Book excerpt: 96 patients séropositifs pour le VIH, avec moins de 200 T-CD4 par ml (des charges virales diverses) et naïfs de tout traitement antiviral, ont été pris en charge entre le 1er janvier 2000 et le 31 décembre 2003 dans le service de médecine interne de l'hôpital de Bicêtre. L'étude rétrospective du suivi de ces patients sur une période de 4 ans a permis de faire quelques observations. Le nombre de femmes parmi les patients augmente, et il s'agit surtout de femmes originaires d'Afrique sub-saharienne contaminées par voie hétérosexuelle. Grâce à la mise sous traitement antiviral, à 6 mois, plus de 80 % des patients suivis ont une charge virale devenue indétectable (inférieure à 400 copies par ml), et 53 % des patients suivis ont un nombre de T-CD4 supérieur à 200 par ml. Même chez des patients très immunodéprimés, une prise en charge efficace permet d'obtenir de bons résultats virologiques et immunologiques.

Nous avons étudié la prise en charge de 260 patients infectés par le VIH (258 VIH-1 et 2 VIH-2), originaires d’Afrique Subsaharienne dans le Service de Médecine Interne du CHU Bicêtre, entre 1998 et 2004. Soixante-et-un % étaient des femmes (âge moyen 34 ans), alors que la population masculine était plus âgée (39 ans). La majorité des patients étaient asymptomatiques cliniquement (76% stade A) versus 18% stade Sida ; leur taux moyen de lymphocytes T CD4 était de 258/mm3, et leur charge virale plasmatique de 3,5 log copies/ml. Un traitement était initié rapidement dans la majorité des cas, 41% des patients ayant un taux de CD4 à l’arrivée inférieur à 200/mm3. Les patients ont été traités selon les recommandations en vigueur, 74% d’entre eux recevant une trithérapie. Dix % des patients Africains étaient inclus dans des essais thérapeutiques. Sous traitement, on observait une remontée des CD4 à plus de 200/mm3 chez 76% d’entre eux à 6 mois, 78% à 12 mois et >80% dans le suivi. La moitié des patients avait une charge virale indétectable (

Au Cameroun, où presque 40% de la population vit en dessous du seuil de pauvreté, la prévalence du VIH est estimée à 5,3%. Depuis 2007, l'accès au traitement antirétroviral (TAR) est gratuit. Est-ce suffisant pour que toutes les Personnes Vivant avec le VIH/SIDA (PvVIH) soient bien prises en charge d'un point de vue médical ? L'objectif principal de cette étude était de déterminer si l'indigence était un facteur prédictif de survenue d'échec thérapeutique. Méthodes. L'indigence était définie à l'aide d'une grille d'évaluation spécifique. Le critère de jugement principal était la survenue d'un échec immunologique. Les critères d'inclusion étaient : âge supérieur à 18 ans et initiation du TAR en 2008. Les critères d'exclusion étaient : prophylaxie post-exposition ou prévention de la transmission mère-enfant. Résultats. Cent trente-huit patients ont été inclus avec un sex-ratio (femme/homme) de 3,6. Quarante-deux pour cent des patients inclus étaient indigents. A l'initiation du TAR, l'âge était de 34 ans, le taux de CD4 était de 173 cellules/mm3 et 67% des patients étaient aux stades 3 ou 4 de l'OMS. La durée de suivi sous TAR était de 32 mois. Treize patients (9,4%) étaient en échec thérapeutique. L'indigence n'avait pas d'impact significatif sur le taux d'échec (10,3% des patients indigents versus 8,8% des non indigents, p=0,75). Conclusion. Cette étude a permis de soulever les difficultés d'un suivi médical sur le long cours pour les PvVIH les plus démunies. Un ciblage précoce des indigents permettrait la mise en place d'un système d'accompagnement individuel de ces patients à haut risque de rupture de suivi.

Les dernières recommandations incitent le médecin traitant à assurer le suivi régulier des personnes vivant avec le VIH (PVVIH). Seulement, il semblerait que le niveau d'implication des médecins généralistes reste limité et que les PVVIH soient généralement suivis en milieu hospitalier. Objectif : A travers l'étude du suivi des patients infectés par le VIH au sein de la maison dispersée de santé Lille Moulin, comment appréhender le rôle du médecin généraliste dans leur prise en charge et mieux comprendre la faisabilité du suivi d'un traitement antirétroviral par le médecin généraliste ? Matériels et Méthodes : Etude descriptive au cours de l'année 2012 (réalisée au sein de la maison dispersée de santé, et au CH DRON) par analyse de dossier. Revue de la littérature actuelle sur le suivi des PVVIH par le médecin généraliste. Recueil par un questionnaire de l'expérience des praticiens de la maison dispersée de santé. Résultats : L'étude porte sur le suivi des patients séropositifs VIH sous traitement antirétroviral (ARV) au cours de l'année 2012, permettant d'inclure 159 patients. Le suivi des PVVIH est réalisable en médecine générale avec l'aide du centre de référence, seulement, l'implication des omnipraticiens semble ne pas se généraliser. Conclusion : Si l'implication de tous les médecins généralistes est peu réaliste à l'échelle nationale, il est intéressant de voir que les praticiens impliqués dans la prise en charge du VIH peuvent offrir une qualité de soin indéniable et plus flexible. Il est donc nécessaire d'informer les médecins généralistes sur la possibilité de suivre leurs patients sous antirétroviraux.
