l'hypertension artérielle (84,1%). les données générées devraient être gérées de manière sécurisée par le patient en priorité pour 85,8% des MG. Elles ne devraient pas contraindre le MG en dehors de la consultation clinique, ni restreindre leur disponibilité en temps pour 75,8%. l'adjonction d'intervenants extérieurs n'était pas souhaitée pour plus de 80% des MG. Conclusion: les OCS sont peu utilisés par les MG. leur bénéfice en tant qu'outils de responsabilisation et d'autonomisation du patient en complément de la relation médecin - patient clinique reste à démontrer

La pandémie de covid-19 a favorisé l’essor de la télémédecine. Objectif Identifier les motivations et les freins des patients à l’utilisation de ces objets. Méthodes Enquête qualitative multicentrique par entretiens individuels semi-dirigés menés entre janvier 2019 et mai 2021, auprès de 16 patients, sélectionnés par échantillonnage non probabiliste volontaire, puis raisonné, jusqu’à suffisance des données. Une analyse de contenu thématique a été réalisée. Résultats La définition d’un objet connecté en santé était mal connue des patients. La plupart en détenait pourtant au moins un. Les patients y voyaient un moyen de se représenter leur santé, de façon ludique. Leurs freins étaient la sécurité des données, le prix, mais aussi la dépendance et l’angoisse qu’ils génèrent, les surcouts d’une telle surmédicalisation, l’altération de la relation et le manque de réassurance en téléconsultation. Ils y voyaient un moyen de consulter rapidement pour des motifs de consultation qui ne les inquiétaient pas. Discussion Les objets connectés et les téléconsultations sont promus par les plateformes privées pour améliorer la santé. Ce travail nous montre les angoisses, les retards de diagnostics, la déshumanisation et les surcouts associés à leur utilisation. On peut discuter de l’apport en terme de santé et d’économie de ces outils amenant souvent à une multiplicité des consultations. Des études complémentaires seraient pertinentes pour évaluer s’ils apportent réellement une valeur ajoutée en terme de santé aux patients.

Book excerpt: Introduction : l'émergence massive des objets connectés en santé est en train de remanier la façon d'appréhender la prise en charge globale du patient. Ces vecteurs d'information sont répartis en deux catégories, à visée de bien/être et à visée médicale. Ces nouvelles TIC peuvent répondre à certains freins rencontrés dans la prise en charge ambulatoire des pathologies chroniques. C'est dans cette optique que notre travail a interrogé la littérature scientifique sur l'intérêt des objets connectés en soins primaires. Méthode et matéeiel : via la base de données Medline (PubMed), notre travail a porté sur une revue de la littérature incluant des revues, des méta-analyses et des essais contrôlés randomisées ou non ayant évalué l'impact de l'usage d'un dispositif connecté en terme d'efficacité clinique, ou biologique ou thérapeutique pour les nosologies ayant une prévalence élevée en médecine générale. Résultats : notre analyse a inclus 25 publications: trois revues, 2 méta-analyses, 19 études contrôlées randomisées et 1 étude contrôlée non randomisée. Dans le cadre de la prévention cardiovasculaire, les résultats ont été significatifs pour la perte de poids, le contrôle l'hypertension artérielle, la correction des dyslipidémies, l'amélioration de l'asthme partiellement controlé ou non controlé. Ces programmes d'intervention ont également un impact positif sur l'observance thérapeutique, et un intérêt pour limiter l'inertie thérapeutique. En outre, les outils ont été en faveur d'une aide de diagnostic clinique des poussées d'insuffisance cardiaque chronique, permettant une prise en charge précoce et limiter ainsi les ré hospitalisations. Par ailleurs, la discordance de certains essais sur le diabète, était due à la compliance moindre de l'auto surveillance glycémique chez des patients DT2 sous anti diabétique oral seul (chez qui l'indication est restreinte) ou due à des effets indésirables cutanés liés au dispositif. Discussion :l'indication de ces TIC nécessite de définir un profil de patient, pour un bénéfice dans le projet de soins. En outre, l'impact de ces capteurs n'est favorable que si une rétroaction y est associée, permettant ainsi une réévaluation clinique, biologique ou thérapeutique. Les données issues de ces vecteurs prennent ainsi une utilité médicale que si elles sont interprétées par un médecin. Toutefois, l'usage à visée médicale doit être encadré : évaluation de validation, de pertinence, de fiabilité, et de protection des données. Conclusion : ce travail intègre l'usage des objets connectés en santé dans une médecine 4P. La prévalence des maladies chroniques et la démographie médicale suscitent une nouvelle façon de penser notre système de soins. Devant ces besoins, ces nouveaux outils auront un impact sur la pratique médicale. Leur émergence dépendra de la formation des patients et des soignants, de leur validation technologique et médicale, des enjeux réglementaires ainsi que du coût lié à leur usage.

Conclusion : L'utilisation des outils numériques en santé n'en est qu'à ses débuts, mais pourrait annoncer une réelle révolution de la pratique médicale. La vigilance doit cependant être de mise sur la sécurisation des données recueillies et leur éventuelle utilisation à des fins détournées de la seule prise en charge de la santé du patient.
