Book excerpt: Les personnes âgées dépendantes ont souvent dans la bouche des dépôts microbiens épais, qui sont difficiles à nettoyer pour les aides-soignants des Établissements d'Hébergement pour Personnes Âgées Dépendantes (EHPAD). La formation et la reconstitution rapide de ces dépôts sont favorisées par des conditions caractéristiques du grand âge et de la dépendance, qui empêchent le nettoyage physiologique de la bouche (bouche sèche, dents mobiles ou cariées, prothèses non fonctionnelles, médicaments, aliments mous et mixés). C'est pourquoi, malgré tout le soin apporté, 72 % des soignants trouvent que l'hygiène bucco-dentaire des résidents est insuffisante. Cette étude observationnelle dans le cadre des soins courants avait pour objectif de valider une fiche simplifiée, pour que les aides-soignants en charge des soins d'hygiène bucco-dentaire puissent comparer un score d'état bucco-dentaire au score de l'état oral précédent. Le critère d'évaluation principal était l'observation qualitative, sur graphe, de l'évolution du score pendant 13 semaines consécutives (9 fiches remplies par les aides-soignants puis 4 par l'étudiant). Parmi les 57 résidents en long séjour éligibles à l'étude, 33 ont été analysés (au moins 4 fiches remplies). Sur un score moyen, le graphe a montré que les mauvais scores étaient améliorés la semaine suivante. Ce travail confirme l'intérêt de valoriser par un score chiffré le travail des aides-soignants, ce qui augmente leur motivation pour effectuer les soins de bouche. En améliorant la lutte contre les réservoirs microbiens oraux, ce nouvel outil devrait aider à lutter contre la perte d'appétit, la dénutrition et les pneumonies d'inhalation chez les résidents en EHPAD.

Particular difficulties include: complex labour legislation; differences in the perception of health risks associated with work experiences amongst workplace parties and policy makers; the burden of costs and consequences being borne by different stakeholders in the system; conflicting incentives and priorities between the multiple stakeholders; lack of consensus about what ought to count as a benefit or cost of intervening or not intervening; multiple providers of indemnity and medical care coverage; and industry-specific human resources practices making it difficult to identify all work-related illnesses and injuries. In addition, most health economics methods books are designed for use in a clinical setting, which cannot be adapted for the workplace setting. In the face of such barriers, it is not surprising that few studies of occupational health and safety interventions contain an economic evaluation. This book aims to lay the foundations for a systematic methodology of economic evaluation of workplace interventions, by identifying the main barriers to research of high quality and practical relevance, and proposing a research strategy to remedy these weaknesses. Context chapters provide a wealth of background material ranging from a presentation of the broad conceptualization of work and health, to suggestions for strategies in confronting the dearth of data often experienced by occupational health and safety researchers. What should have been a few days walk turned into his worst possible nightmare as he stumbled aimlessly through mosquito and crocodile infested swamps. "It was almost six days I'd been without sleep, apart from a short period of unconsciousness and those few moments before I fell out of that tree," he said. " I became demented by the cavalcade of mosquitoes and hallucinating badly". His experience was only to worsen - he waded halfway across a tidal river to be confronted by a large saltie. Darwin historian John Haslett help Stagg map the original route by retracing his steps, even managing to relocate an American Kittyhawk Stagg found crashed in the middle of nowhere.
