L'objectif de cette thèse est de comparer le taux de patients diabétiques observants au niveau du service d'endocrinologie du CHU de Dijon et dans différentes structures en ambulatoire puis de déterminer les facteurs prédictifs de mauvaise observance propres à chaque environnement. Il s'agit d'une étude prospective qui s'est déroulée dans le service d'endocrinologie du CHU de Dijon et dans différentes structures en ambulatoire incluant 106 patients diabétiques recevant un traitement antidiabétique durant une période de 1 mois en milieu hospitalier, et de 2 mois en ambulatoire. L'observance a été évaluée à l'aide du questionnaire de Morisky. Une grande disparité concernant les taux d'observance en milieu hospitalier et en ambulatoire a été constatée. 47,7% des patients en ambulatoire obtenaient une bonne observance contre 16,1% pour les patients hospitalisés. Le nombre de médicaments antidiabétiques est associé de manière indépendante à une moins bonne observance médicamenteuse quel que soit l'environnement où se trouve le patient diabétique. La faible observance apparait également corrélée à une HbA1C plus élevée. L'optimisation de l'observance médicamenteuse apparait donc comme un facteur important à prendre en charge pour optimiser l'équilibre glycémique, notamment chez les patients diabétiques ayant un nombre de traitements du diabète élevé. Le lien entre le clinicien et le pharmacien peut être une piste pour améliorer l'observance médicamenteuse de ces patients.
