Book excerpt: This volume educates physicians on the basic knowledge of ESD, from indication of this technique to assessing suitability and technical aspects of each important step. The text is structured to guide readers to understand the principles, including history, why ESD was designed and developed, indications, as well as how to evaluate lesions and perform actual procedures. Abundant photographs and diagrams illustrate the key points and promote “pattern recognition”. The text also introduces gastroenterologists to early gastrointestinal lesions, which have not been recognized in clinical practice for many years. To assist in the treatment process of ESD, highly practical information is provided, separated by different anatomical locations, as well as management of complications. The volume is also accompanied by an online video library. Written by experts in Asian countries with vast experience with this technique, as well as experts from the U.S., South America, and Europe, Endoscopic Submucosal Dissection: Principles and Practice provides a major reference for gastroenterologists and surgeons on ESD.

Available in PDF, EPUB and Kindle. Book excerpt: Twenty-five years ago, a monograph on gastric cancer was published by the Italian Society of Surgery: that book is recognized as a milestone in the management of these tumors in Italy. Oncological and surgical knowledge in the field of gastric cancer have changed dramatically over the last 25 years. The aim of this book is to offer an essential update on current diagnostic approaches and optimal treatment strategies. It gives comprehensive information on gastric cancer not neglecting the basic sciences, which can shed light on the carcinogenesis mechanisms involved in this pathology. The principles of tailored and multimodal treatment are examined according to the latest guidelines with the aim of providing a valuable synopsis for clinicians. The book not only reviews what has changed in the field of gastric cancer over the past quarter century but also offers the reader a glimpse of the future by describing the ongoing surgical and oncological trials which focus on new treatment frontiers such as immunotherapy and target therapy. It also seeks to incorporate the latest discoveries based on molecular classifications. Further, the book reflects the findings and experiences of Italian experts belonging to the GIRCG (Italian Research Group for Gastric Cancer) and will offer an excellent guide for oncological surgeons in their daily practice of caring for patients affected by gastric cancer.

Book excerpt: In this issue of Gastrointestinal Endoscopy Clinics of North America, we invited pioneers and experts in this field to share recent advancements and expansions of ESD and its technical aspects in different organ systems. In addition, this issue covers the associated techniques of per oral endoscopic myotomy (POEM) and natural orifice transluminal endoscopic surgery (NOTES) to further advance understanding of the latest breakthroughs in endoscopic therapy. Current trends surrounding ESD around the world—in Asia, Europe, and the United States—are addressed as well. Some articles include videos for readers to watch ESD procedures in action, performed by experts, for easier understanding of ESD techniques. The Guest Editors expect that this issue will inspire more physicians to explore the expanding possibilities of endoscopic therapy such as ESD and POEM and to advocate minimally invasive treatment for patients.

Book excerpt: Laparoscopic colectomy has been introduced in the early 90ies as a new technique with proven improvement in short and long term outcome. Despite significant patient benefits the overall adoption rate remains low. On the other hand significant more advanced techniques such as single incision laparoscopy, robotics and transanal access surgery have been introduced in the last several years and are rapidly evolving. Therefore a significant gap between the practicing community surgeon and innovator seems to exist. Advanced Techniques in Minimally Invasive and Robotic Colorectal Surgery bridges that gap and provides a foundation summarizing and organizing all classic and new techniques in minimally invasive colorectal surgery available in literature and from masters within the field. It provides tips and tricks that allow the user to move up the learning curve, manage difficult scenarios, overcome challenges, increase the utilization of minimal invasive techniques and decrease the conversion to open surgery. By enhancing the surgical tool box the surgeon will be able to progress from the novice to the master. Rather than describing operative procedures which may be very biased by an individual author, the guide provides alternative building blocks of various difficulty for different procedures which can be then tailored to the patient and surgeon`s own comfort level and experience. Advanced Techniques in Minimally Invasive and Robotic Colorectal Surgery will serve as a inspirational guide for the innovator of the future. It will allow surgeons of all levels to better adapt to inevitable changes in future techniques and will be of great value to general surgeons, colon and rectal surgeons, minimally invasive surgeons as well as residents and fellows.

Book excerpt: The aim of this book is to present a concise yet instructive overview of the most common interventional and therapeutic gastrointestinal endoscopic procedures. Although there exists a large amount of literature dealing with therapeutic endoscopy, this publication stands out for several reasons: To begin with, it has been written by a highly selected group of international experts who contribute their own tried and tested tips and tricks. The various techniques are described in a homogeneous fashion, making it easy for the endoscopist to single out the important aspects for his or her own practice. Moreover, the data presented follows evidence-based guidelines, but does not inundate the reader with confusing facts and numbers. Finally, the book is illustrated like an atlas, featuring real-life pictures enhanced by explanatory drawings and algorithms. This textbook is directed at residents, fellows, gastroenterologists and surgeons interested in therapeutic endoscopy. As the majority of the procedures described are classics that have stood the test of time, it will be a useful companion for many years to come.

Available in PDF, EPUB and Kindle. Book excerpt: Endoscopic submucosal dissection (ESD) was first implemented in early gastric cancer allowing for en-bloc resection of the lesions. With the experience came the expertise to introduce ESD for early colon cancer (ECC). ESD demonstrates several advantages in comparison with the endoscopic mucosa resection. It allows accurate histological assessment of the depth of invasion, minimizes the risk of local recurrence and helps in the determination of additional therapy. Indications for ESD are placed only after adequate endoscopic morphological classification of the lesions excluding higher risk of nodal metastases. This chapter provides an overview of the application of ESD techniques in ESD for ECC and provides assessment on its technical aspects and complications. In order to decrease the rate of complications a standard protocol for the ESD should be adopted. The protocol includes recommendations for patient selection, bowel and patient preparation, appropriate equipment (knives, endoscopes, and power devices). The chapter will review the current ESD techniques and oncological results. ESD could have great impact on the treatment of early colon cancer. Its role is already proven in rectal localizations and despite the challenges it should be adopted for the colon. Safe strategy for ESD is the cornerstone in decreasing complications, which includes suitable resection of specialized ESD devices.
