Book excerpt: An integrated survey of best practices for the management of patients with implanted prosthetic devices and an insightful examination of the epidemiological, societal, and policy issues associated with their use. The devices covered range from breast, penile, vascular, and joint prostheses to cochlear, ossicular, and dental implants, and include cerebrospinal fluid shunts, cardiac valves, stents, and pacemakers. For each device, the authors consider its pros and cons, detail the best current strategies to keep implanted patients healthy, and evaluate the latest and most promising new diagnostic tests, Clinical counterpoints from distinguished authorities at major centers in the United States and Europe are offered throughout. Follow-up recommendations are summarized in a standardized format that allows comparative analysis and lays the foundation for controlled clinical trials and the eventual establishment of evidence-based guidelines.

The first part of the book focuses on polymers, fibers and textile technologies, and these chapters discuss the manufacture, sterilization, properties and types of biotextiles used for medical applications, including nanofibers, resorbable polymers and shaped biotextiles. The chapters in part two provide a comprehensive discussion of a range of different clinical applications of biotextiles, including surgical sutures, arterial prostheses, stent grafts, percutaneous heart valves and drug delivery systems. This book provides a concise review of the technologies, properties and types of biotextiles used as medical devices. In addition, it addresses the biological dimension of how to design devices for different clinical applications, providing an invaluable reference for biomedical engineers of medical textiles, quality control and risk assessment specialists, as well as managers of regulatory affairs. The subject matter will also be of interest to professionals within the healthcare system including surgeons, nurses, therapists, sourcing and purchasing agents, researchers and students in different disciplines. Available in PDF, EPUB and Kindle. Book excerpt: Increasing evidence suggests that particularly in high-risk patients, endovascular solutions offer substantial advantages compared to vascular surgical procedures. Nevertheless, growing numbers of procedures are associated with an increased incidence of complications. Knowledge of specific complications in different vessel areas will support the interventionist in preventing such adverse events and if necessary, provide considerable reassurance if such complications need to be resolved. In Complications in Peripheral Vascular Interventions, typical and atypical complications are described for all peripheral vessel areas, and methods for how to handle these events are outlined. The book is divided in two parts. Part 1 reviews general aspects on complications in peripheral interventions, and Part II covers the specific vessel areas. Each chapter on the specific vessel area includes Introduction to the frequency and kind of complications in this vessel area Factors identifying high-risk patients for these complications Complications of specific interventional steps and tools Endovascular techniques to resolve these complications Methods to avoid complications Summary Checklist for emergency equipment Offering practical tips for the interventionist in the cath lab, high-volume interventionists share their experiences by reviewing complicated cases and outlining different strategies used in real-life scenarios.

Available in PDF, EPUB and Kindle. Book excerpt: Peripheral vascular disease is a common and disabling malady, and patients seeking treatment may turn to their cardiologist, whom they are seeing for existing cardio-vascular disease, for advice and treatment. Conventional treatment has always been medical management and, inevitably, surgical bypass, even amputation. Because stents have had a significantly high impact on endoluminal treatment outcomes by preventing injury to the lumen, reducing the potential for hyperplasia and restenosis, as well as the likelihood of plaque disruption and embolization, the 20 or so available peripheral vascular stents must be considered an alternative to bypass surgery.

Available in PDF, EPUB and Kindle. Book excerpt: Cardiovascular diseases are still the leading cause of death in developed countries. Revascularization procedures such as coronary artery and peripheral bypass grafts, as well as access surgery represent a 2$ billion market yearly for the US alone. Despite intense research over many decades, no clinically suitable, shelf-ready, synthetic, vascular, small-caliber graft exists. There is therefore still a quest for such a clinical vascular prosthesis for surgical revascularization procedures and access surgery. Many approaches have been tried and are currently under investigation with promising results. These range from acellular and cell-based, stable or bio-degradable, synthetic scaffolds to biological or decellularized grafts, not forgetting self-assembly technologies for in vitro or in vivo VTE. All these approaches can be further enhanced by functionalization, e.g. with growth factors and drug elution. This updatable book aims to cover all the relevant aspects of Vascular Tissue Engineering (VTE) and novel alternatives to develop vascular grafts for clinical applications. The chapters in this book cover different aspects of manufacturing scaffolds with various polymers, mechanical characteristics, degradation rates, decellularization techniques, cell sheet assembly, 3-D printing and autologous mandril-based VTE. All the necessary in vitro tests such as biocompatibility and thrombogenicity are reviewed. Pre-clinical assessment of in vivo experimental models include patency, compliance, intimal hyperplasia, inflammatory reaction, cellular ingrowth and remodeling. Finally, early clinical trials will be periodically updated regarding results, regulatory aspects and post-marketing quality assessment. Furthermore, the reader should get an insight into various approaches, technologies and methods to better understand the complexity of blood surface and cell interactions in VTE. Translational research has yielded early human applications clearly showing the enormous need of research in the field to provide better solutions for our patients and this continuously updated book will hopefully become a reference in the field for life sciences.
