Book excerpt: ix Preface Smart surgeons learn from their own mistakes, smarter surgeons learn from mistakes of others, some never learn . . . You are a resident, overworked and constantly tired; sitting down with your mentor for a brief tutorial. What do you want to get out of these few minutes? To organize your thoughts and approaches to the particular problem; to learn how he -the weathered surgeon -"tackles it"; to grasp a few practical "recipes" or "goodies" and take home a message or two; to laugh a bit and unwind. This is also our goal in this book. We hope that you are not repelled or offended by the non-formal character of this book. This is how emergency abdominal surgery is taught best, by trial and error and repetitions, with emphasis on basics. This is not a "complete" textbook, nor is it a cookbook type manual or discussion of case studies; neither is it a collection of detailed lecture notes or exhaustive lists. Instead, it consists of a series of informal, uncensored, chats between experienced surgeons and their trainees. No percentages, series, elaborated figures or complicated algorithms are included; only a surgeon's narrative, explaining how "he does it" -based on his experience and state of the art knowledge of the literature. No references are included as it was our aim to put down nothing which has not been experienced, confirmed and practiced in our own hands.

Available in PDF, EPUB and Kindle. Book excerpt: There is a large influx of trauma, general surgical, pediatric, neurosurgical and obstetric emergencies as the hospital is one of the major public hospitals located in a densely populated region sitting amidst major routes in the city. It also provides services for emergency cardiovascular, cerebrovascular interventions and cadaver transplants. Daily lessons from this tremendous emergency work inspired us to bring out this book. There is plethora of reference material available in basic sciences related to anesthesiology, namely physiology, pharmacology, physics and anatomy. Similarly, a large number of textbooks are on techniques such as general, regional, monitored anesthesia care as well as super specialties like cardiac, neurosurgical and pediatric anesthesia, etc. But there is a limited availability of books and references for emergency anesthesia. One has to scan and integrate parts from various sources. This book is an attempt to assimilate the scattered information and add clinical expertise. Contents of this book have been divided into nine sections to cover majority of the emergencies. General considerations for adults, obstetric and pediatric sections have been written separately to avoid duplication. An additional challenge is cases with various medical disorders for emergency surgeries. Efforts have been taken to accommodate them in first section and obstetric section. The contributors of this book work together at LTMMC and LTMGH and have a well-balanced team with fresh talent bringing the latest technological knowledge and tenured practitioners supplementing it with applied experience. Part of the team worked on collection of the matter from different sources for an ideal scenario, while the senior faculty members used their vast clinical experience for a more practical and realistic application. These combined efforts have been truly beneficial to achieve our goal to rationally mix latest technology with clinical experience and identify potential ways to conduct anesthesia with best results, at least costs and minimum risk.

Book excerpt: This manual explains how to make the right decisions on the timing and selection of investigations and surgical procedures in emergency and urgent surgical settings and describes the most widely used procedures step by step with the aid of high-quality illustrations. The goal is to address the situations that can arise in almost any emergency department throughout the world, enabling the surgeon on call to acquire or sharpen the knowledge and skills needed to deal with acute surgical problems in the most appropriate way. The reader will gain a sound understanding of the most efficient diagnostic modalities, pre-, intra-, and postoperative decision-making, and surgical techniques and issues in particular circumstances. The manual stems from an initiative by members of the European Society of Trauma and Emergency Surgery (ESTES) and the American Association for the Surgery of Trauma (AAST) to set up and formalize Emergency Surgery Courses to provide specific training in emergency and acute care surgery. It represents a didactic accompaniment to the course that will guide the beginner and maintain a certain degree of standardization among the more experienced.

Available in PDF, EPUB and Kindle. Book excerpt: Variability in care in emergency surgery is well known, particularly in abdominal surgery, and reducing it is fundamental to improving outcomes. It is therefore vital to identify the factors that are key to improving mortality and reducing complications in various surgical emergencies. Under the leadership of the World Society of Emergency Surgery, over 200 leading experts in emergency surgery, critical care and surgical infections established a template to help define and optimise emergency surgery, particularly that relating to the abdomen. This book refines this initial work, crystallising it into a meaningful resource for practising surgeons and health care administrators alike, and paving the way for advances in emergency surgery around the globe. This book sets a benchmark for Emergency Surgery and is a must read for those wanting their patients to do better.

Book excerpt: The aim of this book is to identify and shed new light on the main surgical practices involved in acute care and trauma surgery. Adopting an evidence-based approach, a multidisciplinary team of surgeons and intensivists illustrate basic and advanced operative techniques, accompanied by a comprehensive and updated reference collection. Emerging surgical procedures in Acute Care Surgery are also explored step by step with the help of ample illustrations. Further, the book offers an overview of treatment options for acute organ failure, with a focus on peculiar clinical situations that call for special treatment, including: extracorporeal membrane oxygenation (ECMO), circulatory support and urgent liver transplantation. In order to identify the key determinants in decision-making and therapeutic support that can help avoid critical errors, authors not only define and discuss specific surgical techniques, but also present major issues and controversies related to it. In essence, the book can be considered a “What to do guide” that offers easy access to both basic and advanced lifesaving surgical procedures, due in part to text boxes highlighting “Tips, Tricks and Pitfalls” as well as advice on “Complications Management”. Many chapters are enriched by providing clinical case scenarios, an important educational resource including self–assessment questionnaires.

The concept behind it is that emergency surgeons and physicians not only need the clinical knowledge to manage the different pathological conditions, but that they must also have a full understanding of diagnostic imaging modalities. To this end, each chapter includes a description of a specific acute abdominal disorder. In addition to the clinical presentation and the diagnosis and management guidelines, there is a special focus on imaging studies with clear and concise descriptions, high-quality images and the evolution grading scale to aid interpretation. This easy-to-read book is an ideal source of practical information for acute care surgeons, radiologists and for all the members of the emergency team.

Available in PDF, EPUB and Kindle. Book excerpt: There is a growing need for a book on emergency general surgery in the elderly, as adults age 65 and older are the fastest growing segment of the population worldwide, and their number is expected to double to 89 million between now and 2050. Based on these changing demographics, it is expected that there will be a concurrent rise in the demand for a variety of surgical services, including vascular surgery (with a projected growth of 31%) and general surgery (with a projected growth of 18%), as well as in all sub-disciplines of surgery (thoracic, cardiac, oncology, hepatobiliary, transplant, plastic colorectal, orthopedic, gynecologic, urology and neurosurgery). In addition, older surgical patients often require a different level of care than younger patients during the preoperative, intraoperative and postoperative phases. Many of our geriatric population have multiple chronic illnesses beyond the one for which surgery is required, and therefore are prone to developing postoperative complications, functional decline, loss of independence, and other undesirable outcomes. In response, this book offers surgeons, geriatrists and nurses state-of-the-art surgical approaches and essential information on the complex perioperative care of the elderly.
