L' arrivée du médicament générique sur le marché français s' inscrit dans ce contexte de difficultés socio-économiques et a permis de générer une économie de près de 13,4 milliards d' euros entre 2000 et 2012. Il ne fait pourtant pas l'unanimité auprès des patients qui le pensent moins efficace, plus dangereux. La délivrance d'une information est donc indispensable à sa bonne diffusion et acceptation. Mais qui sont les patients majoritairement réfractaires à leur prescription ? Existe-t-il des facteurs influençant ce refus? L'objectif de cette étude est de mettre en évidence les facteurs socio-démographiques (SD) associés au refus. Méthodologie: Une enquête déclarative a été menée d'août à octobre 2014 dans 5 cabinets de médecine générale de la ville de Sète (Hérault). Un questionnaire, comportant des questions sur le refus ou non de la prescription du médicament générique et certaines caractéristiques SD, était rempli par tous les patients de plus de 18 ans, volontaires se présentant en consultation quel que soit le motif. Une différence des caractéristiques SD était mise en évidence entre le groupe pour et le groupe contre les médicaments génériques à l'aide du Chi 2. Le logiciel Epilnfo 7 a été utilisé pour l'analyse statistique. Résultats: Sur les 506 patients ayant volontairement répondu au questionnaire, 29,3 % refusaient la prescription de médicaments génériques. Les facteurs associés à ce refus étaient: être une femme: OR= I,98, IC95 [1 ,24 - 3,17],70 p =0.0044; avoir plus de 60 ans: OR=1.82, IC95 [1.20-2.78], p=0.0058; ne pas avoir fait d'études supérieures: OR= 1.57, IC95 [1.06-2.42], p=0.042. Conclusion: Le médicament générique est une source d'économie importante. Néanmoins, presque un patient sur 3 déclare les refuser. Le médecin généraliste doit fournir une information adaptée à chaque profil, afin que la substitution soit mieux acceptée. Par ailleurs, le rôle du médicament générique en tant qu'acteur de soins primaires et de santé publique, doit être renforcé par les pouvoirs publics

C'est un département qui accueille de nombreuses personnes de nationalité étrangère. Objectifs : ce travail de recherche décrit les résultats d'obtention des droits sociaux et le recours au médecin généraliste après une consultation aux Urgences, par les patients consultant à la PASS de l'hôpital Delafontaine à Saint- Denis. Matériel et méthode : il s'agit d'une étude quantitative, descriptive, prospective et analytique univariée et multivariée, qui concerne les patients consultant aux Urgences de juillet à novembre 2018, associés à la création d'un dossier à la PASS. Les patients sont rappelés trois mois après leur consultation pour évaluer leur accès aux droits sociaux et leur suivi en ville par un généraliste. Résultat : 285 patients sont intégrés dans l'étude : ce sont majoritairement des hommes (67%), entre 20 et 40 ans (57%), originaires d'Afrique Sub-saharienne (30%) et du Maghreb (29%), en situation irrégulière (60%), sans revenus (45%), familialement isolés (65%), en France depuis plus de trois mois (67%) et sans couverture sociale (80%). Parmi ces 285 patients, 110 ont pu être contactés à trois mois : 10% n'ont pas de procédure de couverture sociale en cours. 38% ont des droits ouverts. 61% ne consultent pas de médecin de ville. 20% sont suivis par un médecin généraliste et 17% le sont dans le cadre des consultations « précarité » de la PASS dédiée. Discussion et conclusion : la PASS des Urgences permet l'ouverture des droits mais ne permet pas l'accès au système de santé classique (suivi par un médecin généraliste). Pour améliorer la prise en charge globale des personnes sans droits, l'orientation vers un généraliste après un passage aux Urgences, via une PASS dédiée systématique, semble nécessaire pour éviter les consultations multiples aux Urgences et permettre de débuter un suivi généraliste sans attendre l'AME ou la CMU.
