Book excerpt: The aim of this study was to investigate the effects of an online, asynchronous mindful-based stress reduction (MBSR) intervention on symptoms of compassion fatigue for medical speech-language pathologists (SLPs). Although there is evidence that modified versions of MBSR programs, such as online-delivery models, can reduce compassion fatigue symptoms among various healthcare providers, the population of speech-language pathology has not been studied previously. In this ABA, single subject research design, a recruitment of seven participants completed and eight-week online, asynchronous mindfulness-based stress reduction program. Participants were full-time employed medical speech-language pathologists holding Certificate of Clinical Competence and state licensure and reported average to high levels of perceived stress. The online, asynchronous MBSR program consisted of one to two-hour weekly modules on a given topic of mindfulness that was accessed online at the leisure of the participant and included practice application activities. Weekly measurements of compassion fatigue symptoms were collected via the Professional Quality of Life Scale, version 5 (ProQOL-5). Results suggested an online, asynchronous MBSR program was effective at reducing compassion fatigue symptoms of both burnout and secondary traumatic stress for medical SLPs. These findings contribute to the body of literature investigating remediation strategies for various healthcare providers experiencing compassion fatigue.

Participants were licensed speech-language pathologists (SLPs) who worked full-time in healthcare and reported experiencing moderate levels of stress. Method: This investigation utilized a single-subject, A-B-A experimental design. Three participants completed this virtual, asynchronous investigation. Participants used a commercially available mobile application (app) to complete one short, guided meditation each day for 15 days. Each session was between 6-17 minutes long. The primary dependent variable, perceived stress, was measured after every third mindfulness session, using the 10-item Perceived Stress Scale (PSS-10). The secondary dependent variable, trait mindfulness, was measured before and after the investigation phase using the Mindful Attention and Awareness Scale (MAAS). Results: All three participants demonstrated statistically significant reductions in perceived stress compared to their baseline scores. When comparing pre- and post-test mindfulness scores, two participants demonstrated a positive change in trait mindfulness. Conclusion: A 15-day mindfulness-based intervention was effective in reducing perceived levels of stress in SLPs working in healthcare. These findings illustrate that brief MBIs might be a potential solution to the growing problem of chronic stress experienced by healthcare workers.

Research has not yet evaluated the program with medical speech-language pathologists (SLPs) or with an online delivery model. This investigation used virtual RBCS to address burnout in medical SLPs during the COVID-19 pandemic. Six medical SLPs participated in this ABA single-subject experimental design. RBCS was held on Zoom during the intervention phase twice a week for eight total sessions. Exercises included brief mindfulness meditations, emotional labeling, and guided reflective discussions on underlying source of stress. An adapted four-question version of the Oldenburg Burnout Inventory (OLBI) collected repeated measures of burnout twice a week across all study phases. Preintervention and postintervention data were collected with full OLBI assessments to further evaluate intervention effects. All participants showed a decrease in burnout at the end of the study, and five experienced statistically significant reductions in burnout due to the intervention. This investigation contributes to the growing body of evidence supporting RBCS for reducing healthcare burnout, offering a means to increase convenience and accessibility through virtual delivery.

Compassion fatigue and burnout are attributed to high turnover rates seen in the nursing field, estimated to reach 400,000 by 2025 (Brown et al., 2018). In addition to the financial burden incurred by healthcare systems, burnout can have detrimental health consequences for the nurses (Ameli et al., 2020; Lee et al., 2019; Pospos et al., 2018). Although most employers provide access to employee assistance programs and promote self-care, accessible and effective stress reduction strategies are rarely used in real-time. Addressing these issues through the implementation of strategies and supportive programs rooted in mindfulness has the potential to decrease burnout and improve retention (Brown et al., 2018). The need to address these issues inspires the following question: Does implementation of a mindfulness-based intervention in the workplace reduce the development of compassion fatigue and burnout among nurses and other healthcare professionals? Purpose and Aims. The purpose of this project was to implement and evaluate a mindfulness-based intervention for high-risk providers in a long-term healthcare facility. The aims of this project were: (a) to assess a baseline level of compassion satisfaction and compassion fatigue, which includes burnout and secondary traumatic stress, among the healthcare staff members; (b) to implement an effective and accessible mindfulness-based intervention; and (c) reduce the negative implications related to compassion fatigue and burnout. Implications for Practice. This project provided findings regarding the current levels of burnout and compassion fatigue experienced by patient-facing staff members and provided effective mindfulness-based strategies that can be utilized to actively mitigate stress and promote resiliency in the workplace. These findings should prompt future allocation of resources toward the reduction of burnout and compassion fatigue at the organizational level"--Abstract.

The independent variable was an 8- week online program adapted from Mindfulness-Based Cognitive Therapy. Dependent variables were Maslach Burnout Inventory: Human Service Survey and Freiburg Mindfulness Inventory. Of the 11 registered subjects, two completed the intervention and 1-month follow-up. Due to the high attrition rate, data collected cannot be generalized. Summary regarding lessons learned with suggestions for combining technology and mindfulness-based interventions in reducing nursing burnout are provided.

While the benefit of MBIs for psychopathology is well established, research on MBIs for physiological conditions is still in its infancy and possible mechanisms of actions are rarely discussed. However, integrating cost-effective MBIs into medical interventions might offer novel multidisciplinary treatment options for conditions with underlying psychophysical or chronic inflammatory states. Accordingly, possible mechanisms of action of MBIs that exert influence on physiological diseases are evaluated. Namely, the mindfulness stress buffering hypothesis, the mind-body genomics model and a reduction in negative affect reduction theory are discussed. The examination indicates that MBIs might work by reducing chronic inflammatory states, altering gene expression and impacting hormonal secretion. Further, the reduction of negative affect seems to be associated to various positive health outcomes. Preliminary research findings reveal moderate effect sizes in the treatment of conditions such as cardiovascular disease, psoriasis, diabetes and chronic pain. However, the results should be treated with caution as control-groups and follow-ups were often absent. Further, a clear definition of mindfulness is necessary for improved scientific operalization. Problematic is that only one MBI incorporates physical exercise, which might confound positive health outcomes. Nevertheless, the reviewed literature indicates preliminary support for the hypothesis, that MBIs may be suitable for complementary treatment of physical condition

Nonpharmacological adjuvants are being increasingly recommended as first-line adjunct treatments for the management of CNP conditions which is consistent with a multidisciplinary biopsychological approach to optimize pain management. Mindfulness-based stress reduction (MBSR) is one of the most commonly applied mindfulness interventions in the treatment of CNP and has been shown to have preferential improvements in pain and overall health-related quality of life. Although mindfulness interventions have shown promise in the treatment of chronic illness and disease, it has yet to be studied in breast cancer survivors living with CNP after medical optimization. Thus, the effect of MBSR on CNP in this highly heterogenous and complex clinical population is unknown. Biological markers are objective measures of biological systems that can be used to gain insight into the mechanisms through which psychological interventions exert their effects. The aim of the first study was to use an interdisciplinary treatment approach to CNP in breast cancer survivors by exploring the effects of MBSR on pain, and biological markers of stress, immune function and cellular aging among breast cancer survivors with CNP after guideline-based medical optimization. A second study was conducted to explore the potential mediating role of mindfulness, and the individual facets, in this change process. Methods: A double-blind randomized waitlist control trial was conducted using a sample of 98 breast cancer survivors diagnosed with CNP. Women were randomized to receive an 8-week MBSR intervention or waitlist control condition after undergoing medical optimization. Biological samples and self-report measures of pain, physical function and emotional function were collected at baseline, two-weeks post-intervention and at 3-month follow-up. Separate analyses were performed using intention-to-treat and per-protocol principles. Results: MBSR was delivered with fidelity and women randomized to MBSR demonstrated a significant increase in mindfulness relative to control. The MBSR intervention did not produce the anticipated stress reducing effects typically associated with mindfulness-based interventions. No differential changes in biomarker expression were observed between groups during the trial. Significant improvements in pain severity (intention-to-treat analysis) and pain-related interference (perprotocol analysis) were observed among women randomized to MBSR relative to control. Change in pain-related interference, but not pain severity, was found to be partially mediated by concomitant change in total mindfulness scores among women randomized to MBSR. Conclusions: Mindfulness interventions have gained significant traction in the treatment of chronic pain conditions. Despite this increase in popularity, our understanding of the association between mindfulness and pain is still evolving. Null findings can be used to help inform our understanding of adjuvant interventions in complex clinical patient populations, and identify what works for who, when and why. Mindfulness-based interventions are not homogenous, nor are they a panacea for disease and illness.

According to the National Nursing Workforce Survey there are approximately 4.2 million Registered Nurses (RNs)., approximately 950,000 Licensed Practical Nurses/ Licensed Vocational Nurses (LPNs/LVNs), and approximately 325,000 Nurse Practitioners (NPs). The nursing workforce challenges have included: increasing demands, substantial number of nurses retiring, and concerns about compensation and work-related stress. Coronavirus-19 exacerbated daily stressors for nurses due to long hours, understaffing, and potential burnout in healthcare personnel. There is evidence that nurses who participate in mindfulness activities may experience significant changes related to stress, anxiety, and mood; however more quantitative research studies are needed to investigate and substantiate the impact of mindfulness-based interventions. In addition, minimizing stress may enhance efforts for job satisfaction and retention. In conclusion this quantitative research intervention found positively statistically significant differences for five individual constructs of the Perceived Stress Scale (PSS) and the Self Compassion Scale (SC) and further supports the notion that mindfulness meditation may reduce work-related stress and improve self-compassion among those in the nursing population.

Book excerpt: Objective: This investigation aimed to evaluate the effectiveness of a mobile mindfulness application (app) on the perceived stress levels and academic resilience of undergraduate speech-language pathology (SLP) students. The participants were full-time students at a small, private, southern university, reporting moderate levels of perceived stress. Method: This investigation utilized a single-subject, A-B-A-B withdrawal design. Three participants completed this asynchronous, mobile app-based investigation. The participants used a commercially available app to complete a short mindfulness session for a total of 15 days. Sessions ranged from 6-17 minutes in duration. The dependent variable, perceived stress, was measured daily using the 10-item Perceived Stress Scale (PSS-10). Academic resilience was measured at the end of each phase using the Academic Resilience Scale-30. Results: Results were mixed for both perceived stress and academic resilience among participants. Statistical significance was found in some but not all participants in reducing stress as compared to baseline and withdrawal scores. Conclusion: Results were inconclusive as to whether or not a app-based mindfulness intervention is effective in reducing perceived stress and increasing academic resilience in undergraduate SLP students. These findings indicate that further research needs to be conducted but that app-based mindfulness may be a potential solution to help combat the notable problem of chronic stress reported by undergraduate speech-language pathology students.

Healthcare professionals are experiencing workplace stress and professional burnout in substantial numbers, which has resulted in multiple negative consequences including decreased productivity, poor staff retention, and increased organizational costs. While effective multi-pronged approaches to address stress and burnout among healthcare professionals account for the problem at all levels from individual to organizational, a growing evidence base supports the implementation of mindfulness-based interventions as one strategy to reduce stress and burnout and build resiliency. Objectives: The purpose of this DNP project was to design and pilot a quality improvement project of a mindfulness-based intervention to reduce stress and burnout among inpatient psychiatric clinical staff at a large, urban, academic medical center. This project had four objectives: 1) implement a brief mindfulness-based intervention that fits seamlessly within the current workflow structures in the medical center's inpatient psychiatric units; 2) evaluate the intervention's effectiveness in reducing symptoms of stress and burnout; 3) assess the viability of the intervention’s design and acceptance by clinical staff; and 4) provide recommendations for its sustainability as an ongoing program for the medical center's clinical staff. Methods: This project used a mixed methods design utilizing quantitative and qualitative data to reach its objectives. The mindfulness pilot program involved ten 12-minute audio sessions combining evidence-based didactic material and meditation practice. The sessions were administered free of charge to participants via a smartphone-based meditation app online and completed independently before or during work shifts at times chosen by participants. Pre- and post-intervention surveys utilized the Maslach Burnout Inventory (MBI) in addition to several Likert-scale survey questions, open-ended questions, and optional semi-structured interviews with which to collect insights from participants about both the variable being measured (stress and burnout) and the intervention used (mindfulness for healthcare professionals). Discussion: A small sample size limited the ability to determine effectiveness and feasibility. Post-intervention evaluative feedback indicated that participants experienced a reduction in stress but also had difficulty completing all ten sessions. Results and participant feedback both suggested that addressing systemic causes of burnout can improve participation and buy-in for individual-based interventions like the one in this project. Implications for Practice: Results from this project can help determine the extent to which a mindfulness program for clinical staff at a large urban hospital can provide a viable intervention to reduce stress and burnout, improve staff retention rates, and increase workplace satisfaction. Book excerpt: Medical students experience elevated stress, burnout, depression, suicidal ideation, and compassion fatigue. Previous research has demonstrated that a variety of mindfulness and self-compassion interventions can improve stress, burnout, and self-compassion in this population. However, these programs are not consistently implemented and, given the costly nature of program implementation, it is imperative to improve the effectiveness of interventions on psychological and behavioral health outcomes through systematic development. The specific aims were to: 1) assess feasibility and acceptability of the Self-compassion, Yoga, and Mindfulness for Burnout: Integrating Online Sessions and Interpersonal Support (SYMBIOSIS) program, and 2) evaluate preliminary psychological and behavioral health outcomes. A mixed-methods approach was utilized to investigate feasibility, acceptability, and effects of the program using validated measures and open-ended questions. Participants were 20 first- and second-year medical students at one U.S. mid-western medical college who participated between October 2020 to December 2020. Participants were enrolled in one of two groups for the virtual 8-week program via Webex. Participants completed surveys at pre, post, and four weeks after the intervention. They also completed weekly home practice assessments. Of the 19 participants who completed the program (95% retention rate), all participants attended 6 or more sessions. Repeated measures ANOVAs revealed that participants had significant improvements in burnout, self-compassion, perceived stress, and health promoting behaviors across time. These results were supported by qualitative themes of increased social support, wellness skills, and overall positive experiences. Findings suggest that the SYMBIOSIS program was feasible and acceptable to medical students, and improved psychological and behavioral health outcomes.
