Si au cours des cinquante dernières années les politiques de santé mentale ont tenté de répondre à cette complexité par la création d'une diversité d'instruments, ceux-ci ont fait l'objet de critiques de la part des acteurs et semblent avoir échoué à organiser l'offre de soins pour répondre de façon efficiente aux besoins sur l'ensemble du territoire. Cette thèse analyse deux dispositifs récents de la politique de santé mentale visant à répondre à ces critiques : les projets territoriaux de santé mentale (PTSM) et le fond d'innovation organisationnelle en psychiatrie (FIO). Mobilisant une perspective de sociologie interactionniste, l'objectif de cette recherche était notamment d'évaluer comment la mobilisation collective autour du PTSM et du FIO a contribué à transformer l'organisation de la psychiatrie et de la santé mentale en Seine-Saint-Denis. Une enquête qualitative a été menée entre septembre 2020 et août 2021 : quatorze entretiens semi-directifs et l'étude d'un corpus documentaire ont été réalisés. La Seine-Saint-Denis se distingue par des difficultés socio-économiques majeures et une offre de services de santé mentale inférieure à la moyenne. Son système de santé mentale se caractérise par l'existence d'un grand nombre de réseaux de coopération professionnelle. Le PTSM et le FIO ont permis de renforcer un peu plus les réseaux existants et de créer quelques coopérations nouvelles, mais n'ont pas entraîné de recomposition majeure des relations entre acteurs. L'introduction des instruments sur le territoire a fait suite à un mouvement social au sein de l'hôpital Ville-Evrard contre l'obligation de faire partie d'un GHT. Obtenant gain de cause, Ville-Evrard, acteur historique et central du territoire, a pris le leadership de l'élaboration du PTSM. La construction d'un collectif, première étape de l'élaboration du PTSM, a nécessité de définir le périmètre des acteurs concernés par la santé mentale, puis d'identifier et de mobiliser des acteurs. Pour les acteurs, s'engager dans la démarche, supposait d'en attendre un bénéfice : ils y ont vu un outil pour faire avancer leurs propres projets. Un travail de diagnostic, d'élaborations de solutions et de priorisation a ensuite été mené. L'élaboration du diagnostic a notamment reposé sur un travail difficile de recueil et de compilation de données. Outre les effets d'interconnaissance, l'élaboration du PTSM a constitué un lieu de savoir et de réflexion, un outil de connaissance du territoire et un vecteur de circulation des idées permettant l'adoption de normes et de valeurs partagées. Au total, le PTSM a permis à la fois de faire émerger des projets, de les sélectionner et d'aligner derrière eux les acteurs de la santé mentale du département. Le positionnement de l'ARS dans le cadre de cette démarche a été difficile à trouver et jugé insuffisant par les acteurs qui ont également émis des critiques à l'encontre de l'État. L'étude du FIO a, quant à elle, permis d'étudier la manière dont les acteurs se saisissent des appels à projet pour financer des projets. Si la démarche des PTSM a permis de renforcer la coopération entre acteurs et de faciliter la construction d'un certain nombre de projets, il existe un risque que le niveau de coopération atteint ne se maintienne pas. En effet, les acteurs rapportent un état d'incertitude quant au devenir du PTSM et attendent des clarifications sur le financement des actions du projet. Pour eux, la mise en place d'un dispositif de suivi de la démarche est indispensable. L'enjeu est de taille : outre l'amélioration de l'offre de soin et des parcours, il s'agit de rétablir la confiance des acteurs en la capacité de l'État à mener une réelle politique de santé mentale.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? While this outbreak bears some unique distinctions to past outbreaks, many characteristics remain the same and contributed to tragic loss of human life and unnecessary expenditure of capital: insufficient knowledge of the disease, its reservoirs, and its transmission; delayed prevention efforts and treatment; poor control of the disease in hospital settings; and inadequate community and international responses. Recognizing the opportunity to learn from the countless lessons of this epidemic, the National Academies of Sciences, Engineering, and Medicine convened a workshop in March 2015 to discuss the challenges to successful outbreak responses at the scientific, clinical, and global health levels. Workshop participants explored the epidemic from multiple perspectives, identified important questions about Ebola that remained unanswered, and sought to apply this understanding to the broad challenges posed by Ebola and other emerging pathogens, to prevent the international community from being taken by surprise once again in the face of these threats.
