To cope with accumulation of unfolded or misfolded proteins, the ER has evolved a group of signaling pathways termed “Unfolded Protein Response (UPR)” or “ER stress response” to align cellular physiology. To maintain ER homeostasis, transcriptional regulation mediated through multiple UPR branches is orchestrated to increase ER folding capacity, reduce ER workload, and promote degradation of misfolded proteins. In recent years, accumulating evidence suggests that ER stress-triggered transcriptional reprogramming exists in many pathophysiological processes and plays fundamental roles in the initiation and progression of a variety of diseases, such as metabolic disease, cardiovascular disease, neurodegenerative disease, and cancer. Understanding effects and mechanisms of ER stressassociated transcriptional reprogramming has high impact on many areas of molecular genetics and will be particularly informative to the development of pharmacologic avenues towards the prevention and treatment of modern common human diseases by targeting the UPR signaling. For these reasons, ER stress response and transcriptional reprogramming are a timely and necessary topic of discussion for Frontiers in Genetics. LVH develops in the course of valvular or congenital heart disease, or when part of the myocardium is damaged by long-standing ischemia or infarction. In the hypertrophied heart the muscle fibers increase in size, not in number. The fibers are found to contain a larger number of myofibrils and the cell organelles are larger. From epidemiologic studies it is known that even mild LVH is associated with myocardial ischemia, ventricular arrhythmias, and sudden cardiac death. Most cases of LVH show focal degenerative tissue changes including cellular atrophy, myofibrillar disorganization, interstitial fibrosis, and loss of intracellular connections. Myocardial dysfunction develops and, unlike the functional adaptive changes found in pure hypertrophy, is not reversible by surgical treatment of the valvular heart disease or medical correction of hypertension. Interstitial fibrosis, intracellular changes of musc Ie cells, and loss of contract ile tissue lead to poor mechanical function and undoubtedly increase the risk of ischemia, arrhythmias, or sudden death, a well-recognized problem in patients with a variety of heart diseases. Even When successfully treated, the patients may remain at risk if the compensatory hypertrophy is not fully reversed. Epidemiologic studies conducted in the Framingham population in the early 1950' s demonstrated LVH according to electrocardiographic criteria in 1. 5% of the population; 2% of the population had LVH according to chest X-ray criteria.

Book excerpt: Endemic fluorosis is caused by excessive intake of fluorine in drinking water, air, food, tea and other media in a specific natural environment for a long time, which involves at least 25 countries, such as China, India, Vietnam, Iran, Egypt, Mexico, Argentina and influences more than 200 million of population. China and India are the countries with the most extensive endemic fluorosis and the most serious damages. There are three main intake sources of endemic fluorosis, e.g., drinking water, coal-burning pollution and drinking tea. Coal-burning type of endemic fluorosis was established in Guizhou Province of China in 1970’s. The residents in the areas used indoor stoves with no flue to burn coals with high content of fluoride to bake grains in the autumn and heat in the winter. This results in excessive intake of fluorine and then leads chronic poisoning. It has been confirmed that 13 provinces of China with 34.3 million of population are influenced by this type of fluorosis with severe injuries to most organs of human body. The content of this book covers the epidemiology, pathological changes, molecular pathogenesis and clinical treatments of the disease. This book is a suitable reference for the researchers and graduate students in this field, which is helpful to improve the understanding of this disease and to take preventive measures.
