Book excerpt: Contexte : En dépit du 4ème plan de lutte contre la douleur qui vise à favoriser l'offre de proximité pour la douleur chronique (DC), il n'y a pas de structure dévolue en Corse. Présente chez 15% de la population française adulte, la DC est insuffisamment traitée et évaluée. Objectif principal : recueillir les modalités d'utilisation des antalgiques de paliers III dans la douleur chronique non cancéreuse (DCNC). Objectifs secondaires : identifier les facteurs d'influence intervenant dans leur prescription, recueillir les besoins des médecins généralistes de Corse, et proposer un guide pratique. Méthodologie : enquête descriptive de pratique reposant sur un questionnaire quantitatif, ciblant les médecins généralistes libéraux en exercice de Corse. Résultats : Sur 30 médecins inclus, 93,3% étaient prescripteurs d'antalgiques de paliers III devant une DCNC. Ils craignent avant tout un rapport bénéfice risque défavorable. Les formes LP sont les plus employées (fentanyl, notamment après 75 ans). Certaines situations nécessitent une utilisation hors AMM. Les échelles d'évaluations ne sont pas systématiquement utilisées, contrairement au ressenti et à la qualité de vie. Le recours à l'algologue en cas d'échec n'est pas systématique. Malgré les effets secondaires, il y a peu d'arrêt de traitement. Le manque de temps, de formation et de structures de soins dédiées à la DC sont les facteurs gênant la gestion de la DCNC les plus souvent cités (56,6% des médecins interrogés). Conclusion : L'emploi de palier III dans la DCNC est usuel malgré les craintes. Les médecins réclament des structures de soins pour les épauler et expriment le besoin de formation pour parfaire leur prise en charge.

Book excerpt: Objectif: Connaître les habitudes de prescription des opioïdes chez les patients âgés de plus de 75 ans, en médecine ambulatoire, en cas de douleurs récentes intenses ou exacerbation de douleurs chroniques. Matériel et méthodes: Etude quantitative de type descriptive transversale réalisée entre juillet et août 2015 en Mayenne, Sarthe et du Maine et Loire. Un questionnaire en ligne a été envoyé aux médecins généralistes installés dans ces départements. La population étudiée comprenait 600 médecins en activité. Résultats: 190 médecins généralistes ont répondu (31%). 61% se disaient à l'aise avec la prescription d'opioïdes chez les patients âgés en fin de vie. Ils étaient peu nombreux à prescrire des morphiniques en cas de douleurs intenses récentes (26%) et de douleurs liées aux soins (29%). 28% prescrivaient des morphiniques hors AMM. Les 3 principaux freins à la prescription de morphiniques étaient: les effets secondaires, les comorbidités et la polymédication. Les professionnels étaient 67% à avoir déjà bénéficié d'une formation à la prise en charge de la douleur. Le mode de formation le plus fréquent était la « journée douleur » pour 67% d'entre eux. L'outil le plus demandé restait le site internet informatif. Conclusion: Il existe une des améliorations dans la prise en charge de la douleur et de l'utilisation des opioïdes chez les sujets âgés de plus de 75 ans en médecine de ville. Les médecins généralistes étaient mieux formés qu'il y a quelques années, mais restaient demandeurs d'outils de formation. Certains pourraient être proposés: guide de bonnes pratiques ou brochure avec coordonnées des correspondants.
