Book excerpt: INTRODUCTION : L'outil STOPP (Screening Tool of Older Person's prescriptions) est un outil de détection de prescriptions médicalement inappropriées (PMI) chez e sujet âgé. Cet outil comprend 65 critères. OBJECTIFS : Évaluer l’optimisation des prescriptions chez la personne âgée, à l'issue d'une hospitalisation en service de gériatrie, après sensibilisation de l'équipe médicale au bon usage du médicament et à l'utilisation de l'outil STOPP. Évaluer la traçabilité dans le dossier médical lors de l'arrêt, maintien ou initiation de PMI à l'issue de l'hospitalisation. D2terminer si le maintien ou l'initiation de PMI à la sortie d'hospitalisation est justifiée. MATERIELS et METHODES : Étude observationnelle, monocentrique, rétrospective réalisée au sein du Pôle de médecine interne gériatrique thérapeutique de l'Assistance Publique des Hôpitaux de Marseille, en unités de court séjour gériatrique (CSG) et de soins de suite et de réadaptation (SSR) sur les patients de 65 ans et plus, hospitalisés du premier janvier au quinze avril 2013. Une analyse des ordonnances à l'entrée et à la sortie par un binôme médecin-pharmacien, a permis le recueil des PMI répondant à ou ou plusieurs critères STOPP. Pour chaque PMI détectée, une analyse du dossier médicale a permis de déterminer si il existait ou non une traçabilité de l'attitude médical vis-à-vis de cette PMI. Enfin, un médecin expert a analysé l'ensemble des PMI selon les critères STOPP présentes à a sortie d'hospitalisation et a déterminé le caractère approprié ou non de ces prescriptions. RESULTATS : Sur les 192 patients inclus, 69 patients (35,9%) possèdent une ou plusieurs PMI à l'entrée versus 51 (26,6%) à la sortie, soit une diminution de prévalence de PMI selon les critères STOPP de 26,1 %. Les facteurs associés à la présence de PMI à l'entrée sont le motif d'hospitalisation pour chute (p= 0,001), la polymédication (p= 0,001 si ≥ 5 médicaments) et l'existence d'antécédents de dépression (p=0,02) ou de maladie de Parkinson -p=0,02).
