Book excerpt: Given an increasingly complex web of financial pressures on providers, studies have examined how the hospitals' overall financial health affect different aspects of hospital operation. In our study, we analyze this issue focusing on hospital access and quality by introducing an important aspect of the financial incentives, soft budget constraints (SBC), that takes into account both hospital's current and past financial health as well as their expected financial outlook (i.e., whether there is a sponsoring organization to bail them out). We develop a conceptual framework of SBC by considering the resultant incentives on cost control and quality improvement innovations; and examine the effect of SBC on the following aspects of access and quality: safety net service survival and AMI mortality rates. We find that hospitals with softer budget constraints are less likely to shut down safety net services. In addition, hospitals with softer budget constraints appear to have better mortality outcomes, suggesting that the reduced incentive to engage in cost control innovation as the result of SBC outweighs the dampening effect of quality improvement innovation.

This book, Transformation in Action, attempts to fill the void that exists between planning, budgeting and implementation in the public health sector. Drawing on local and international research and policy experiments, it outlines an alternative system called the Performance Budgeting System (PBS). Unlike the current budgeting system, the PBS system integrates financial planning and management with strategic, operational, performance and quality management objectives to ensure that, within budget constraints, service delivery outputs have the greatest possible impact on health outcomes. In so doing, this system can contribute substantially to achieving the objectives of recent public sector reform initiatives and the ideals of the Constitution.

At the same time, new forms of managed care organizations emerged. Enrollment in preferred provider organizations increased from 12.2 million individuals in 1987 to 58 million in 1992, and enrollment in point-of-service plans increased from virtually none in 1987 to 2.3 million individuals in 1992. In addition, the percentage of traditional fee-for-service plans with some form of utilization review increased to 95 percent in 1990 from 41 percent in 1987.Measuring the effects of the changing delivery system on the costs and quality of health care services has been a difficult task, resulting in considerable disagreement as to whether or not costs have been affected. In a recent report, the Congressional Budget Office recognizes two new major findings. First, managed care can provide cost-effective health care at a level of quality comparable with the care typically provided by a fee-for-service plan. Second, independent practice associations can be as effective as group- or staff-model HMOs under certain conditions.In the future, we are likely to see a continued movement of Americans into managed care arrangements, an increase in the number of physicians forming networks, a reduction in the number of insurers, an increase in the number of employers joining coalitions to purchase health care services for their employees, and a health care system that is generally more concentrated and vertically integrated.

In this paper, we examine how public sector service providers respond to unexpected capacity constraints in the specific context of public health clinics. We exploit an exogenous reduction in public health clinic capacity to quantify nurses' trade-off between patients treated and time spent with each patient, which we treat as a proxy for a quality v. quantity decision. We provide evidence that these small and generally insignificant effects on nurse time favor public sector employees prioritizing quality of each interaction over clearing the patient queue.

Book excerpt: According to the Centers for Medicare and Medicaid Services, national healthcare expenditures reached $2.5 trillion in 2010. Federal, State, and local governments accounted for forty-three percent ($1 trillion) of these expenditures. The question arises as to whether the allocation of health care services in a region or sub-region can be optimized to reduce health care spending and improve health outcomes. A region that has a poor distribution of health care services may find that health outcomes are worse than a region with a good distribution of health care services. However, little research has been conducted to ascertain the effects of these decisions on health outcomes. This study explored how the allocation and amount of health care services affects the health outcome within the region. By better understanding the impact of the amount and distribution of health care services, policy makers will be in a position to better allocate funding for the location and distribution of health care services. This study used a spatial econometric approach coupled with a recently developed measurement, mortality amenable to health care, to answer the research question for the state of Florida. Data was obtained from a number of sources, primarily the Florida Department of Health and the Area Resource File. A gravity model was constructed to measure acute care, hospitals and rural clinics, as the level of spatial accessibility. This gravity model controlled for supply and demand interactions and allowed people to travel large distances to receive care. A travel impendence factor controlled for people being less likely to travel past numerous facilities when seeking care. Models were constructed for the entire state, urban areas, and rural areas to determine how the results varied. Age-adjusted overall mortality analyses and sensitivity analyses of gravity model measures were performed. In the overall state analyses, the concentration of primary care physicians and physician assistants were found to be statistically significantly associated with the age-adjusted overall mortality rate for the county. For both explanatory variables an increase in the health care service spatial accessibility was associated with a corresponding decrease in the age-adjusted overall mortality rate for the county. The urban county analyses revealed a positive association for the following health care service explanatory variables: primary care, nurse practitioner, and physician assistants. In the rural models, no explanatory variables were significant. These results suggest that spatial accessibility of health care services in the rural counties of Florida do not have an effect on the mortality amenable to health care rate or age-adjusted overall mortality rate. This study found that health care resources do not meaningfully influence health outcomes as measured by mortality amenable to health care. Similar to other studies, these results suggest that establishing the link between quantity and distribution of health care services to health outcomes (quality) is difficult. Based on these findings, programs aimed at improving spatial accessibility in Florida should be reviewed critically.
