Book excerpt: Para 2009 el gasto en salud, es decir la suma del gasto público y privado dedicado a nivel mundial a las actividades de salud preventivas y curativas, fue calculado en el 10% del PIB, aunque con grandes variaciones entre regiones, p.ej., en América del Norte fue del 15%, mientras en Latinoamérica fue del 7,7%, en el África Subsahariana del 6,6% y en el Asia Oriental y el Pacífico del 4,4% (Indicadores del Desarrollo Mundial, Banco Mundial); el crecimiento desde 1996 hasta 2009 fue sólo de 1,2 puntos, con el agravante de que en los 7 años previos no hubo ningún crecimiento, si observamos que el gasto en el 2009 fue el mismo que el del 2002. De mantenerse esas tendencias y teniendo en cuenta la crisis económica global, no se espera que en la actual década haya grandes cambios en el total de recursos disponibles para salud. En este escenario de restricción de recursos la salud pública enfrenta grandes desafíos: 33,3 millones de personas en el mundo viven con VIH/SIDA, de estos 2,5 millones son menores de 15 años y se estima que habrá 2,6 millones de nuevos casos por año (UNAIDS, Informe Mundial, 2010). Gracias a los avances en el tratamiento, la supervivencia de las PVVS ha aumentado y el VIH/SIDA ha pasado a convertirse en una enfermedad crónica. Asociado a la epidemia del VIH y a otros factores de índole económico y social, enfermedades como la tuberculosis y la malaria que antes se consideraban controladas, han reemergido con mayor severidad y resistencia al tratamiento. Mil millones de personas en el mundo están afectadas por enfermedades tropicales, que hasta ahora no habían recibido la atención debida (Neglected Tropical Diseases) y ahora pasan a ser una prioridad.

It will review developments and progress since the publication of the 1991 and 1997 Institute reports. For technical contracting reasons, the new project was split into two phases. During the limited first phase, a committee appointed by IOM planned and convened a 1-day workshop to examine a subset of topics as background for the second phase of project. As was agreed upon with the sponsor of the workshop, the Centers for Disease Control and Prevention (CDC), the topics were: methodological and policy issues related to the conceptualization, definition, measurement, and monitoring of disability and health over time; trends in the amount, types, and causes of disability; disability across the age spectrum and in the context of normal aging; and secondary health conditions. The phase-one workshop was held in Washington, D.C. on August 1, 2005. Its participants included researchers, clinicians, social service professionals, policy experts, and consumer representatives and advocates. The meeting agenda and list of participants are included in Appendix A. Workshop on Disability in America: A New Look- Summary and Background Papers summarizes the workshop presentations and discussions. The background papers prepared for the workshop are included in Appendixes B through O. Some papers were submitted and circulated in advance of the meeting, whereas others were first presented at the meeting. The analyses, definitions, and views presented in the papers are those of the paper authors and are not necessarily those of the IOM committee. Likewise, the discussion summary is limited to the views of the workshop participants.
