Book excerpt: Le diagnostic de dysphasie entraîne des retentissements sur la vie entière du sujet, puisqu'il s'agit d'un trouble structurel, spécifique, sévère et durable du langage oral (affectant également le langage écrit). Une prise en charge orthophonique intensive permet à l'adolescent et au jeune adulte dysphasique de maîtriser les aspects formels du langage oral. Si la littérature montre des difficultés résiduelles concernant à la fois le langage oral et le langage écrit, peu d'études reflètent leurs impacts sur la qualité de vie de l'adolescent et du jeune adulte dysphasique. Ainsi, nous avons étudié les difficultés résiduelles de 13 sujets dysphasiques âgés de 15 à 20 ans. D'une part, nous leur avons proposé une évaluation concernant le langage élaboré et le langage écrit à travers des tests standardisés (PELEA et Vol du PC). D'autre part, nous leur avons proposé une évaluation plus subjective pour évaluer les répercussions de leur trouble du langage sur l'aspect fonctionnel du langage et sur la qualité de vie, par le biais d'un questionnaire que nous avons créé et proposé en parallèle à des sujets témoins pour établir des normes. Nos résultats montrent que les sujets dysphasiques ont des difficultés durables pour comprendre et utiliser le langage élaboré mais également dans la maîtrise du langage écrit. De plus, nos résultats reflètent un impact des troubles du langage sur l'utilisation fonctionnelle du langage et la qualité de vie. Enfin, cette étude nous a permis d'établir quelques corrélations entre les évaluations standardisée et subjective. Ainsi, ces deux types d'évaluation permettent d'éclairer davantage l'évolution de la dysphasie chez les sujets diagnostiqués et le retentissement de cette pathologie sur leur qualité de vie.

Malgré les biais que nous avons pu rencontrer lors de notre étude, nous pouvons en déduire que le groupe est une modalité efficace et pertinente pour travailler la compétence pragmatique et les habiletés sociales.

En effet, les problématiques qu'ils vivent peuvent nuire à leur réussite académique et mener au décrochage scolaire sans l'obtention d'un diplôme (Franc et Gérard, 2004). Elles dérangent également le développement de leur autodétermination, un facteur de protection du décrochage scolaire (Blanchard, Pelletier, Otis et Sharp, 2004), qui regroupe les habiletés et attitudes d'une personne lui permettant d'agir de manière autonome sans être influencée par les autres (Wehmeyer et Lachapelle, 2006). Les parents peuvent adopter des comportements qui soutiennent ou non ce développement. En plus de devoir être adapté aux caractéristiques et capacités individuelles de l'adolescent (Wehmeyer et Lachapelle, 2006), ce support doit toutefois être perçu positivement par celui qui en est témoin pour être qualifié d'efficace (Connell et Wellborn, 1991; Ryan et Grolnick, 1986; Steinberg et al., 1994). Or, on en sait peu sur les particularités de ce soutien parental auprès d'adolescents ayant une dysphasie et sur leur point de vue à cet égard. S'inspirant du cadre épistémologique interprétatif, cette recherche a permis de dresser un portrait des comportements adoptés par des parents pour soutenir le développement de l'autodétermination de leur adolescent dysphasique et de connaitre leur opinion sur les comportements cités. Au total, quatre adolescents dysphasiques et six parents ont participé à la recherche. Les adolescents ont d'abord été invités à remplir l'échelle d'autodétermination du Laboratoire de Recherche Interdépartementale en Déficience Intellectuelle (LARIDI) (Wehmeyer, Lachapelle, Boisvert, Leclerc et Morrissette, 2001) afin de déterminer leur niveau moyen d'autodétermination. Des entrevues individuelles semi-dirigées ont ensuite été réalisées avec eux et avec leurs parents afin de relever les comportements parentaux adoptés pour supporter l'autodétermination des adolescents rencontrés et l'opinion qu'ils en ont. Enfin, un journal de bord a été utilisé pour décrire, entre autres, la méthodologie déployée et les stratégies exploitées pour faciliter la compréhension et la participation des adolescents aux différentes étapes de la recherche (Boutin, 1997). La technique d'analyse inductive modérée (Savoie-Zajc, 2011) fut retenue afin de créer des catégories conceptualisantes des données recueillies (Blais et Martineau, 2006). Ces catégories sont basées sur l'ancrage théorique choisi, soit la théorie de l'autodétermination (Deci et Ryan, 1985) et sur les quatre caractéristiques définies par Wehmeyer et ses collaborateurs (2001) qui y sont liées : l'autonomie, l'autorégulation, l'empowerment psychologique et l'autoréalisation. Ainsi, de nombreux comportements parentaux adoptés pour supporter le développement de ces quatre caractéristiques ont pu être répertoriés. Par ailleurs, l'opinion des adolescents consultés sur ces comportements parentaux s'est révélée être favorable, ces derniers indiquant apprécier la majorité d'entre eux. Des tendances se dégagent de l'analyse des résultats et permettent d'établir deux principaux constats. D'une part, il appert que les parents supportent conjointement le développement des compétences langagières de leur adolescent et leur autodétermination. D'autre part, les résultats montrent que ces derniers accordent une importance particulière au développement de la confiance en soi et à la gestion de l'anxiété de leur adolescent, deux comportements associés à leur empowerment psychologique (Wehmeyer et al., 2001). Cette recherche a également permis de faire trois recommandations aux parents pour soutenir davantage le développement de l'autodétermination de leur adolescent. Afin de bonifier ce support, les résultats font ressortir l'importance pour les parents : 1) d'optimiser le développement de l'autonomie de leur adolescent; 2) d'établir des objectifs à court, moyen ou long terme avec lui et 3) de l'aider à connaitre ses faiblesses pour mieux les affronter.
