Abstract: Prior clinical studies have documented that high blood pressure is one of the prominent risk factors leading to cerebrovascular disease and subsequent cognitive impairment. The present study aims to examine the relationship between blood pressure and brain and cognitive function in a community sample. This study recruited 41 elderly persons aged 60-70. Multiple imaging modalities were adopted to assess white matter microstructure, regional brain volume, and resting-state neural activity while a set of neuropsychological tests was used to assess cognitive function. With blood pressure measured at clinical interview, correlation and regression analyses were performed. Results showed reduced white matter integrity with increased systolic blood pressure in the splenium of the corpus callosum and inferior longitudinal fasciculus in the absence of change in brain volume or neural activity. Also, increased systolic blood pressure was found to be correlated with poorer cognitive performance in information processing speed. The results held significant after controlling age, sex, and education. These observations of the subclinical sample suggest that high blood pressure relates to subtle changes in the brain and cognitive deficits. Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.

Book excerpt: Hypertension represents one of the major modifiable health concerns in the U.S., with over one-third of adults classified as hypertensive, and another one-third meeting the classification for pre-hypertensive. Older adults are at the highest risk for hypertension. Although results have been mixed, a majority of the literature suggests that hypertension is associated with increased cognitive decline in older adults, particularly in frontally-mediated cognition such as executive functioning, processing speed, and attention. White matter hyperintensities (WMHs) and altered white matter microstructure are two consequences of hypertension that are thought to mediate the relationship between hypertension and cognitive aging. The goals of this study were to examine the impact of hypertension on two major white matter tracts that connect posterior regions of the brain with the frontal lobe and WMHs, to identify whether baseline blood pressure, baseline white matter tract integrity and baseline WMH volume contribute to frontally-mediated cognitive performance at a baseline visit, and to examine the longitudinal changes in white matter integrity and cognition in individuals who were hypertensive at baseline compared to baseline normotensives. Sixty older adults with both baseline and 3-year follow-up cognitive and imaging data were analyzed. Results indicated no significant relationships between blood pressure and white matter integrity or cognition at baseline or longitudinally. However, results suggested significant relationships between lower white matter integrity and worse cognitive performance on tests of executive functioning and processing speed. Although blood pressure did not significantly contribute to brain aging in this sample of healthy older adults, future work might identify other possible factors that could influence the relationship between aging and cognitive decline.

Book excerpt: Abstract: Cognitive impairments in memory and executive function are commonly observed with aging in humans. Age-related cognitive decline is exacerbated by hypertension, but it is less clear whether hypertension can lead to cognitive impairment in young adults. Rhesus monkeys evidence cognitive impairment with age that parallels that seen in humans and have been established as an appropriate animal model to examine the neurobiological basis of age-related cognitive decline. The development of a technique to produce chronic hypertension in the monkey has permitted the expansion of this model to evaluate the effects of hypertension in both young adult and aged monkeys. We used this model to examine whether hypertension alone is sufficient to cause impairment in learning and memory in the young adult.In order to gain a better understanding of the etiology of cognitive decline with hypertension or age, we then evaluated the integrity of cerebral microvasculature in hypertensive and in aged cognitively characterized rhesus monkeys using magnetic resonance imaging to measure cerebral blood volume (CBV) and image analysis on serial brain sections immunohistochemically stained for type IV collagen to quantify the microvasculature. Lastly, we utilized dynamic R2-star mapping to assess hippocampal vasoreactivity to carbon dioxide challenge from early adulthood to old age. The results demonstrated that induction of hypertension in young adult monkeys impaired relearning of a previously learned rule, but not visual recognition memory. Morphological measurement showed an increase in vascularity of the cingulum bundle with age but not in three other preselected areas, but this change did not correlate with cognitive impairments. MRI measurement of CBV in gray matter increased with duration of hypertension, but did not correlate with cognitive function. Functional vasoreactivity to carbon dioxide challenge was reduced in the right hippocampus, but not the left with age. The right hippocampal vasoreactivity, but not left, correlated with performance on two of three hippocampal dependent memory tasks in middle-aged animals. Together, the results of these studies suggest that hypertension in young adulthood can impair some aspects of cognition, and while both structural and functional microvascular alterations occur with aging or hypertension, only functional alterations correlate with cognitive function.

The effect of antihypertensive therapy on preventing or reducing the rate of cognitive decline is unclear. The studies in this thesis examined the association of elevated blood pressure in older adults on changes in specific cognitive domains and tested the hypothesis that antihypertensive treatment reduces the rate of cognitive decline in older adults with mild hypertension. 507 older individuals (70-89 years) were recruited from a general practice population (250 normotensives, and 257 hypertensives who participated in an international, placebo- controlled trial of candesartan). Cognition was assessed annually for 3-5 years using a comprehensive computerised assessment battery and tests of executive function. Analysis of cognitive function at baseline showed hypertensive subjects performed worse than normotensives across a range of tasks (Chapter 3). Exploratory factor analyses were conducted on the baseline data to derive composite scores used to characterise five domains of cognition and reduce the number of statistical comparisons (Chapter 4). Regression analyses were performed for each participant to calculate individual slopes of decline on the five domains, to provide a sensitive method of analysing repeated assessment data with differential length of follow-up. The primary analysis showed that candesartan-based therapy reduced cognitive decline associated with hypertension on Attention and Episodic Memory, with a trend for Speed of Cognition; effect sizes were small-to-moderate. There were no effects on Working Memory or Executive Function (Chapter 6). The normotensive subjects showed less cognitive decline than the hypertensives (Chapter 7). These data suggest that the rate of cognitive decline associated with hypertension in older adults may be reduced by blood-pressure-lowering. Analysing individual slopes of decline on empirically-derived domains of cognition provides a sensitive and feasible methodology for assessing the effects.
