Eccentric strength was examined on the right quadriceps of 29 healthy individuals (9 men, 20 women) ranging in age from 18-35 years of age. Strength gains were quantitatively represented by isokinetic peak torque (N/m) values obtained by pre-and posttest measures. These measures were taken before and after a 4 week step down regimen, consisting of sets of 3 x 10 repetitions performed 3 times weekly. Efficacy of NMES was examined by observing differences in strength gains between three groups: control, eccentric exercise, and combined NMES and eccentric exercise groups. A 2 x 3 Factorial ANOVA was utilized to determine significant differences between and within groups using the mean peak eccentric torque values. Strength gains within groups were found, yet did not prove significant (F=1.68). In addition, no significant difference between treatment groups was found (F=.35). Neuromuscular electrical stimulation did not prove to facilitate eccentric quadriceps strength over exercise alone. However, exercise alone also did not demonstrate an advantage over the control group. In conclusion, because this protocol is currently used as a treatment approach in clinics, more research is suggested utilizing different populations, and variations in protocol. Quadriceps weakness continues to be a problem for anterior cruciate ligament (ACL) deficient patients who make up a large population in orthopedic physical therapy clinics. However, this condition is not solely limited to the ACL deficient patients, but is a concern for patients with other diagnoses such as patellofemoral pain, meniscectomy, and lateral release. The clinician, therefore, is faced with the responsibility of using the appropriate exercise and modalities to meet the specific needs of these patients.
