Book excerpt: Contexte : en 2003 un rapport de l'OMS dit que l'amélioration de l'observance thérapeutique serait l'avancée médicale la plus importante pour la santé des populations. Beaucoup d'études ont recherché comment l'améliorer du point de vue du patient. Cette étude a eu pour but de voir quel était le point de vue, 11 ans après ce rapport de l'OMS, d'un acteur majeur de cette observance : le médecin généraliste. Méthode : il s'agit d'une étude qualitative par entretiens semi-dirigés auprès de médecins généralistes recrutés de façon raisonnée et en variation maximale. L'analyse thématique des entretiens a été réalisée à l'aide du logiciel QSR Nvivo 10. Résultats : douze entretiens ont été nécessaires pour obtenir la saturation des données. Les médecins généralistes ont une bonne connaissance médicale théorique. Ils connaissent la problématique de l'observance thérapeutique mais ils ont des difficultés à l'appréhender dans leur pratique. Les causes sont liées à la relation médecin-patient, à leurs représentations, au temps, à la formation médicale initiale et continue. Les médecins généralistes sont conscients de leurs difficultés et décrivent des freins liés à leur pratique et aux patients. Ils désirent y remédier en proposant des solutions pour améliorer la sensibilisation de tous les intervenants, professionnels de santé ou non, à cette problématique. Conclusion : l'amélioration de la formation médicale continue, des outils d'aide au diagnostic, un meilleur fonctionnement du réseau de soins et une sensibilisation au problème de la non observance des populations semblent être de bonnes pistes. Mais la formation médicale initiale à la relation médecin-patient est essentielle. Notre étude montre que les représentations des médecins généralistes influencent la façon dont ils traitent l'observance thérapeutique.

Book excerpt: Contexte : Les généralistes semblent de plus en plus confrontés à des patients présentant une souffrance morale liée au travail. Les objectifs de cette étude étaient de décrire les pratiques des médecins généralistes du Nord-Pas-de-Calais face à leurs patients présentant ce type de souffrance, d'identifier les difficultés et solutions paraissant intéressantes pour améliorer leur prise en charge. Méthode : Enquête quantitative, descriptive et transversale, par questionnaire à destination d'un échantillon de 350 généralistes du Nord-Pas-de-Calais tirés au sort, avec relance téléphonique. Recueil des données du 20 mars au 30 mai 2015. Résultats : L'analyse de 182 questionnaires (taux de réponse de 52%) a permis de décrire les pratiques de ces médecins généralistes. Ils privilégient l'écoute attentive de leurs patients et la prescription d'un arrêt maladie avant le recours aux thérapeutiques médicamenteuses. La collaboration avec le médecin du travail et le spécialiste de la santé mentale est fréquente mais entravée par de nombreux freins. Un accès rapide et facilité pourrait notamment améliorer leur collaboration avec ces professionnels: identification des différents partenaires par création d'annuaires, remboursement d'un certain nombre de consultations chez le psychologue, création de structures spécialisées sur le sujet, obtention rapide d'un avis psychiatrique par création d'un numéro-vert. Les centres de Pathologies Professionnelles sont une aide intéressante mais encore peu utilisée. La formation initiale est améliorable, la formation continue doit être axée sur les rencontres interdisciplinaires et les besoins pratiques des généralistes. Conclusion : L'interdisciplinarité, pourtant indispensable pour une prise en charge optimale de ces patients, reste actuellement limitée. Des solutions existent et sont à prendre en compte pour optimiser les soins et lutter contre la désinsertion professionnelle.

As is stressed in the Global Strategy on Human Resources for Health: Workforce 2030, there can be no viable national, or global, health system without an effective health workforce. The Global Strategy, adopted at the Sixty-ninth World Health Assembly in May 2016, challenges the erroneous narrative of health workers as a unit of cost in the production of health. The evidence instead presents an intersectoral agenda on the pre-condition of equitable access to health workers in the attainment of universal health coverage, along with a dynamic labour market understanding of the substantive impact on education, employment, jobs and innovation in the health and social care economy. The Global Strategy, therefore, enables governments and other relevant stakeholders to adopt a holistic, rather than fragmented, approach to ensuring that the health workforce contributes both to improved health and to broader socioeconomic development.
